    Persistent neglect, unsafe and filthy

    I would not place a loved one here. I found persistent urine odors, dirty rooms, soiled bedding, bedsores and missed baths; staff were often slow or unavailable, ignored call lights, delayed meds and emergency care, and communication with administration was unresponsive. The facility is understaffed, unsafe and poorly maintained (broken equipment, outdated beds, hazards), and rehab staff sometimes misrepresented patients' abilities. A few caregivers and therapists were kind and worked hard, but those positives did not offset the neglect and safety concerns - strongly not recommended.
