    Clean facility attentive compassionate staff

I'm very pleased with the clean, well-kept facility and the warm, welcoming staff. My loved one had a private room, good meals, attentive CNAs and nurses, and thoughtful spiritual support - special thanks to Stephanie, Lacey, and Gina McCullough for their leadership and responsiveness. Staff respected privacy, offered compassionate care and warm hugs; overall a very good experience that I'd recommend.

    Summary of reviews

Reviewer feedback for Valley Falls Terrace is markedly mixed, with distinct patterns of both praised strengths and significant operational concerns. Many families and residents describe compassionate, attentive care from CNAs and nursing staff, clean rooms and common areas, and the availability of private rooms. Admissions and discharge processes are sometimes characterized as smooth, and several comments highlight supportive, engaged leadership and helpful admissions coordinators. Social and spiritual needs, celebratory meals, and individualized hygiene attention are additional positive themes cited by families who had favorable experiences.

Contrasting these positives, a number of reviewers describe service inconsistencies that suggest systemic operational weaknesses. Staffing levels and shift-to-shift coverage appear uneven, which reviewers link to delays in routine care, therapy, and monitoring. These gaps correlate with concerns about fall-prevention practices, pressure-injury risk management, and hydration and nutrition oversight. There are also recurring issues around communication: families describe difficulty obtaining timely updates, and some note limited or delayed communication with physicians and clinical leadership.

Dining and environmental feedback is mixed. Several reviewers praise cleanliness and adequate meal service, while others report inconsistent food quality and interruptions to meal continuity. Sanitation and incontinence-care practices are described as variable across stays, indicating uneven standards in day-to-day care activities. A small number of reviewers raised concerns about restraint use and about how acute issues are escalated, including transfers to higher levels of care.

Management impressions are likewise mixed. Some reviewers single out director-level staff for supportive leadership and positive change, while others perceive administrative priorities (for example, paperwork) taking precedence over bedside oversight. The overall picture is of a facility capable of providing excellent individualized care under some circumstances, but with operational variability that can produce markedly different experiences. Prospective residents and families may benefit from asking specific, current questions about staffing levels, fall and wound-prevention protocols, infection-control measures (including urinary tract infection prevention), meal-service consistency, and routine family communication practices before admission.

    About Valley Falls Terrace

Valley Falls Terrace sits at 400 Locust Grove Road in Spartanburg, South Carolina, and serves as a skilled nursing facility with 88 certified beds, providing both short- and long-term care for seniors who need 24-hour nursing and help with daily activities like bathing, dressing, and eating, and at this place, teams managed by Administrator Lacey Smith handle medication, transfers, supervision by a physician, and tasks like dressing or bathing, and when residents need it, a 24-hour call system's there for help, and they've got nurse staffing at 3.25 hours per resident each day, though there's a nurse turnover rate of 49.4%. The facility has had 17 total inspection deficiencies in its reports, including two related to infection and others about assessment, care planning, food and fluids, and accident hazards, and in April 2025 they had a standard inspection that showed four deficiencies, so folks considering Valley Falls Terrace should know these are in the public record. Residents get meals from a professional chef, weekly housekeeping, and access to all kinds of amenities, like fitness rooms, a movie theater, arts and music rooms, a spa and sauna, a game room, a library, walking paths, and there's both indoor and outdoor programs, plus social events, movie nights, and transportation when it's needed, and all this is done inside a Continuing Care Retirement Community that offers various levels of care, with private bathrooms, cable TV, Wi-Fi, furnished rooms, air conditioning, even little kitchenettes in rooms, which can make things a bit more comfortable since everybody's needs are kind of different these days. Valley Falls Terrace accepts Medicaid and Medicare, which helps with finances for many families, and they state that they accept all residents regardless of things like race, religion, orientation, or disability, and they also have resident and family councils set up, so concerns and changes can be brought up and talked over. The facility is affiliated with Fundamental Healthcare, and they keep aiming to give comprehensive medical care while knowing families want honesty, so people interested should look carefully at inspection reports and pay attention to ongoing issues, especially with food, safety, and infection control, which are important for everyone's well-being.
