    Dangerously understaffed and poorly managed

I don't recommend this place. After an ownership change the facility became dangerously understaffed and administratively-driven-calls were ignored, responses slow, a bedsore and broken nebulizer went unaddressed, and several caring staff were fired or clearly underpaid. A few nurses and aides were genuinely compassionate and the rehab/therapy, food, bakery and activities were very good, but the old building, maintenance and cleanliness problems (leaking A/C, dirty linens, messy rooms) plus rude management forced me to move my loved one to a safer facility.

    Summary review

The reviews for Quantum Rehabilitation And Nursing present a strongly mixed and polarized picture, with several consistent positive features alongside multiple, recurring serious concerns. On the positive side, many reviewers note warm, personalized interactions from specific caregivers — staff who learn residents' names, read stories, and organize social activities such as movie nights, bingo, and patio parties. The facility reportedly offers robust rehabilitation resources (physical, occupational, and swallowing therapy), an on-site rehab center downstairs, and some reviewers praise the rehab outcomes. Dining and amenities receive favorable mentions as well: reviewers cite very good food with menu choices, an in-house bakery, and complimentary coffee (including sugar-free options). A subset of reviews also describe the facility as clean and well kept, and some families specifically call out professional, polite nurses and aides who provide helpful care and therapy-focused services.

However, a distinct and troubling pattern appears in many other reviews that describe substantial declines in care and safety, particularly following a change in ownership or management. Multiple reviewers report chronic understaffing, staff being underpaid, high staff turnover, and the firing of long-time caregivers. These staffing problems are linked to specific negative outcomes: slow or ignored calls from families, unresponsiveness from management (including allegations of rudeness from a director), and concrete safety incidents such as bedsores and a broken nebulizer that reportedly went unnoticed. Several reviews go further to allege abuse, neglect, or unsafe conditions and explicitly advise avoidance of the facility or call for it to be closed. These are severe claims that point to systemic issues rather than isolated incidents.

Facility and maintenance issues are another frequent theme. While some describe the environment as clean, others report dirty rooms, stained or unclean dressers, soiled bedsheets and clothes, leaking A/C units, and generally old or broken building infrastructure. Complaints about cheap beds and mattresses and inadequate phone lines for communication also appear. Taken together with staffing shortages and underpaid employees, these observations suggest problems with resourcing and operational oversight that affect day-to-day resident comfort and safety.

Management and administration emerge as a central axis of the conflicting reviews. Several accounts attribute the decline in care quality to a recent ownership change and the involvement of a private management company, describing actions such as dismissing valued staff and prioritizing cost-cutting. Reported consequences include an administrator resignation, ongoing management turnover, and perceptions of administrative greed. Nevertheless, some reviews still report pockets of genuine compassion among remaining staff and positive experiences with individual caregivers and therapy teams.

Overall sentiment is divided: there are clear strengths in programming, rehab services, and certain compassionate staff members, but there are also consistent, serious concerns about understaffing, management practices, safety, and cleanliness. The most common and pressing themes across reviews are (1) a decline in consistency and safety following management changes, (2) staffing shortages and low morale leading to neglectful behaviors or poor responsiveness, and (3) a mixed experience dependent on which staff members are on duty. For prospective residents or families, the reviews suggest that outcomes may vary widely and that current systemic issues — especially those involving safety, neglect, and management instability — warrant careful inquiry. Visitors should ask specific, recent questions about staffing levels, turnover, incident reporting, maintenance practices, and how the facility has addressed any documented safety concerns before making placement decisions.

    About Quantum Rehabilitation And Nursing

Quantum Rehabilitation And Nursing sits at 63 Oakcrest Ave, Middle Island, NY, and most folks know it for its strong focus on neurological and post-acute rehabilitation, with services that help adults, especially those working through brain injuries, stroke, or neurological conditions, work on recovery in an environment that stays clean and follows strict infection control, always going by guidelines from the CDC and government. The place has skilled nursing services available around the clock, seven days a week, for both short- and long-term care, plus 24/7 admission if someone's situation changes suddenly, and they've even made it so you can do a virtual tour if you can't make it in person.

There's a Brain Injury Specialty Program on site as part of their inpatient rehab, and people there work with a team that has experts in occupational therapy, speech therapy, and physical therapy, so you'll see residents getting individual skills training, sometimes in a state-of-the-art gym filled with equipment you don't often see elsewhere, all while knowing the place handled accreditation back in 2023 and keeps up with regular standards and reviews. Folks who have Alzheimer's or other forms of dementia can stay in the secure memory care section, which cuts down on wandering and confusion, and everyone can use the community amenities, like transportation and parking, and they'll give rides when needed, plus remote chats are easy with options for video calls like Facetime, so family can keep in touch.

Quantum is run by Paragon Management, and you'll usually hear it's listed with senior care referral services such as A Place for Mom, since they focus a lot on connecting seniors and their families with the right fit for care. The rooms are described as beautiful and the shared spaces aim for an elegant touch, but most people talk more about how the rehab services stand out, particularly for neurological problems and COVID recovery, since the staff takes their roles seriously and the programs are designed to help folks work toward getting back some independence, whether it's needing memory support, physical therapy, or recovering skills after an injury or illness. The Medicare 30-day coverage window is available, and when it comes to getting certified or meeting standards, the team helps with steps and survey prep, so the place stays up to date in field reviews. Accommodations are available for residents, making it possible for adults with physical or mental health needs to get whatever assistance they require, day or night.
