    Kind staff, clean facility, recommended

I'm very pleased with this facility - staff are kind, attentive, and treat residents like family, following doctors' orders and going out of their way to help. The building and dining rooms are clean and attractive, meals are good, and there are plenty of activities (live music, a cozy bistro, and daily programs); my in-law felt safe, loved, and peaceful here, so I highly recommend it.

    Summary of reviews

Overview: Reviews present a divided picture of Jefferson Memory Care. Many families and some staff describe a warm, well-maintained environment with caring, attentive caregivers and a robust activity schedule. At the same time, there are repeated accounts pointing to operational and management weaknesses that contribute to a mixed overall experience.

Care and staff: Care quality appears inconsistent. Numerous comments praise caregivers as compassionate, calm, and family-oriented; families described timely assistance, respectful interactions, and peaceful end-of-life care in some cases. Conversely, there are recurring accounts of delays in attending to resident needs and variability in staff responsiveness and competency. These patterns suggest that resident experience depends strongly on individual caregivers and on staffing stability during a given shift.

Dining: Physical dining spaces are generally well regarded—dining rooms are described as attractive and clean, and the facility hosts social meal settings. However, there are concerns about meal quality and continuity: reviewers cite inconsistent food quality and instances of missed or insufficient meals. Prospective families should ask about meal planning, portioning, and procedures used to ensure consistent meal service.

Activities and facility features: Activity programming is a clear strength, with 4–6 daily activities noted, regular live music, sensory-support materials, and accessible outdoor courtyards. The facility’s interiors and common spaces, including a bistro area, are frequently described as pleasant and well cared for, which supports social engagement and resident comfort.

Management, staffing, and operations: Management and workplace culture are the most polarized areas. Some remarks commend a caring administration and supportive staff culture, while other accounts describe punitive management practices, poor communication, and problematic personnel decisions. Operational patterns that emerge include staffing and scheduling instability, pressure on staff to secure coverage, and concerns about payroll or unpaid training. There are also statements raising policy-compliance and labor-practice concerns; these characterize serious workplace issues that families and prospective employees may want to verify independently.

Safety and compliance concerns: Several comments raise issues tied to staff-welfare and safety policies (for example, expectations around driving or shift coverage) and sanitation or pest-control concerns in specific areas. These items point to gaps in operational oversight rather than a consistent facility-wide condition, but they merit due diligence, particularly when safety or hygiene are priorities for a prospective resident.

Notable patterns and guidance for families: The dominant pattern in the reviews is variability: strong programmatic elements (activities, pleasant spaces, and many engaged caregivers) coexist with operational weaknesses (staffing instability, management inconsistency, and sporadic service issues). Prospective residents and families should ask targeted questions about current staffing ratios, turnover rates, meal-service procedures, recent inspection or complaint history, pest-control protocols, training and payroll practices, and how the facility handles urgent clinical needs and end-of-life care. Arranging multiple visits at different times of day can help assess day-to-day consistency in staffing, meals, and activities.

    About Jefferson Memory Care

Jefferson Memory Care sits in Jefferson, Wisconsin, and has been taking care of older adults since August 1, 2014, focusing mainly on memory care and assisted living, and you'll find it at 414 Co Rd Y, in a building that tries to feel more like a home than an institution, with secured areas and special features like alarmed exits, personal safety pendants, and private courtyards to make sure people who might wander stay safe. The place is run by a team led by Mr. Ryan D Burr, and it operates as a Limited Liability Company, offering both double and single rooms with private bathrooms, kitchenettes, cable TV, telephones, and Wi-Fi, so residents can have some of their home comforts along with safety.

Though the business holds an A+ rating with the BBB, it's not accredited, and with about 22 reviews, it's got a score that averages 3.6, which is neither at the bottom nor the top, but right in-between, and a person can expect a respectful, secure setting especially for folks living with dementia or other memory impairments. The staff receive regular training in Alzheimer's and dementia care, and they focus on each person's needs, providing help with daily things like getting dressed, transfers, bathing, medication, laundry, and meals-those meals come from a chef and can be adjusted for allergies and diabetes. Nurses are available by phone 24 hours a day, and there's always someone in the building to check in on residents.

Residents can fill their days with activities like fitness programs, gardening, walking outside, movie nights, music, art, and games, and there's a library and wellness rooms too; for people who might get overwhelmed, staff can gently guide them back to activities in a way that helps them feel included instead of lost. The place offers transportation, parking options, and help moving in, so families get support from the beginning, and staff involve families in care decisions, keeping everybody informed.

Jefferson Memory Care has a community focus, aiming to help people stay connected, do things they enjoy or maybe try some new hobbies, and keep up their independence and dignity for as long as possible. The facility has designed their rooms and shared spaces to lower stress and boost both safety and social life, believing that everyday purpose and simple comforts help everyone feel better. If someone needs personalized care for dementia or memory issues, Jefferson Memory Care provides structured support, a secure space, and routines intended to make life easier and give some peace of mind to both residents and their loved ones.
