    Loving attentive staff, excellent rehab

I was nervous at arrival, but the loving, attentive staff - nurses, CNAs, therapists, rehab team, housekeeping and medical staff - made my stay wonderful. Rehab was amazing, I recovered and went home happy and healthy, and the recreation team kept me active with daily programs, trips and socialization. The team honored my wishes in family meetings, treated me with dignity and respect, and I would definitely go there again and highly recommend it.

    Summary review

Overall sentiment across the review summaries is strongly positive, with repeated emphasis on high-quality clinical care, excellent rehabilitation services, and a compassionate, attentive staff. Multiple reviews single out the nursing staff, therapists, CNAs, housekeeping, and medical personnel as delivering excellent, respectful care. Reviewers report successful rehabilitation outcomes, with several patients recovering and returning home “happy and healthy,” and some indicating they would choose the facility again. The consistent use of terms such as “wonderful,” “amazing,” “devotion,” and “grateful” underlines a prevailing sense of satisfaction among residents and families.

Clinical care and rehab: The facility’s rehabilitation team and therapists are frequently praised for effective therapy and recovery support. Reviewers specifically note the rehab team and therapists as “amazing” and attribute positive discharge outcomes to their programs. Nursing care is repeatedly described as caring and attentive, with staff noted for honoring patient wishes and preserving dignity. Several reviews mention coordinated care across clinical roles (nursing, therapy, CNAs, medical staff), suggesting good interdisciplinary teamwork focused on recovery and patient-centered goals.

Staff attitude, communication, and family involvement: A major theme is the staff’s helpfulness and responsiveness. Reviewers highlight compassionate interactions, family meetings, and active involvement in addressing healthcare issues, which indicates effective communication with families and respect for resident preferences. The presence of family meetings and mentions of honoring wishes and dignity suggest the facility prioritizes person-centered care and keeps families informed and involved in decision-making.

Activities, socialization, and services: Beyond medical care, reviewers praise programming and services that enhance resident quality of life. The facility offers a full-day program, daily activities, organized trips, and transportation services, which promote socialization and engagement. The recreation department receives specific commendation for creating an active environment that residents enjoy. These amenities are linked in the reviews to improved morale and satisfaction, reinforcing that the center supports both clinical recovery and social well-being.

Operations and areas for attention: While overwhelmingly positive, a couple of reviews note concerns that bear watching. Some new residents felt nervous upon arrival, which is common during transitions to post-acute care and could indicate an opportunity to strengthen orientation or admission support. Another recurring note is that some staff appeared overwhelmed; this may point to occasional staffing pressures or workload challenges even though staff dedication and compassion remain evident. Reviewers did not provide substantial detail on facility physical conditions, dining, or administrative issues, so conclusions about those domains are limited by the available summaries.

Overall assessment: The collected reviews paint Split Rock Rehabilitation and Health Care Center as a facility with strong clinical capabilities, particularly in nursing and rehabilitation, and a warm, family-oriented culture. The active programming and supportive services add meaningful social and practical value. The primary actionable observations are to continue supporting staff to prevent burnout and to enhance admissions/orientation processes to reduce initial resident anxiety. Given the frequency of positive endorsements and reports of good recoveries, the facility appears to be a reliable choice for post-acute rehab and skilled nursing care based on these reviewer impressions.

About Split Rock Rehabilition And Health Care Center

Split Rock Rehab & Health Care Center works as a skilled nursing facility that focuses on helping people recover and manage their health with both short-term and long-term care services, and you'll find that they set up care programs based on each person's needs, paying close attention to different types of health issues. The center provides skilled nursing care, and if someone needs dialysis, they offer onsite dialysis with advanced NxStage equipment through a team-up with Dialyze Direct, and they've also got treatments for specialized needs like trach dialysis and ventilator dialysis, which comes in handy for people who need tracheostomy care or ongoing ventilator support. Split Rock also puts effort into wound care, rehabilitation therapies, Alzheimer's and dementia care, plus hospice and palliative care for those at different stages of illness or recovery. The place stands out by helping families get involved in planning care and encourages them to stay in touch, whether that's through visits, phone calls, or sending letters-showing that they try to keep a family feeling even when you're dealing with tough times. They have a group of staff who know their jobs and try to be friendly, which helps make the place more comfortable. Folks at Split Rock can take part in recreation and activities made to fit their needs, and they can count on management to help with care planning and making sense of different health challenges. You see, they run an adult daycare program that includes transportation to and from home, with meals and snacks, meaning people can get help during the day but still spend evenings at home. People recovering from major illness, surgery, or injury can use the short-term rehabilitation programs designed to help them get better and go home sooner, while those needing more help can stay longer in a homelike setting. After residents leave, the staff often keeps in touch through their home care service to make sure everything goes smoothly. With a 4-star CMS rating, Split Rock tries to meet several needs with skilled care, rehab therapies, and a steady support system for both health management and daily living.
