    Positive care, some communication concerns

I had a generally positive stay - CNAs and nurses were kind, attentive and often went the extra mile; therapy, activities and transportation were excellent, rooms were clean and recovery-focused. That said, reviews are mixed: I saw issues with communication, occasional night-shift/medication lapses and some insurance/discharge confusion. Visit first and keep an eye on care, but overall I'm grateful for the team's support.

    Summary review

Overall sentiment: Reviews for Lone Tree Post Acute are highly polarized, with many families and residents offering strong praise for specific staff members, therapy outcomes, and engagement programs, while a substantial minority report serious concerns about safety, clinical competence, and responsiveness. The dominant pattern is one of uneven experience: many individuals describe compassionate, proactive teams that achieve good rehabilitation outcomes and smooth discharges, while others describe neglectful episodes, medication and wound-care errors, and staffing lapses that pose significant risk.

Care quality and clinical issues: Therapy and rehabilitation receive the most consistent positive feedback. Multiple reviewers credit PT/OT and the rehabilitation team with helping residents regain independence and return home, calling the therapy staff stellar, knowledgeable, and professional. At the same time, there are recurrent and specific clinical complaints: delayed or missed pain medication, confusion about controlled-substance protocols, IV line mismanagement (untaped lines, alarms ignored), wound and urostomy care deficiencies, and reported infections including UTIs and skin conditions. These reports indicate variability in nursing competency and clinical oversight; some families experienced excellent nursing care and clear clinical coordination while others filed formal complaints and warned of unsafe conditions. Such variability suggests pockets of high-quality clinical practice alongside instances of poor execution and oversight.

Staffing, responsiveness, and safety: Many reviews single out compassionate CNAs and nurses by name, praising individualized attention, kindness, and going above and beyond. Equally prominent are reports of ignored call lights, hour-plus delays in response, especially on night shifts, and instances where basic needs (water, toileting, bathing) were not met promptly. Night-shift performance appears to be a recurring pain point. Several reviewers warned of elder-abuse-level neglect and recorded experiences that led to police reports or formal complaints. The coexistence of exemplary caregivers and serious allegations of neglect points to inconsistent staffing, training, or supervision practices.

Facility, cleanliness, and amenities: A majority of comments describe a clean, well-kept facility, with renovated areas and rooms free of urine odor, while a smaller but notable subset describe unpleasant smells, dirty areas, and general uncleanliness. Food and dining also drew mixed reactions: some praise adequate meals and nutritionist involvement, while others cite poor food quality and weight loss. The activities program is frequently mentioned positively — reviewers commend staff who run engaging programs, bingo, crafts, and social events that contribute to residents' quality of life. Transportation assistance and accessible administration were also highlighted as strong operational features.

Communication, management, and transitions: Positive reviews point to proactive care conferences, helpful social work, and an accessible administrator who coordinates care well. Several families appreciated individualized discharge planning and successful transitions home. Conversely, multiple reviews describe poor communication about medical issues, weekend discharge surprises, and insurance-driven early discharges that left families uneasy. Reviewers repeatedly advise prospective residents to visit in person, meet staff, and monitor care closely. The presence of both highly responsive management and reports of unaccountable behavior suggests leadership can be effective but may not be uniformly influencing all shifts or departments.

Patterns and recommendations: The most consistent theme is variability. There are many named staff who consistently receive praise, indicating strong individual performers and teams (notably in therapy, activities, and certain CNAs/nurses). However, recurring negative themes around medication management, wound and ostomy care, nighttime responsiveness, and occasional cleanliness or odor issues are serious and were reported often enough to be considered systemic risks. Potential residents and families should weigh the facility's strong rehab and activities reputation alongside the risk of inconsistent nursing care by: visiting the unit and meeting staff, asking about staffing levels on night and weekend shifts, clarifying medication and wound-care protocols, confirming the expected length of stay and discharge planning processes, and establishing clear lines of communication with leadership. Overall, Lone Tree Post Acute can provide excellent rehabilitation and compassionate care in many cases, but variability in performance — particularly in nursing responsiveness and complex medical management — warrants vigilance and proactive advocacy by families.
