    Compassionate clean responsive rehab recommendation

I was very pleased with River Ridge in Calallen - the staff were consistently friendly, compassionate, and professional, and nurses and therapists were helpful and responsive. The facility was immaculate, peaceful, and felt like home with good meals, activities, and strong family communication. They coordinated hospice and discharge needs smoothly; I would recommend River Ridge for rehab or longer-term care.

    Summary of reviews

Reviewer experiences of River Ridge Nursing & Rehabilitation are mixed, with a clear split between accounts of high-quality, compassionate care and accounts detailing operational shortcomings. Many families and patients praised individual caregivers — nurses, CNAs, and therapists — for being attentive, skilled, and empathetic. Positive descriptions emphasize effective PT/OT rehabilitation, coordinated discharge planning (including transportation and home-health arrangements), helpful hospice collaboration, and an activities program that includes painting, bingo, and exercise. Several reviewers highlighted a clean, well-maintained facility and a family-oriented atmosphere that supports trust and comfort.

At the same time, a substantial number of reviews describe recurring operational concerns. The most consistent themes are understaffing and high staff turnover, which reviewers link to slow responses to call lights, delays in toileting and bathing assistance, and uneven attention during busy periods. There are multiple mentions of medication-administration inconsistencies and gaps in clinical oversight, including missed or delayed medications and coordination problems between dietary service and staff administering insulin. Relatedly, some accounts indicate lapses in monitoring of medical equipment and treatment protocols.

Dining and dietary management elicit both praise and criticism: while some families found the food nearly excellent and appropriate, others experienced late meals or incorrect diet handling for residents with diabetes. Cleanliness reports are also inconsistent — many note an exceptionally clean, odor-free environment, while others describe significant sanitation concerns in particular rooms or units and raise pest-control issues in some areas. Additional operational weaknesses include inconsistent leadership communication, poor personal-property accountability, and weaknesses in incident escalation and family notification processes; these administrative gaps amplify the impact of frontline staffing problems when adverse events occur.

In summary, River Ridge demonstrates clear strengths in compassionate caregiving, rehabilitation services, activities programming, and—according to many reviewers—facility upkeep. However, persistent patterns around staffing levels, medication and dietary coordination, equipment monitoring, and inconsistent sanitation and property controls suggest areas needing management attention. Prospective residents and families should weigh the facility’s strong clinical and social offerings against these operational concerns, and consider visiting, asking specific questions about staffing ratios, medication-safety protocols, infection-control measures, and property safeguards before making placement decisions.

About River Ridge Nursing & Rehabilitation

River Ridge Nursing & Rehabilitation sits in a spot where folks can get both long-term skilled nursing care and help for recovery, and the place has a total of 120 certified beds, but on most days about 84 residents stay there, so it's a busy place, but not overcrowded. Staff give about 2.46 nurse hours per resident every day, which is below what the state average is-folks tend to expect closer to 3.4 nurse hours per person per day in Texas. The nurse turnover rate is a bit high at 57.4%, which runs above the state average of 51.8%. River Ridge works as part of Touchstone Communities, and it runs for profit under a limited liability company set-up.

The staff handle skilled nursing, long-term care, rehabilitation therapy, and memory care for older adults with Alzheimer's or other dementias. They have respite care for folks who need a short stay. There's help all day and night with nursing care, medication management, and all those extra things like wound care, pain control, and help with bathing or eating. A care team listens and works with each resident to make their own care plans, though inspections found some issues with care plan development and PASARR screenings. The place gets its food and snacks made with help from a registered dietician, offering meals to fit residents' dietary needs; folks can eat in the dining room or in their own rooms, and the kitchen tries to do home-style local cooking.

Amenities include both private and semi-private rooms, and a secured memory care unit for people who need extra protection. Rooms come with flat-screen cable TVs, and the facility has Wi-Fi, plus outdoor courtyards and covered patios so folks can sit outside if they want. There's private dining and living areas for gatherings. Activities, planned by an activity director, cover a lot-art, music, gardening, and visits out, all meant to help residents stay active and build friendships. Community engagement is encouraged, with social media options like Facebook and Instagram for families who want to keep in touch.

River Ridge underwent routine inspections, with one most recently on October 30, 2024, finding six deficiencies, one tied to infection control. The facility had 23 total deficiencies over time, with 2 related to infection issues, so people should check those inspection reports to get a full picture. While River Ridge offers services from skilled nursing and memory care to specialized therapy and post-acute care for recovery, it does face some challenges with nurse staffing and turnover, but the goal remains to provide individualized support and a comfortable environment for every resident.
