    Clean, compassionate staff; excellent rehab

I had a very positive experience - the facility was spotless and odor-free, staff from DON Cruz Leal, Betty and Amanda to the CNAs and therapists were compassionate, professional, and treated my mother like family. Excellent rehab/PT, nourishing meals and engaging activities kept her improving and happy; clear communication and dependable care made me comfortable, with only minor room for improvement.

    Summary of reviews

Review feedback for Windsor Nursing and Rehabilitation Center of San Diego is largely positive with repeated emphasis on clinical rehabilitation, compassionate caregiving, and a clean, welcoming physical environment. Many families and residents describe strong outcomes from therapy (physical, occupational, and speech), consistent wound-care expertise, and attentive nursing that promotes functional gains and dignity. The facility’s communal areas, festive decorations, and active programming contribute to a home‑like, small‑community atmosphere that residents and families find comforting.

Staffing and interpersonal care receive frequent praise: reviewers highlight compassionate nursing staff, engaged CNAs, and visible leadership in nursing management. Several comments single out rehabilitation clinicians and wound-care personnel for effective, patient‑centered therapy. Communication with families is often described as timely and informative, and administrative staff are credited with assistance around insurance and admission paperwork. End‑of‑life care and dementia programming are noted as thoughtful and respectful when those services were required.

Dining and activities are strengths in many accounts. The food is commonly described as appealing and nourishing, and activities programming (group exercises, arts and crafts, themed months, Bingo and other social offerings) appears robust and well attended. Housekeeping and facility maintenance are repeatedly described as attentive, producing clean rooms and common areas with no persistent odor concerns.

Notwithstanding the overall positive tenor, a minority of accounts identify operational weaknesses that prospective families should probe. These include medication‑management concerns (including at least one instance involving a missed diuretic dose with clinical consequences), inconsistent adherence to diet orders, and episodic lapses in clinical oversight or care coordination. Some reviewers also describe variability in staff conduct or leadership tone across shifts and occasional delays in family notification. These comments point to unevenness in day‑to‑day operational consistency rather than a uniform deficiency.

Recommendation for visitors and decision makers: verify current protocols for medication reconciliation and handoffs, ask how dietary restrictions are managed and how dietitian involvement is ensured, and request examples of family‑notification procedures for clinical changes. Also inquire about staffing patterns by shift and the facility’s processes for escalation and clinical oversight. Overall, Windsor demonstrates clear strengths in rehabilitation, cleanliness, activities, and compassionate caregiving, with a few operational areas where additional safeguards and consistency would reduce risk and improve family confidence.
