    Caring staff unsafe and disorganized

I'm grateful some staff were kind, patient and attentive-supportive therapy, helpful window visits, good food, and they helped get my loved one home. However the place felt understaffed and disorganized: residents were left unsupervised, nurse calls often delayed, communication and info-sharing were poor, and I witnessed unsafe/ethical lapses (including inappropriate transport). Mixed feelings-caring people, but serious safety and communication problems that make me wary to fully recommend.

    Summary review

Overall impression: The reviews for Creswell Health And Rehabilitation Center are mixed and polarized. Several families describe caring, professional staff, effective therapy services, good food, and helpful discharge support — enough that those reviewers would recommend the facility. At the same time, other reviewers report very serious concerns about safety, staffing, and clinical oversight that lead them to characterize the facility as a poor or even dangerous rehab option. The result is a clear pattern of inconsistent experiences where positive interpersonal care and rehabilitation outcomes coexist with notable operational and safety issues.

Care quality and therapy services: Multiple reviewers praise the clinical and rehabilitative aspects of the facility. Positive comments highlight attentive care for specific clinical needs (for example, careful attention to swallowing and appropriate modification of foods), supportive physical and occupational therapy, and staff who helped with the transition back home. These accounts suggest the facility can and does provide effective, patient-centered rehabilitation and discharge planning for many residents.

Staff behavior and professionalism: Staff behavior is among the most frequently discussed topics and is reported in strongly contrasting terms. Several reviewers call staff kind, patient, honest, and professional, and they appreciated regular communication and the team’s focus on doing the right thing. Conversely, other reviewers describe staff-related problems such as finger-pointing, lack of teamwork, and reports that aides ignored or sedated patients and did not ensure adequate nutrition. This split indicates variability in staff performance and possibly differences between shifts or teams.

Safety, staffing, and operational concerns: The negative reviews raise significant safety and operational concerns that should not be overlooked. Specific incidents cited include a resident left unsupervised, a transport that occurred without necessary oxygen, and extremely slow responses to a nurse call (one report of more than an hour). Understaffing is repeatedly mentioned and appears linked to delayed responses, poor monitoring, and breakdowns in basic care. There are also complaints about inadequate inter-staff communication and failures to pass along critical information between caregivers. Together these issues point to systemic problems (staffing levels, supervision, and communication processes) that can produce real safety risks.

Communication and family experience: Family communication is another area of mixed feedback. Some families report honest, regular updates and a professional team that kept them informed. Others experienced difficulty reaching staff by phone or received poor or incomplete information, contributing to frustration and mistrust. The presence of both strong and weak communication experiences suggests inconsistency in how information is conveyed to families and how responsive different staff members are to family inquiries.

Facilities, visits, and dining: Practical aspects such as visitation and dining received positive notes: window visits were allowed and appreciated, and food quality was described as fantastic by some reviewers. The specific mention of food texture modification for swallowing (e.g., blending items like a cinnamon roll) demonstrates attention to individual dietary needs in certain cases.

Overall pattern and recommendations for consideration: The overall tenor of these reviews is one of variability — the facility demonstrates clear strengths in therapy services, some compassionate staff, effective discharge assistance, and meal quality. However, recurring and serious negatives (safety lapses, understaffing, slow call responses, transport without oxygen, and inconsistent communication and teamwork) create significant concerns for potential residents and families. For families considering Creswell Health And Rehabilitation Center, the reviews suggest verifying current staffing levels, asking specific questions about supervision and transfer protocols, confirming how the facility handles oxygen-dependent transports and emergency responses, and seeking references from recent families. For the facility, addressing staffing shortages, strengthening handoff and documentation practices, ensuring consistent family communication, and reviewing transport and safety protocols would reduce the most serious concerns reflected in these reviews.
