Eger Health Care & Rehabilitation Center sits at 140 Meisner Ave in Staten Island, and it's a nonprofit skilled nursing facility with 378 beds, where they care for people needing short-term rehab, long-term or chronic care, memory care, Huntington's Disease care, or end-of-life help, and you'll find a main building and an assisted living building spread across the grounds with a big Eger sign up on the hill and buildings that hold folks' rooms, a café, a gym, and extras like a beauty salon, barber shop, a piano lounge, meditation room, internet lounge, laundry services, security, even a playground and outdoor patios with gardens for some fresh air, and you know, there are rooms for folks to gather or enjoy a meal or relax when they want a change of scene. Eger Healthcare's been around for years under Eger Lutheran Homes and Services, following Lutheran values, offering care whether or not you can pay, and it's clear from the records that they aim for solid care because Medicare and Medicaid approved all the beds, and there's been no penalties or fines in the past three years. The center isn't in a hospital or a Continuing Care Retirement Community; it runs on its own with a board of directors, and there's a Resident Council if anyone wants to bring up concerns or talk with staff. If you look at the CMS ratings, they got 5 stars overall-same for health inspections and quality of resident care-and as of June 18, 2018, they had no violations, but staffing levels come in at 3 stars with about 48 minutes of registered nurse time per resident a day, which isn't the highest but seems to get the job done, and there's physical therapy for those who need it, averaging about 7 minutes per day per resident. People here get specialized care, especially the short-stay folks after a hospital stay who want to get back on their feet-with a re-hospitalization rate at 24.1%-and the numbers say only 1.1% got antipsychotic medication, with pressure ulcers at 0.2% and high vaccination rates for flu and pneumonia among these residents. Long-stay residents see strong scores too-five stars for their category-with 1.31 hospitalizations and 0.31 emergency room visits per 1,000 days, 1.3% falls with major injury, a pressure ulcer rate of 5.2% in high-risk residents, a urinary tract infection rate of 2.0%, and 1.0% with a catheter, which aren't zero but are better than many places, and yes, some folks do decline in movement or need more help over time (about 18.1% and 8.5%, respectively), and 4.1% report moderate or severe pain, but those numbers are tracked and reported. Nurses, aides, and therapists work together, with safety and infection protocols in place, extra PPE training, respiratory etiquette, early testing, all handled under the eye of an Infection Prevention and Control Nurse and a medical director, so they're watching out for illness. Programs at Eger focus on patient care days, and the care team pays attention to physical, mental, and emotional needs. Specialized services include home care, children and adult hospital care for those with special needs, Huntington's, memory problems, and especially end-of-life comfort, offering people and families some peace in hard times. With the amenities, services, and stable staffing, Eger Healthcare doesn't try to be anything it's not; it's a large nonprofit skilled nursing and rehab center that aims to give solid, compassionate care to seniors and people with special needs in Staten Island.

Beautiful grounds; inconsistent, unsafe care
I liked the beautiful, wooded grounds, clean rooms, active calendar and several kind, professional nurses and therapists. However chronic understaffing caused slow responses, safety and medical lapses (falls, missed meds, even an unnecessary catheter/UTI) and inconsistent, sometimes uncaring treatment - my overall impression is mixed and I'd be cautious.
