    Compassionate staff; homey facility; concern

I had a largely excellent, 5-star experience - compassionate, family-like staff who went above and beyond, a clean, well-maintained, homey building, engaging activities and strong rehab/CNA instruction (Julie Ann Arron was exceptional). Staff treated residents like family and kept a positive, caring atmosphere. My main concern: I observed substance use among residents and learned complaints were filed with the state with little management action - that needs to be addressed.

    Summary review

Overall sentiment in the collected reviews is strongly positive, with the vast majority of comments highlighting the quality of personal care, the warmth of the staff, and the cleanliness and upkeep of the facility. Reviewers repeatedly describe staff as compassionate, attentive, and genuinely invested in residents’ wellbeing — often using phrases such as “treated like family,” “staff love residents,” and “goes above and beyond.” Nursing and therapy teams are described as knowledgeable and collaborative, and leadership is credited with fostering a culture of teamwork and pride. Multiple reviewers explicitly recommend Pickett Care and Rehabilitation Center and report positive outcomes from rehab services being available close to home.

Staff and training are a recurring strength across the reviews. Several summaries praise CNA training and instruction; one review specifically names Julie Ann Arron for excellent, clear teaching, indicating that education and staff development are valued at the facility. Staff morale and friendliness are emphasized frequently — reviewers note welcoming attitudes, smiling staff, and a family-oriented atmosphere. This carries over into daily resident experiences: activities are engaging, residents are described as being involved and happy, and staff-resident relationships are characterized as close and personally invested.

Facilities and amenities receive consistent praise. The building is described as clean, well-maintained, and comfortable — with multiple reviewers calling it the “cleanest nursing home.” Dining is positively noted (described as “awesome” or “great”), and rooms are described as warm and clean. Several reviews specifically call out the physical environment as home-like, adding to the sense of comfort and dignity for residents. Rehab services and therapy are highlighted as strengths, offering residents local, comprehensive care and contributing to positive recovery experiences.

Family support, end-of-life care, and community orientation are additional themes. Reviews mention compassionate end-of-life support and family-centered care, and several reviewers emphasize that the facility feels family-oriented and community-involved. This is reflected in comments about staff appreciation, community pride, and an atmosphere that puts residents’ best interests first. Some reviewers emphasize a positive organizational change or leadership presence (one review references a new Administrator, Leslie, and a welcome-home sentiment), suggesting responsiveness to leadership and a focus on continuous improvement.

The principal negative theme that appears across the review summaries centers on substance use among residents and related management concerns. A subset of reviews reports drug use among residents and states that complaints were filed with the state. These reviewers express concern about patient care quality and perceive management as not adequately addressing the issue. Although these reports are mentioned less often than the positive themes, they are significant because they raise safety and oversight concerns that reviewers felt compelled to escalate to external authorities. This negative thread is the main recurrent criticism and stands in contrast to the otherwise positive accounts of care quality and staff compassion.

In conclusion, Pickett Care and Rehabilitation Center is frequently described as a high-quality, compassionate, and clean facility with strong staff relationships, effective rehab services, and a home-like environment that supports resident engagement and family involvement. Training and staff competence (including positive mentions of specific instructors) are notable strengths. The most important area for improvement, based on these summaries, is addressing reported substance use among residents and ensuring transparent, effective management responses to safety and regulatory concerns. If those issues are being actively addressed by leadership, communicating corrective actions and oversight measures to families and the community would help align the facility’s strong positive reputation with confidence in safety and regulatory compliance.

About Pickett Care and Rehabilitation Center

Pickett Care and Rehabilitation Center sits in a quiet neighborhood of Byrdstown, Tennessee, close to Dale Hollow Lake, and offers 69 beds for seniors needing different levels of help, whether short-term respite care, rehabilitation, or long-term skilled nursing care, so you'll probably find it's small enough for staff to know the residents' needs but still has room for specialized help and activities. Managed by Signature Healthcare since 2007, this for-profit facility says it gives the warmth and comfort of a home and has a family-based organizational culture, focusing on treating everyone with dignity and respect. Staff follow a resident-centered approach, planning each person's care based on individual needs, wants, and even religious or cultural backgrounds, which shows up through their care navigation and quality of life programs, and they even offer interfaith spirituality activities for those interested. The center's membership in the Eden Alternative Registry and the Eden Registry's recognition means they try to do more than just standard care, moving toward person-directed support so people can keep their independence and sense of self as much as possible.

Pickett Care reports an average of 54 residents each day and employs nurses with a turnover rate lower than the state average, and those nurses are around for 3.81 hours per resident each day, which is almost at the Tennessee average, and that should help with getting more personal attention. The place is a member of the Tennessee Health Care Association and TNCAL and offers CNA training programs, so some staff may be learning on the job but others have experience and qualifications. Pickett also tries to keep patients active and involved with life enrichment programs, and their rehabilitation services support both physical and emotional health, making sure not to overlook folks' spiritual needs or cultural backgrounds.

Recent inspection history does show some trouble though-fifteen total deficiencies show up in their reports, including infection control, protecting residents' belongings, and keeping families informed, but state inspectors said no actual harm occurred, and management seems aware that changes need to be made to meet federal standards. The most recent standard inspection happened more than two years ago, even though these are usually supposed to happen every 9 to 15 months, and the record shows complaints, with two deficiencies found from a February 2025 complaint. Owners include several holding companies and private individuals, so the organization is not locally owned.

People looking for care at Pickett Care and Rehabilitation Center will find a small community with a range of services, a staff-to-resident ratio that helps with personal care, and a push towards resident-focused care plans, but inspection history and ongoing management mean it's wise to check for the latest reports and think carefully about the environment.
