    Nice facility but safety concerns

I appreciated the clean facility, friendly/dedicated staff, good food and activities, but my experience was mixed and worrying. I heard residents moaning for help, calls were often unanswered or hung up on, doors and privacy were ignored, belongings went missing, and I dealt with rude maintenance - safety and communication were inconsistent. Despite some caring nurses, my father won't return.

    Summary review

Overall sentiment across these review summaries is mixed, with a notable split between strong praise for frontline caregiving and several serious operational and safety concerns raised by other reviewers. Multiple accounts describe staff as caring, friendly, respectful, and experienced, and many families and residents reported positive day-to-day experiences such as attentive feeding assistance, informative staff communication, enjoyable meals, and varied activity programming. At the same time, a number of reviews raise red flags about responsiveness, cleanliness, personal property security, and resident safety that cannot be ignored.

Care quality and staffing: Several reviewers emphasize that direct-care staff treat residents with dignity and respect, are attentive (including hands-on help with feeding), and appear dedicated and diligent. Comments such as "residents well loved and cared for," "friendly and caring staff," and that it is a "great place to work and be" indicate that many staff members provide compassionate, person-centered care. However, this positive picture is uneven: other reviews report neglectful situations (residents moaning for help, being helpless or scared) and state that family members are worried about their loved ones' welfare. This contrast points to inconsistent care experiences, suggesting variability in staff performance, supervision, or staffing levels across shifts or units.

Facilities, cleanliness, and environment: There are also mixed reports about the facility environment. Several reviewers remark the facility is friendly, clean, and free of odors, with amenities like a patio area and organized outings (for example, state fair trips). Activities programming is frequently praised — Rainbow Room events, bingo, crafts, games, and movies are mentioned as regular offerings that support socialization and engagement. Conversely, other reviews describe dirty conditions, terrible smells, and a rude maintenance person, indicating that environmental standards may fluctuate. The presence of both positive and negative accounts suggests that some areas or shifts maintain cleanliness and an engaging environment while others fall short.

Safety, privacy, and operations: A recurring set of operational concerns comes through strongly. Several reviewers note privacy issues (units not separated), a crowded nurses' station, and doors being left open — circumstances that can contribute to resident confusion, loss of privacy, and potential elopement or injury. Complaints about poor responsiveness to call systems (calls going unanswered or calls being hung up on) and unclear or unhelpful call handling are particularly significant because they directly affect resident safety. Missing personal belongings (Harley Davidson shirts, hoodie, hat) and reports of rude maintenance staff add to worries about personal property management and staff professionalism.

Communication and management: While some families praised informative communication from staff, multiple summaries report ongoing communication breakdowns and a sense that calls are not handled properly. The repetition of "hung up on me" and "calls not answered" indicates systemic issues with phone or nurse-call responsiveness or with staff training in family communication. The combination of operational, privacy, and responsiveness problems suggests management and oversight areas that likely need attention to ensure consistent standards of care and safety.

Patterns and implications: The reviews reveal a clear pattern of polarized experiences — many firsthand accounts praise compassionate caregivers and enriching activities, while a distinct subset cites serious safety, cleanliness, and responsiveness problems. This split may indicate inconsistent staffing, variable training, or uneven supervision across shifts or units. For prospective families or oversight bodies, these patterns recommend targeted follow-up: verify call response protocols and average response times, check procedures for handling residents who are confused or prone to wandering, review laundry/property tracking and inventory procedures, and ask management about maintenance staff conduct and environmental cleaning schedules. A tour during different times of day and conversations with multiple families or staff types could help reveal whether the positive experiences are consistent or if the negative reports are concentrated in specific circumstances.

In summary, Seneca Trail Healthcare Center receives meaningful praise for staff compassion, resident engagement activities, and some aspects of facility life and dining. At the same time, repeated reports of poor call responsiveness, privacy and safety lapses, missing belongings, and inconsistent cleanliness point to operational weaknesses that can materially affect resident well-being. The facility appears capable of delivering very good care in many respects, but the variability in reviews warrants careful inquiry and monitoring by families and regulators to ensure consistent, safe, and reliable care for all residents.

    About Seneca Trail Healthcare Center

Seneca Trail Healthcare Center sits at 1115 Maplewood Avenue in Lewisburg, West Virginia, and has room for up to 80 residents, usually about 77 people live there each day, and it offers skilled nursing, memory care, hospice, assisted living, and rehabilitation services for seniors. This place holds both SNF and NF dual certification, so it can take care of people with different needs, and it takes Medicare, Medicaid, and private pay, making it possible for people from many backgrounds to come here, and the management has been handled by Health Care Facility Management, LLC since April 2023, all under the legal name GB WV Maplewood Leasing Co., LLC, and it's affiliated with Communicare Health. The facility runs as a licensed and certified nursing home, and gets regular inspections for life safety, infection control, nutrition, and dietary practices, so it's subject to surveys for complaints and recertification. In the most recent standard inspection from September 10, 2024, there were 9 deficiencies found, especially related to infection control and handling of garbage, and over time, a total of 25 deficiencies have been reported in different inspection reports, so while the place keeps up with federal and state requirements, issues have been found in some areas. Nurse turnover here is about 34.7%, and nursing staff average 3.77 nurse hours per resident every day, which means staffing stays consistent with nursing home standards.

They have skilled nursing and intermediate care for those who are very frail or depend on others, as well as short-term rehabilitation for people moving between hospital and home, including post-surgical care and help with complicated medical needs. The staff includes medical experts who regularly check and update each person's care plan, meeting with residents and their families to make things personal and give thorough healthcare and social support, and they also offer physical, speech, and occupational therapy right there in the building. There's a PACE center called InnovAge Roanoke Valley running on site, and a physician-run clinic gives more medical help if it's needed. For people who have dementia or Alzheimer's, the center offers special support, including activities like crafts, bingo, pet therapy, and holiday parties to help with emotional and mental health, so residents can feel engaged and cared for. There are specialized services, hospital-level equipment, and private spaces focused on comfort, and the facility emphasizes a supportive environment for everyone who lives there. In addition, the team gives long-term care, palliative care, and hospice care for seniors with serious illnesses, working with residents and families to make each day as comfortable and meaningful as possible. Seneca Trail serves seniors from the Lewisburg area and around West Virginia, helping people who've reached a stage where they need more hands-on care and support to keep their best quality of life.
