    Compassionate care, excellent rehabilitation outcomes

I placed my mom here and I'm very pleased. Denisha Cano, Sunni, Lettie, nurse Nick and social worker Jamie Raspberry were compassionate, professional and went above and beyond; outpatient OT/PT and in-house physical therapy were excellent and made my mom noticeably stronger and more independent. Staff communicated clearly, treated us with kindness, and made us feel valued.

    Summary of reviews

Live Oak Nursing and Rehabilitation Center elicits a mixed but actionable pattern of feedback. Strengths are concentrated in clinical rehabilitation and interpersonal care: several reviewers praised outpatient OT/PT services as highly effective, noting measurable strength and independence gains and personalized therapy plans. Individual clinicians and social-work staff received frequent positive mention by name; these comments describe professional, supportive interactions that contribute to good therapy outcomes and smoother discharge planning.

Nursing and caregiving interactions are also commonly described in positive terms. Reviewers used language like compassionate, kind, and patient when describing bedside staff and aides, and several named employees were singled out for helpfulness and clear guidance. Activities programming and on-site amenities were described as planned and welcoming, and dining received favorable remarks in some accounts.

Concurrently, reviewers identified operational weaknesses that may affect families during intake and day-to-day communication. Admissions and scheduling processes were described as inconsistent in timing and outcomes, with at least one account of an agreed admission not proceeding. Multiple comments highlight delays in staff responsiveness, inconsistent phone-answering and message-response processes, and gaps in family communication—issues that can complicate coordination of care and raise frustration for relatives.

A more serious concern raised by a reviewer involved medication-administration practices; this was described as an improper administration method in a single account. The language used in the reviews suggests a potential need for clearer medication-protocol oversight and auditing. More generally, reviewers described variability in care coordination during transitions (admission, therapy start, and transfer), indicating opportunities to standardize handoffs and information flow.

For prospective residents and families, the facility appears to offer strong rehabilitation services and a compassionate frontline staff that many families value. At the same time, those considering Live Oak should explicitly review admissions timelines, confirm the facility's medication-administration protocols, and ask how family communication and phone responsiveness are managed. An in-person visit and direct questions about therapy outcomes, staffing response expectations, and care-transition procedures would help to clarify whether the facility’s strengths align with the prospective resident’s priorities.
