    Medication errors, neglect, poor communication

I had a family member at this facility and I would not recommend it. Medication errors, nurse inaction and denial of a needed hospital transfer contributed to end-of-life neglect; staff communication was inconsistent and management rude. The place looks clean but bathrooms were dirty (urine on lids), food and service were often disgusting (dripping trays, ostomy care ignorance, bedsores), and two-person rooms offer no privacy. The only bright spots were a good PT therapist, flexible therapy and outdoor walks/grilling - but overall care, safety and communication were unacceptable.

    Summary review

Overall sentiment in the reviews is mixed but strongly weighted toward serious concerns about clinical care, communication, hygiene, and management, despite a few positive notes about therapy and activities. The most consistent positive theme is the quality of physical therapy: reviewers repeatedly note a good PT and flexible therapy options that include outdoor walking and occasional social activities such as hamburger grilling. A subset of staff are described as responsive, able to provide prompt help, and some meals or events were described positively.

However, these positives are outweighed in many reviews by significant and recurring negative themes about direct patient care and safety. Multiple reviews allege neglect, inadequate nursing response, and inaction in the face of medical issues, including a failure to address a gallbladder illness, denial of requested hospital transfer, end-of-life neglect, and at least one report tied to a patient death. Reports of bedsores, inadequate ostomy care, and medication errors indicate lapses in basic clinical care and monitoring. Techs and some clinical staff are described as lacking skills and acting unprofessionally, which compounds these clinical concerns.

Communication and coordination problems are another prominent pattern. Families report poor staff-family communication, inconsistent information among staff members, and a lack of discussion about patient goals or progress. Administrative confusion about insurance coverage and length of stay, combined with late notifications about discharge, creates additional stress and appears to contribute to mistrust. Several reviews highlight contradictory statements from different employees, suggesting systemic lapses in handoffs and internal communication.

Facility cleanliness and resident dignity are frequent issues. While the center may "look clean" on first impressions, reviewers report dirty bathrooms, urine on toilet lids, and dripping lunch trays left in rooms, indicating that actual practice does not match appearances. Privacy concerns are also mentioned, and the absence of private rooms (only two-person rooms available) is a recurring dissatisfaction. Reports of rude management and poor treatment of patients further erode confidence in the facility's culture and oversight.

Dining impressions are mixed: some reviewers praise food and outdoor grilling events, while others describe meals as "disgusting" and note poor handling (e.g., dripping trays). Activities beyond therapy—such as outdoor walking—are viewed positively, but these programmatic strengths do not compensate for the more serious clinical and sanitary shortcomings described.

In summary, the reviews paint a picture of a facility with meaningful strengths in physical therapy and some engaging outdoor activities, and with pockets of responsive staff. At the same time, there are repeated and serious complaints about clinical care, neglect, hygiene, communication failures, and management tone. These issues—particularly reports of bedsores, medication errors, denial of hospital transfer, and end-of-life neglect—are high-risk concerns for prospective residents and families. The pattern of inconsistent information, late discharge notices, and confusion about insurance suggests systemic administrative problems. Prospective families should weigh the strong PT and activity offerings against persistent reports of unsafe or inadequate nursing care, poor hygiene in practice, and unreliable communication when making placement decisions.

About Franklin Wellness & Rehabilitation Center

Franklin Wellness & Rehabilitation Center sits at 1287 West Main Street in Franklin, Tennessee, and offers a mix of skilled nursing, short-term rehab between hospital and home, and long-term care for people who need help with everyday activities, plus respite care if families just need a short break, and the place has 88 licensed beds with both private suites and shared rooms so folks can pick what works best for them, and it's good to know they offer daily life enrichment activities, social programs, and even a beauty shop for residents who like to keep up with self-care, and the outdoor courtyard gives some fresh air and space to relax or see visitors, while the dining facility keeps everyone fed with proper meals, though records show a dietary standard deficiency labeled F0812 that found potential for harm but not actual harm, and a nurse turnover rate at 81.9% which is much higher than the Tennessee state average, though reported nurse hours per resident each day are just a little better than the state number, sitting at 3.96 hours, which means someone's around a fair bit. Residents get access to satellite TV, WiFi, and tablets like iPads, so there's entertainment and a way to stay in touch, and every room, both private and semi-private, gets those basic amenities, while a strong rehabilitation program connects to both home health and hospice care if that's ever needed, and Certified Nursing Assistant (CNA) Training happens on site, which might interest anyone looking to work here since there are jobs open. Franklin Wellness & Rehabilitation Center was accredited by the Joint Commission, it's tree-covered and reasonably modern, but federal inspection reports note ten total deficiencies, one linked to infection protection, and another about properly letting residents self-administer drugs when they're able, showing that while the staff focuses hard on safety and encouragement, there are still areas they work on improving. The manager's been Pamela Benson since April 2024 and Bonnie Randolph since January 2025, but Ben Anderson is listed as administrator, and the place is owned directly by Franklin Holdco, LLC with several indirect owners like Anshel Niederman and Ecm Holdings, plus it's under the Ahava Healthcare umbrella. Social media links lead to Facebook and Meta logins, and they keep their website at franklinctrhc.com, and if you're needing somewhere open 24 hours a day for ongoing wellness and rehabilitation care, Franklin Wellness & Rehabilitation Center remains a provider many families check when considering spaces for loved ones around Franklin.
