    Compassionate effective rehabilitation and care

I placed my 92½-year-old parent at Cloverdale Health Center and was impressed by the professional, respectful, and genuinely caring staff - nurses, CNAs, doctor, and the PT/OT/speech team all felt like family. The rehab and physical therapy were excellent and key to recovery; the kitchen crew exceeded expectations and there were plenty of activities and beautiful views. The facility is welcoming and mostly clean, and we'll truly miss the people, though four-person rooms can be crowded and we experienced one delayed X-ray and occasional cleanliness lapses. Overall I highly recommend Cloverdale for compassionate, effective rehabilitation care.

    Summary review

Overall sentiment across the reviews is strongly positive, with the dominant theme being high-quality, rehabilitation-focused care delivered by a broadly praised staff. Reviewers repeatedly highlight professional, respectful, and attentive nurses, CNAs, therapists (physical, occupational, and speech), and kitchen and food-service teams. Many accounts emphasize successful recovery outcomes, particularly through the facility's rehabilitation program and its strong physical therapy department. Multiple reviewers explicitly say they would recommend the facility to family and friends and note that residents and families formed close, warm relationships with staff.

Care quality and staffing receive consistently high marks. Reviewers describe nurses, CNAs, doctors, and therapy teams as caring, supportive, and attentive; some CNAs and therapists were singled out by name and acknowledged for their contributions. The staff dynamic is described as teamwork-oriented and safety-focused, and several comments characterize staff as treating residents like family. That said, there are isolated reports of less positive interactions—one reviewer noted an annoyed CNA—and these indicate some variability in individual staff behavior even though the overall picture is positive.

Therapy and rehabilitation are among the strongest, most frequently praised aspects. The facility is described as rehabilitation-focused, with effective PT/OT and speech therapy that contributed meaningfully to stroke recovery and other rehabilitative goals. Reviewers frequently cite rapid progress and good outcomes, and they recommend the rehabilitation program for anyone needing post-acute or restorative therapy. Personal testimonials include statements about staff helping residents recover and about families planning to miss the people and services after discharge.

Facility amenities and day-to-day life also draw favorable comments. The building is frequently described as clean and well-maintained; activities and social programming (movies in the dayroom, many social events) are noted as available and appreciated. Dining is repeatedly commended: the kitchen crew and food service are praised and in at least one review explicitly said to have exceeded expectations. The location and view are distinctive positives—reviewers mention a beautiful, panoramic hilltop outlook and pleasant sky views.

Room configurations are a mixed bag. Two-person rooms are described as excellent and comfortable, while four-person rooms receive criticism—reviewers point out that some four-person rooms are connected and share a single toilet, which reduces privacy and comfort. Prospective residents should weigh room type carefully when touring the facility.

There are a few important concerns and patterns that temper the otherwise favorable assessments. A minority of reviews report issues with cleanliness and with administrative or clinical follow-through. One reviewer reported a serious care coordination lapse: a resident became very sick and there was reportedly no home health order in place after discharge, plus an X-ray result was delayed by about two weeks. These are isolated reports in the set of reviews but are substantive and suggest that communication, documentation, and timely handling of diagnostic results can be areas to confirm during inquiries. Additionally, while most staff interactions are praised, at least one review noted an annoyed CNA, indicating some variability in staff demeanor.

In summary, Cloverdale Healthcare Center presents as a strong choice for rehabilitation and post-acute care: the facility's therapy departments, attentive nursing and CNA staff, engaging activities, good food, and scenic location are repeatedly commended. Most reviewers emphasize professionalism, safety, and supportive relationships between staff and residents. However, prospective residents and families should be mindful of room selection (prefer two-person rooms if privacy is a priority), verify cleanliness and housekeeping standards during a visit, and ask specific questions about how the facility manages care coordination, discharge planning, home health orders, and turnaround for diagnostic results. These precautions will help ensure that the generally high standards reported in most reviews apply in any individual case.

    About Cloverdale Healthcare Center

Cloverdale Healthcare Center sits at 300 Cherry Creek Rd in Cloverdale, California, and offers skilled nursing and rehabilitation for people who need both short-term and long-term care, and the staff there seem friendly and work together well, and people describe the atmosphere as warm and nurturing, which helps residents and staff build trust and friendships, and it's clear the place makes patient safety a focus, especially during COVID-19. The facility has 72 beds, with a choice of private rooms that hold two beds and shared rooms with four beds, with shared toilets, and the rooms are spacious and get plenty of daylight while the grounds outside are landscaped nicely, so the whole place feels homelike. Emergency care is available all day and night, and skilled nurses and in-house doctors lead care around the clock, making sure residents are cared for as soon as they need help. They do rehabilitation with in-house therapy that's hands-on and uses state-of-the-art technology, and there are full programs for wound care, neurological recovery, lymphedema, pulmonary care, post-operative needs, and different kinds of therapy like physical, occupational, and speech, and they even help with cognitive exercises and treatment for swallowing disorders, so if someone needs complex care, the staff seem equipped for it. Cloverdale Healthcare Center runs specialized activities and offers a calendar of social events, from religious services to adult education, and they plan shopping outings so residents can stay active, and they also provide hospice care and help people plan for discharge when it's time to go home. The staff take care of light housekeeping, laundry, personal care, and social services, and they handle medicine through a pharmacy service on site, and make sure therapeutic diets are available and the food is satisfying and nutritious, which is important for recovery and comfort. Residents here get skilled nursing services that cover wound care, enteral feeding, restorative nursing, and pain management, and the building and equipment are modern, so everything feels up to date but still homey, and there's always support on hand. The center isn't taking new patients for now, so people wanting to move in need to check back later.
