    Excellent rehab, inconsistent nursing care

I have mixed feelings. The grounds and neighborhood are beautiful, rooms are large and bright with good storage, the facility is generally clean, meals are excellent, and the PT/OT rehab teams were outstanding - my family saw real improvement. Many staff were friendly and caring (Salina, Rachel, Heather, Alana and Martha stood out), but nursing and communication were inconsistent: missed monitoring (including unmonitored urine output that led to hospitalization), medication and blood-sugar errors, unanswered calls/mailbox, short-staffing, and occasional rude or uncaring interactions. Because of those safety and communication issues, I'd consider this good for rehab but would not trust it for long-term care.

    Summary review

Overall sentiment in these reviews is mixed but consistent in several key themes. The facility is frequently praised for its rehabilitation services: physical therapy and occupational therapy teams receive strong, repeated positive mentions and many families describe good functional outcomes and positive short-term rehab experiences. Several reviewers explicitly call the therapy teams "amazing," and note that the facility can be a "huge blessing" for rehab. Meals and dining are commonly mentioned as a positive ("delicious meals"), and many reviews describe clean, bright, spacious rooms with large windows and good closet space. The campus and neighborhood are also favorably described — mature trees, flowers, birds, and a park-like setting contribute to a pleasant environment.

Staffing perceptions are mixed and highly inconsistent. Multiple reviewers single out individual staff members (Salina, Rachel, Heather, Alana, Martha Hunt) as having no complaints and being attentive; other comments call staff friendly, courteous, and caring, and say residents feel at ease. Activity programming and follow-up care by specific staff are also highlighted as positives. However, there is a prominent and recurring concern about inconsistent staff performance: several reviewers report some employees as uncaring or engaging in uncalled-for conduct. This inconsistency extends to nursing and clinical staff, where more serious issues are described.

Clinical-care concerns are among the most significant negative patterns. Reviews describe insufficient nursing coverage and short-staffing, weak onboarding or training for staff, medication administration errors, and inadequate monitoring of conditions such as blood sugar. There is at least one report that unmonitored urine output led to hospitalization — an indication of potentially serious oversight failures. These kinds of lapses have led some families to state they would not recommend the facility for long-term care, though they may find it acceptable for short-term rehabilitation. Trust issues are explicitly mentioned; several reviewers warn that while some staff are excellent, nursing care in particular should be verified and monitored by family advocates.

Communication and responsiveness are additional areas of concern. Multiple reviewers mention unanswered phone calls, an unresponsive mailbox, and long stretches without updates on a resident's condition (one report cites 14 days). These communication gaps exacerbate worries about clinical care and create frustration for families trying to stay informed. Cleanliness is mostly described positively, but isolated reports of dirty bathrooms, trash on floors, and a noisy air conditioner indicate some variability in environmental maintenance.

Management appears to have taken some positive steps according to a few reviewers who note professional staff who follow through and implement changes. Pricing is mentioned as roughly in line with peers, which may make the facility competitively positioned for those seeking community-based rehab. Overall recommendation from reviewers tends to split along care-type lines: the facility is generally recommended for short-term rehab and therapy where PT/OT teams are strong; it is viewed more cautiously for long-term skilled nursing because of reported clinical and communication shortcomings. Prospective residents and families should verify current nursing staffing levels, medication administration and monitoring protocols, and communication practices. They may also wish to identify specific staff members praised in reviews and ask management about training, supervision, and corrective actions taken regarding the clinical lapses described.

About Riverside Lifelong Health & Rehabilitation Smithfield

Riverside Lifelong Health & Rehabilitation Smithfield, part of Riverside Health System, is a long-term care community that offers a mix of assisted living, skilled nursing, memory care, rehabilitation, and independent living options, so there are places for seniors who need different help, and somebody needing support with bathing, dressing, medication, or transfers can get that, and there are nurses on staff with care teams focused on helping folks stay independent as long as possible and get appropriate assistance when it's needed. The building has 106 beds in all, with 34 beds set up for assisted living and 72 for skilled nursing care, and most rooms are fully furnished, pretty cozy, and come with private bathrooms, air-conditioning, cable, and WiFi, which is something folks like, and they provide meals restaurant-style, with help for special diets like diabetes.

For people with memory loss, the community's memory care wing has specialized programming for those with Alzheimer's and related conditions, as well as for folks with mild cognitive impairment, and there are memory-enhancing activities to help keep minds active. There's adult day services, respite care for families who need a break, and outpatient physical therapy, plus home health care, hospice, palliative, and in-home care available for those living in the area. The Martha W Goodson Center and several wellness and fitness spaces help residents stay active, and there's a computer center, gaming room, library, garden, fitness room, and organized group activities, and scheduled transportation is available for those who need to get to appointments or go shopping.

Residents get daily housekeeping and linen service, help with moving in, 24-hour call systems, and supervision around the clock. There's a focus on healthy living with programs on elder abuse prevention, technology training, fall prevention, and a resource library filled with books, guides, educational articles, and helpful links, so families can find the support and information they need. There's a directory of over 26,000 programs and community events, legal and estate planning recommendations, and other senior resources. Staff turnover for nurses is higher than average, but nursing hours per resident per day are a bit above what the state usually has, and the health care team is present for up to 16 hours each day.

There have been some issues noted in the past, such as a failure to post some resident rights information and some infection control problems, but measures have since been put in place to control infections. The community is a part of the Virginia Health Care Association and is recognized for quality care by the state. With a household model and a focus on lifelong health, Riverside Lifelong Health & Rehabilitation Smithfield tries to give seniors access to as much support, social engagement, and health services as possible, whether someone comes for short-term recovery after an injury or plans to stay for long-term care.
