    Spotless facility, alarming resident neglect

My experience was wildly mixed: the facility is spotless (sparkling floors, no odor), meals are nutritious, the rehab team was phenomenal, and many staff were friendly and helpful with Medicaid. However I witnessed dangerous neglect - a bedsore from neglect, a resident left in feces for about 12 hours, multiple falls, unresponsive/unprofessional nurses, no doctor evaluation, poor communication, improper infection control and overmedication. I'd trust them for short-term rehab only; I would not recommend for long-term care given staffing, transport and administrative problems.

    Summary review

Overall sentiment across the reviews is highly mixed, with a stark contrast between accounts of excellent, rehab-focused care and reports of severe neglect and safety failures. Several reviewers praise the facility's rehabilitation program, cleanliness, nutrition, and friendly staff; others report serious incidents including bedsores, residents left in soiled conditions for long periods, multiple falls, and emergency hospital admissions. This polarity suggests substantial variability in care quality that may depend on specific units, shifts, or individual staff members.

Care quality and clinical oversight show the widest spread of experiences. Positive reviews describe a safety-focused approach: fall-prevention pads, good team communication, attentive rehab therapists, and successful outcomes leading to patient satisfaction and willingness to return. Conversely, negative reviews describe alarming lapses: patients left on commodes/bedpans for many hours, development of pressure injuries, alleged overmedication, and a reported incident where a doctor discharged a patient with a diuretic without personally evaluating them. Several families reported delays in physician evaluation (doctor not seen by day 5), mislabeling of diagnoses such as dementia, and poor wound care practices including improper glove use during bandage changes, raising infection-control concerns.

Staffing and behavior appear inconsistent. Multiple reports cite unprofessional or unresponsive nursing staff, short staffing, alarms frequently going off, and instances of staff yelling at patients. At the same time, other reviewers emphasize friendly, helpful caregivers and strong team coordination. Administrative and communication problems recur: families describe lack of responsiveness to concerns, delays in care plans, inadequate notification about changes, and a perception of 'warehousing'—residents receiving minimal attention rather than personalized care. These management and communication issues often compounded clinical problems and contributed to family distress.

Facility environment and services receive mostly positive comments. The physical plant is described as pretty and very clean, with sparkling floors, no noticeable odors, and seasonal decorations. Rooms are basic but adequate, and residents may personalize spaces with their own furniture. Meal service is generally seen as nutritious, though meal choices can be limited and the dining experience likened to a hospital setting by some. Activities like bingo and daily programming are available, though reviewers note the atmosphere is not as warm as assisted living in some cases.

Operational areas with recurring complaints include transportation, discharge/transfer coordination, and equipment. Several reviewers mention unreliable transport services, appointment or surgery cancellations due to a single-source transportation system, and difficulty arranging outside medical follow-ups. Equipment concerns include inadequate beds (hand-crank issues) and other supply limitations. Positive operational notes include assistance navigating Medicaid and successful transitions for some residents who returned home after rehab.

Patterns to note: (1) Experiences are highly variable—some residents receive excellent rehabilitative, clean, and attentive care, while others experience neglectful and unsafe care. (2) Problems are concentrated around staffing, communication, physician oversight, and basic nursing care tasks (wound care, toileting assistance, fall prevention). (3) The facility physical environment and therapy services are frequently praised, indicating organizational strengths in rehab and maintenance even when clinical/nursing consistency falters.

In summary, Radford Health And Rehab Center shows clear strengths in rehabilitation services, cleanliness, and certain staff/teams who deliver compassionate, effective care. However, significant red flags appear repeatedly: reports of neglect, falls, pressure injuries, poor infection control, inconsistent physician involvement, and administrative failures. Prospective residents and families should weigh the facility's strong rehab reputation and clean environment against the documented variability in nursing care and oversight, and seek specific assurances about staffing levels, wound-care protocols, physician coverage, communication practices, and transportation before committing.
