    Compassionate care and excellent amenities

My parent stayed here and I was consistently impressed by the compassionate, attentive staff-nurses, therapists, and caregivers were professional and caring. The facility is clean and well maintained with engaging activities, good rehab services, a shuttle, pet-friendly outdoor spaces, and tasty meals; I felt comfortable entrusting my loved one to their care and would recommend it.

    Summary of reviews

Overall impression: St. Clare Meadows presents as a largely well-maintained senior care center with many features families value — notably a highly regarded therapy program, a generally compassionate nursing team, active engagement opportunities, and onsite amenities. Reviewers consistently highlight the facility's strengths in rehabilitation, attentive bedside care, and a clean physical environment with sizable common areas, an outdoor patio, pet-friendly policies, a shuttle, and services such as an on-site hair salon.

Care and staff: Clinical and frontline staff are frequently described as experienced, personable, and attentive. Therapy and rehabilitation services receive particularly strong praise, with several accounts calling the therapy department excellent. At the same time, a recurring operational issue is staffing levels: the facility appears to operate under persistent understaffing and high workload pressure, which reviewers link to slower assistance responses and difficulty meeting some resident needs. Front-desk interactions and many nurse contacts are described positively, with staff remembered by name and treated as caring and professional.

Dining and personal services: Dining is noted as a strength in many accounts — meals are often described as fresh and tasty with varied options. However, there are operational inconsistencies: continuity of meal service and reliable breakfast delivery are sometimes lacking, and accommodating special or therapeutic diets has been difficult in some instances. Laundry and personal-item handling have been identified as an area of concern, with reports of misplacement and mix-ups indicating weaknesses in garment tracking and collection procedures.

Communication, management, and clinical follow-up: Several reviewers identified gaps in communication between the facility, attending physicians, and families, which can complicate care coordination and discharge planning. Related concerns include pressure or unclear communication around billing and insurance and limited flexibility around discharge or visitation in certain circumstances. There are also mentions of inconsistent follow-up and communication after clinical incidents, including concerns that arose around a resident death; these comments point to opportunities for stronger incident-response protocols and family outreach.

Activities and facilities: Activity programming and community involvement are positive features, with daily activities, community events, and physical spaces that support socialization. The physical plant is described as clean and updated in parts, with new sections and comfortable waiting and common areas. Amenities such as shuttle service, pet access, outdoor seating, and a salon contribute to quality-of-life advantages for residents.

What to verify when touring: Prospective residents and families should confirm current staffing ratios and average response times to assistance calls, processes for accommodating therapeutic diets, laundry and personal-item tracking procedures, billing and insurance communication practices, and the facility's approach to visitation and clinical-incident communication. The facility's strong rehabilitation program and caring staff are clear positives, but operational inconsistencies in staffing, meal continuity, laundry, and family/physician communication are important to evaluate during a visit.

About SSM Health St. Clare Meadows Care Center

SSM Health St. Clare Meadows Care Center sits at 1414 Jefferson St. in Baraboo, WI and offers a range of care options like assisted living, skilled nursing, memory care at Pine Villa, and rehabilitation services like physical, occupational, and speech therapy, and you know, there are 102 certified beds available and nurses on staff 24/7, so help's always on hand. The center's backed by SSM Health, which means you get access to expert guidance on things like long-term care, family caregiving, and even info on long-term care insurance, which can be handy if you're trying to plan ahead or help someone you care for. They've got policies in place for things like pressure ulcer care, making sure residents don't develop sores, and strict rules about physical restraints and bed rails, using them only if a doctor says they're needed, though it's important to know there's been an infection-related deficiency reported in their inspections, so things aren't perfect, but they do get monitored for safety and quality of care. The nurse turnover rate is at 34%, and each resident gets about 4 nurse hours a day, which keeps things running but isn't the very highest, just steady enough that the basics get covered. You'll find a warm atmosphere with spacious common areas, pretty grounds, and places like an indoor atrium, main street shops, a beauty salon, and outdoor patios, so it's not all medical-there's a bit of life and comfort to it. Meals cater to different needs, with options for no-sugar or low-sodium, and rooms can have private baths, air conditioning, cable TV, Wi-Fi, and easy access for people with disabilities, so the basics are met for comfort and convenience. They also offer spiritual support, social activities, and recreation, and families can use respite care for short stays, or look into hospice when it's necessary, which helps cover the whole range from lighter assistance to heavy care. There's a focus on supporting people with memory challenges, especially through Pine Villa Memory Care, and the campus even has Pine Glen Senior Apartments and The Pines Assisted Living for folks who want a different level of independence. Reviewers tend to rate it about 4.3 out of 5 from 48 reviews, showing that most families find the care and services reliable, even if there's room for improvement, and like most places of its kind, they're licensed and follow rules on safety and inspections, so you know care standards have to be met.
