    Warm, professional staff and care

I placed my parent at Grand Terrace and am very pleased - the staff are warm, professional, and go the extra mile. Nursing, respiratory and rehab care are excellent and available 24/7; the place is clean and homey, activities and food are good, and the admin team helped with insurance, so I would recommend it.

    Summary of reviews

The reviews of Grand Terrace Rehabilitation and Healthcare describe a facility with notable clinical strengths alongside operational inconsistencies. Clinically, the facility is repeatedly praised for its rehabilitation program (PT/OT/SLP), strong respiratory services including 24/7 respiratory-therapist availability, and the ability to provide bedside dialysis and other high-acuity interventions. Families and referral sources frequently highlight effective wound care, therapy-driven functional gains, and successful discharge planning that enabled many residents to return home.

Staff interactions are a prominent positive theme: many reviewers describe nurses, CNAs, therapists, and case managers as compassionate, attentive, and proactive. Several named staff and leaders were singled out for above-and-beyond service, and the business office and case-management teams received multiple mentions for helpful insurance and Medicaid assistance. The activities program is another strength — reviewers note an engaged activities director, varied programming, and a warm, home-like atmosphere that residents and families appreciate. Dining is generally characterized as home-style and accommodating of dietary needs.

Notwithstanding these strengths, a consistent pattern of operational concerns appears across reviews. Staffing variability — especially during nights and weekends — is linked to delayed responses to call lights, limited therapy availability on certain days, and perceptions of uneven attention. Equipment availability and maintenance gaps were described, including lift or mechanical-device shortcomings and limited wheelchair access, which can affect mobility and transfer safety. Communication gaps between administration and families, and inconsistencies between advertised services and what families experienced, also recur as practical concerns.

A subset of reviews raises more serious issues that warrant direct follow-up during any tour or placement discussion. These include concerns about medication-management processes and about property control or financial handling; such allegations should be investigated with facility leadership and, when appropriate, corroborated through documentation or regulatory channels. Families should also ask for specifics about staffing levels by shift, weekend therapy schedules, equipment maintenance protocols, and emergency-response procedures.

In summary, Grand Terrace appears to offer strong clinical capabilities — particularly in rehabilitation, respiratory therapy, and wound care — and many families report positive, caring interactions with staff and effective case management. Prospective residents and families should balance these clinical strengths against operational variability by asking targeted questions about staffing consistency, equipment readiness, medication oversight, family communication, and safeguards for resident belongings and finances before making placement decisions.

About Grand Terrace Rehabilitation and Healthcare

Grand Terrace Rehabilitation and Healthcare is a skilled nursing facility with 93 certified beds that gives both short-term rehabilitation and long-term care for people needing help between hospital and home or for those who are very frail and rely on nursing care around the clock. The staff includes skilled nurses who provide 24-hour care under in-house physician direction, and the facility brings in state-of-the-art therapy services right on the premises, using both hands-on and modern approaches to help residents regain strength and mobility at their own pace, with personalized care plans created together with residents, their families, and healthcare providers. The grounds are nicely landscaped, and you'll find both private and semi-private room options, along with well-appointed accommodations and a range of amenities like housekeeping, laundry services, and nutritious meal options from satisfying menus designed to meet different dietary needs. There's a variety of activities each month aimed at keeping residents active and engaged, so people who live here can enjoy a welcoming and comfortable environment while they focus on recovery or ongoing care. Grand Terrace is managed by Mcallen Care Associates, Inc., Soon Burnam, and Nefi Guzman, all since December 2023, and is directly owned by Fannin County Hospital Authority, while also being affiliated with The Ensign Group. Staff deliver 3.40 nurse hours per resident each day, but it's important to know that reports show a nurse turnover rate of 58.4%, and there have been 12 documented inspection deficiencies, including 2 related to infection, so the facility follows set policies to make sure areas are safe, clean, and comfortable for everyone and works to create effective treatment and care plans tailored to each person's unique needs. Grand Terrace holds a 4-star overall quality care rating from Medicare.gov which can be helpful for families looking at the different options available for their loved ones' needs.
