    Good therapy but understaffed, inconsistent

I moved my mom into this large facility and found mixed results. The PT/OT staff were caring and knowledgeable and some aides were polite and helpful, but the place is clearly understaffed-desks unmanned at times and nurses stretched thin. Rooms ranged from clean, hospital-like to shared and junky; building cleanliness and the food were inconsistent. Activities were hard to find and scheduling unreliable, and communication had delays (they did apologize), so weigh the good therapy care against staffing and cleanliness issues.

About Willow Creek Nursing And Rehabilitation Center

Willow Creek Nursing And Rehabilitation Center sits over on Wayne Memorial Drive in Goldsboro, North Carolina, and has room for about 200 residents, though lately the daily count comes closer to 160, and you'll find they focus on both short-term rehab and longer-term nursing care, which means there's always staff around to help folks with everyday needs like getting up, washing, dressing, getting meals, and making sure medications are sorted, and if someone needs more, they've got wound care, treatments for strokes or chronic illness, and specialized therapies like physical, occupational, or speech (even outpatient if someone is just visiting for a while), and for things like amputation and learning to use a prosthesis, the team's used to working on that too, with therapists and nurses on the floor day and night, as nurse staffing averages about 3.82 hours per resident per day, which is a bit more than the state average. While a lot of the staff have come and gone over the years, as seen in the nurse turnover rate of 54.5% (a touch above what's typical for North Carolina), Principle Long Term Care, Inc. has owned and run the place since 2011, with Lynn Hood joining as manager in 2017, and they've clocked up about 25 deficiencies in inspections lately, including infection issues and not always following the rules about keeping families and doctors in the loop when things happen to residents, plus sometimes lacking in the way they tailor care plans to each person, with the last big inspection in December 2024 noting seven deficiencies and resulting in a fine. The center puts a lot into keeping the place safe, such as regular cleanings, air filters, mask requirements when called for, and checks for illness, while also sticking to CDC advice and working with experts for infection control. They try to create a warm community by running events, encouraging social engagement, and offering amenities that aim for residents' comfort and a sense of belonging, and their approach is to see each person as an individual, with plans developed by a whole team looking at someone's physical, emotional, social, and spiritual well-being, and they keep an eye on daily achievements, independence, and keeping spirits up. They carry a relatively low online rating, about 1.9 out of 5 based on the latest 37 reviews. Care here covers ongoing help for those staying long-term, as well as extra support for folks just out of the hospital or recovering from surgery, and the focus remains on helping each person achieve the best level of health possible before heading home, if that's an option, all with an emphasis on respect and trying to preserve dignity even as needs change.
