    Neglectful, unsanitary, dangerous nursing home

I chose this place because it's close and the staff seemed friendly, but my experience was terrible. Routine neglect - missed meds and IVs, delayed orders, wounds not changed, infrequent showers, raw food and scarce snacks - led to dehydration, aspiration pneumonia, a UTI and an emergency hospital transfer; weekend staffing and emergency response were poor. The building is small, expensive, outdated, smells at the entrance, has mice in the halls and lax security; roommate hygiene was an ongoing problem. There are activities and some caring staff, but overall the care was unacceptable and I cannot recommend it.

    Summary review

Overall sentiment in these reviews is mixed to strongly negative, with a number of reviewers noting some positive aspects but multiple consistent and serious concerns. Positively, several reviewers reported friendly or nice staff and described care as at least satisfactory in some instances. The facility offers private rooms and is conveniently located near family, which was a decisive factor for some families planning to place or keep relatives at Walnut Cove. A few families expressed trust in familiar staff members and even provided positive endorsements, and some reviewers said the facility initially seemed good on admission.

However, the dominant themes across the summaries are problems with basic care, maintenance, and clinical responsiveness. Multiple reviewers described failures in everyday personal care: residents not bathed or showered on schedule, infrequent showers, wounds not changed in a timely way, and scarce snacks. Several reviewers reported medication shortages or delays in medication orders. These issues were sometimes linked to more serious clinical outcomes — dehydration, urinary tract infections, aspiration pneumonia, renal failure, and other complications were reported, and reviewers described transfers to hospital when the facility failed to provide timely treatment. There are specific allegations that necessary interventions (for example, IV administration) were not performed and that staff or management were slow or unresponsive to escalating medical needs.

Facility upkeep and environment are additional areas of concern. Reviewers mention small rooms, broken-down or outdated furniture, general poor maintenance, unpleasant smells at the entrance, and poor ventilation. Sanitation problems were highlighted explicitly, including reports of mice in hallways and floors that are not kept clean. Roommate hygiene problems were also cited, implying issues with managing resident needs and communal living standards. Together, these complaints suggest that both environmental cleaning and facility maintenance fall short of expectations for many reviewers.

Staffing, conduct, and management responsiveness are recurring issues. Although some staff are described as nice and trustworthy, other reviewers reported staff yelling at patients, neglectful care, and lack of security. Several comments point to problems occurring or worsening on weekends, implying potential staffing shortages or reduced oversight during those times. Reviewers repeatedly described slow responses to emergencies and medical declines, with at least one account linking delayed emergency response and poor care to a death. These are serious allegations and indicate patterns of inconsistency in both day-to-day caregiving and clinical oversight.

Dining and activities receive less attention but are still noted. Food quality problems (including reports of raw food) and scarce snacks were mentioned. Activities are present but reportedly underused by residents in some cases — meaning programming exists but may not be engaging or sufficiently supported. Cost is another consistent concern: several reviewers stated the facility is more expensive than alternatives, which is particularly troubling given the number and severity of complaints about care, cleanliness, and responsiveness.

In summary, while Walnut Cove Health And Rehabilitation Center offers some positives — private rooms, convenient location, and occasionally attentive staff that families appreciate — the reviews collectively identify multiple systemic issues. The most significant problems are inconsistent and sometimes neglectful clinical care (medication delays, untreated wounds, dehydration, infections), serious outcomes requiring hospital transfers, environmental cleanliness and pest problems, maintenance deficiencies, and variable staff behavior and responsiveness. These pattern-based concerns, especially those tied to patient safety and clinical treatment delays, overshadow the limited positives in the reviews and led many reviewers to advise against the facility or to express strong dissatisfaction.

About Walnut Cove Health And Rehabilitation Center

Walnut Cove Health And Rehabilitation Center sits at 511 Windmill Street in Walnut Cove, North Carolina and has been open since 1974. The place has ninety certified beds, with six private rooms and forty-two semi-private rooms, and can handle up to ninety residents-sometimes more, sometimes less, but there's usually about eighty-one folks staying at a time. It offers long-term care, skilled nursing, and post-acute care around the clock for people who need help with health and rehabilitation. Folks get occupational therapy, physical therapy, speech therapy, and care for medically complex conditions, including support for Alzheimer's and dementia. There's a nurse on duty at all hours, though nurse hours per resident are 2.71 a day, which is lower than the North Carolina average of 3.8, and nurse turnover in the last year was 45.9%, a bit under the state average. The center's been a part of Consulate Management Company III, LLC since December 2021 and is owned by Centennial Healthcare Holding Co. LLC, making it a for-profit business that's linked to Consulate Health Care and other related care groups. Life at the center includes planned activities like garden time, craft making, movies, outings, group events, and visits from performers. Pastoral support and a beauty/barber shop are there, too, and there's a gym for rehab, plus spaces outdoors for fresh air. The center's got programs to help folks go back home safely, tailor-made care plans, and social activities aimed at helping people keep their dignity and live well. The staff tries to take care of the whole person-body, mind, and spirit. Couples can room together if needed. Over the last few years, inspectors reported both care and safety issues-there were deficiencies found with matters like accident hazards and supervision, including a fine of $8,827 from a July 2024 inspection and another fine of $16,801 from December 2024. The center had complaints and an infection-control report in the last thirty-six months. Walnut Cove Health And Rehabilitation Center calls itself an Ambassadors of Care facility and aims for personalized, compassionate care, but like all care centers, it's had its share of challenges along the way.
