    Mixed care inconsistent sometimes unsafe

    I've had mixed, often conflicting experiences. Many places were clean, had friendly, caring staff, and delivered excellent rehab or hospice at times. Too often staffing was inconsistent-long waits, missed meds, poor communication, and gaps in therapy. I witnessed unsafe handling, improper transfers/discharges, cramped rooms, infections and rehospitalizations. Management felt disorganized at times, so quality seemed uneven. I wouldn't trust a facility without personally touring, checking staffing/therapy schedules, and confirming communication practices.
