    Friendly staff, serious care failures

I have mixed feelings. The staff are incredibly kind, polite and helpful, and the facility is clean, pleasant smelling, welcoming and comfortable with a convenient one-story layout. But I witnessed horrible wound and feeding-tube care that led to rehospitalization, unclean commodes and urine odor, theft of a $150 gift card, aging cramped rooms, spotty aide competence and skimpy meals/activities. Despite the friendly team, I cannot recommend this place.

    About Milford Center

Milford Center sits at 700 Marvel Rd in Milford, Delaware, and runs as a nursing home with both short-term stays and longer care periods, which means folks can come for a little while or for as long as they need. The place is part of Genesis Healthcare and has 136 certified beds, usually caring for about 100 residents each day, so it's not too crowded but it's busy. Staff include a medical director, an admissions director, and a rehabilitation director, which helps keep things organized. There are both private and semi-private rooms along with a dining room for restaurant-style meals and some nice gardens outside for folks to enjoy, so there are spaces to eat and relax. The center offers skilled nursing, medical, and rehabilitation care, plus it handles dementia and Alzheimer's care, with a memory support unit and programs that the Alzheimer's Association recognizes for dementia care education, so people with memory needs have focused help. Services reach across a wide range, with care for older adults, people needing rehabilitation after hospital stays, and those who require long-term nursing, so it's set up for many levels of help. The place also has home health options, hospice care, assisted and independent living, memory care, and support for veterans as a We Honor Veterans partner, and offers help with daily living, mental health, diabetes, oncology, maternal and child care, pediatric and AIDS care, infusion therapy, enterostomal therapy, and case management. Staff provide about 3.98 nurse hours per resident per day, which is lower than the state average, and the nurse turnover rate is high. Milford Center has had some issues with quality and safety, appearing as a Special Focus Facility Candidate because of serious past problems, including not always providing enough food and fluids or following food safety standards, and having a few infection control deficiencies, which are worth knowing. There's a fitness room for exercise and all the usual services you'd expect like respite care, long-term care, memory support, and post-hospital rehab, and some programs offer case management and referral help. The center accepts Medicare, Medicaid, and most private insurances, so paying can be flexible. People looking for skilled nursing, short or extended stays, memory care, and extra services like psychiatric or home health programs can find those here, though it helps to look closely at recent inspection reports since the facility has a past with care and safety citations.
