    Excellent rehab, systemic safety failures

I had a mixed, stressful stay: some therapists, CNAs and aides were exceptional and the rehab was excellent, but systemic problems are real. I saw medication negligence (missed meds), rude/unresponsive nursing staff, slow or no response to call buttons, safety lapses (falls, oxygen mishandling), poor communication from administration, billing nightmares, and spotty cleanliness/food. Management was often unhelpful; you need to closely watch care if you choose this place.

    Summary review

Overview: Reviews for Reunion Plaza Senior Care And Rehabilitation Center present a strongly polarized picture: many families and residents praise staff, therapy, and the physical environment, while a substantial number report serious care failures, safety incidents, and administrative problems. The most consistent positive themes are high-quality rehabilitation services, compassionate individual staff members, a welcoming and homelike atmosphere in many units, and engaging activities. The most serious negative themes are medication errors, unresponsiveness and understaffing, medical neglect leading to severe outcomes, and inconsistent management responsiveness.

Care quality and clinical safety: Clinical experiences vary widely. Numerous reviewers report excellent nursing and therapy care—therapists and rehabilitation programs are repeatedly singled out as effective, and several families describe strong recoveries and positive rehab outcomes. At the same time, there are numerous reports of medication mishandling (missed doses, medications not given, incomplete medication lists), delayed lab results, inadequate wound care that led to infection, oxygen therapy mishandling that created safety concerns, and incidents so severe some families link them to deaths or transitions to hospice. Several accounts describe staff needing physician intervention to correct lapses. These mixed reports point to pockets of very good clinical practice coexisting with dangerous lapses in others.

Staffing, responsiveness and professionalism: Staff competency and demeanor are another clear split. Many reviews praise CNAs, nurses, and specific administrators for being compassionate, responsive, and going above and beyond—some staff members are named repeatedly for exemplary service. However, a significant number of reviews cite rude or unprofessional nursing staff, aides with poor attitudes, and staff who are non-responsive to call lights or resident needs. Night and weekend coverage and certain hallways are mentioned as understaffed. Reviewers link some poor behaviors to high turnover, corporate/state staffing constraints, or overburdened employees. The result is inconsistent day-to-day experiences that appear dependent on unit, shift, or which staff are on duty.

Facilities, cleanliness and equipment: Physical aspects are likewise mixed. Many reviewers praise the facility's appearance—inviting landscaping, seasonal decorations, spacious common areas, and a clean or newer therapy wing. There are numerous positive comments about private rooms for rehab and comfortable visiting areas. Conversely, there are multiple reports of poor cleanliness in some areas (urine smell, sun-bleached/frayed carpeting, broken equipment in hallways), housekeeping lapses (sheets not changed, empty bedpans left out), and reports of non-working life-support equipment or broken items. These contradictions suggest variability in maintenance and housekeeping across units or over time.

Dining and activities: Dining and activities receive both praise and criticism. Several reviewers describe outstanding, hot meals and a full activity calendar (Bingo, live music, church services, seasonal parties), plus amenities like a beauty shop and coffee access. Yet other accounts complain of cold, salty, or burned food, small portions, lack of snacks, and limited dining space. Activity levels are noted as reduced on weekends or nights by some families, which contributes to perceptions of residents being isolated at times.

Administration, communication, and billing: Administration and communication show a stark divergence across reports. Some administrators and business office staff are lauded for being timely, knowledgeable, and compassionate, with specific employees receiving praise for going above and beyond. Conversely, many reviews describe unresponsive management, dismissive head nurses, poor communication with families and case workers, and unprepared case coordination that delays discharges or equipment provision. Financially, several families report billing issues, alleged overcharges, Medicare or coverage delays, and even threats of collections. These administrative problems compound clinical concerns and erode trust for a number of reviewers.

Safety and serious incident reports: A small but significant subset of reviews describe severe safety failures: residents left unattended or cold, privacy breaches (exposed patients), falls without timely assistance, and at least one account referencing life-threatening delays and alleged death due to lack of care. These reports are particularly alarming and are a recurrent reason families moved loved ones out or strongly advised against placing someone at the facility.

Patterns and variability: A central theme is inconsistency. Positive and negative reports often come from the same facility—many reviewers explicitly praise the staff, then note a decline over time or contrast excellent units/staff with problematic ones. Staffing shortages, turnover, and differing experiences by shift/hour appear to drive much of that variability. Several reviews attribute declining quality to corporate/state pressures and staffing mandates, suggesting systemic issues rather than isolated personnel problems.

Takeaways for families and prospective residents: - Strengths to consider: strong rehab services, many compassionate individual caregivers, a welcoming environment in several units, a rich activity program, and good amenities (beauty shop, visitation areas). For short-term rehab stays, multiple reviewers recommend the facility based on therapy outcomes. Specific staff and leadership have earned community trust in several accounts. - Red flags to monitor: medication administration practices, responsiveness to call lights (especially nights/weekends), wound and oxygen care procedures, housekeeping and equipment condition, and clarity on billing and insurance handling. Repeated reports of serious incidents and administrative unresponsiveness warrant direct, proactive questioning during any tour or admission process.

Recommendations when evaluating this facility (based on recurring review themes): Ask about current staffing ratios on the specific unit and shift, verify medication administration protocols and how missed doses are handled, check wound and oxygen therapy protocols, inquire about meal examples and weekend activity schedules, confirm billing practices and insurance coordination, and meet or speak with unit leadership (DON/administrator) who will be responsible for the resident. Given the mixed reports, prospective residents and families should tour the unit where their loved one would be housed, request recent inspection or deficiency information, talk to multiple families if possible, and agree on a clear communication plan with named contacts.

Overall, Reunion Plaza Senior Care And Rehabilitation Center receives both strong endorsements and severe criticisms. Many families experienced excellent rehabilitation outcomes and compassionate individualized care, while others report dangerous lapses, neglect, and administrative failures. The reviews suggest that experiences are highly dependent on unit, shift, and specific staff—making careful, individualized evaluation and ongoing advocacy essential for anyone considering placement here.

People get help with meals if needed, choosing from menu options with their "Dining Your Way" program, which feels more like a restaurant than an ordinary cafeteria. Reunion Plaza also offers specialized services like podiatry, optometry, and dentistry on-site, along with pharmacy and lab services, so you won't need to travel far for those things, and residents can visit the on-site beauty and barber salon too. They have a LifeWorks wellness program with activities to keep minds and bodies active, and they offer rehabilitation-both for short stays and as outpatient therapy-including physical, speech, and occupational therapy. Oxygen therapy, restorative nursing, IV, enteral, and parenteral therapies are available, and staff help seniors manage respiratory issues as well as other medical conditions, along with providing hospice when needed. The center provides social services and family involvement programs to support emotional health, and there's a long-term care ombudsman to help resolve complaints. Run as a non-profit, the center's been managed by Fannin County Hospital Authority since 2017 and under Pf Reunion Snf Ops, LLC since 2021, plus it's tied to Stonegate Senior Living, while staff work to create a welcoming place that feels like home. Nurse staffing hours average 2.87 per resident each day, which is lower than the state average, and they've seen a higher nurse turnover than the Texas average-about 68.4%. There have been 85 deficiencies according to inspection reports, including some serious issues involving infection control, resident safety, and accident hazards, with a few matters called immediate jeopardy to residents, so families may want to check for more details about those reports and see how concerns get handled. Reunion Plaza has experience with rehabilitation and memory care, so they'll try to match care and programs to what each senior needs, aiming to offer medical attention, social support, and daily help in one senior care center where staff try to be friendly and ready to answer questions for families or those considering a stay.
