    Caring staff; unsafe, inconsistent care

I appreciated several CNAs and administrator Suzette - some staff were genuinely caring, professional, and made the transition smoother. But I also experienced alarming neglect: help-button malfunctions, catheter neglect that led to ER visits and repeated 911 calls, unresponsive administration, and reports ignored. The building is old, poorly maintained with parking and cleanliness issues and a depressing, unsafe vibe. Overall, despite excellent individuals, I could not trust the facility's consistency of care.

    Summary review

Overall sentiment is mixed but leans toward serious concern due to repeated reports of neglect, safety issues, and inconsistent leadership. Across the reviews there is a clear split: a portion of families and residents praise the interpersonal warmth and professionalism of particular staff members (particularly CNAs and one administrator named Suzette), while a substantial number of reports describe lapses in basic clinical care, safety, and communication that have led to emergency room visits and 911 calls.

Care quality: Reviews reveal a wide variation in the quality of direct care. Positive comments emphasize compassionate, attentive CNAs who make residents feel comfortable and cared for. However, multiple reports document CNA neglect, catheter neglect, forced bed rest contrary to a patient’s ability, and incidents serious enough to require emergency room visits. These negative accounts include delayed responses to calls for help, malfunctioning or ignored help buttons, and a front desk nurse described as inattentive. The contrast between praise for some caregivers and allegations of neglect suggests inconsistent standards of care and potential gaps in clinical oversight or training.

Staff and management: Staff impressions are polarized. Some reviews highlight staff who 'go the extra mile,' describe the administrator as effective and coordinated, and report smooth transitions from home to the facility. Conversely, several reviews complain about poor communication, an unresponsive administration, unprofessional comments toward patients, and language barriers among staff that impede care. These conflicting perspectives point to variability in leadership responsiveness and staff performance; some families find management helpful, while others feel ignored and frustrated. The repeated mention of a specific administrator by name (Suzette) in positive contexts suggests that leadership experience can differ by shift or by which staff are involved.

Safety and incidents: There are multiple serious safety-related themes. Repeated 911 calls, reports of patients left alone in pain, catheter neglect, and at least one account of forced bed rest raise red flags about medical supervision, response protocols, and regulatory compliance. One reviewer described being transferred to another facility as 'hell on earth,' indicating that poor outcomes may extend beyond this location when transitions are made. Patient wandering and a sense of an unsafe environment were specifically mentioned, reinforcing concerns about resident monitoring and environmental safety.

Facilities, cleanliness, and atmosphere: Physical plant issues and atmosphere are recurring complaints. Several reviewers describe an old facility in need of renovation, strong or foul smells, and general cleanliness concerns. Some characterize the atmosphere as depressing or even 'creepy'/'horror-movie vibe,' which, combined with reports of staff hiding from residents, contributes to a perception of an unwelcoming or unsafe environment. Positive comments about friendly staff are sometimes tempered by notes that the facility needs maintenance and has parking challenges.

Patterns and notable contrasts: The dominant pattern is inconsistency. Where one reviewer experiences competent, compassionate care and an effective administrator, another experiences neglect, unresponsiveness, or potential mistreatment. Frequent mentions of catheter neglect, delayed responses to help calls, and emergency interventions indicate systemic problems rather than isolated incidents. Language barriers and staff training concerns are recurring explanations families give for inconsistent care. Dining and activities receive little to no mention in the reviews provided, so no reliable conclusions can be drawn about those areas from this dataset.

Bottom line: The reviews paint a facility with caring individuals who can and do provide excellent support for some residents, but also with serious, repeated failings in communication, clinical care, safety, and the physical environment for others. Families considering this facility should be aware of the mixed experiences: positive personal interactions and smooth admissions exist alongside troubling reports of neglect, emergency incidents, and environmental deficiencies. The pattern suggests uneven staffing, possible training gaps, and facility maintenance issues that warrant direct inquiry and close monitoring if choosing placement here.

About Greenfield Care Center of Fairfield

Greenfield Care Center of Fairfield stands as a skilled nursing facility with 90 certified beds, and usually there are about 54 residents staying each day, and you'll notice it has both assisted living and memory care for people with Alzheimer's and other conditions, which is important for families trying to make the right decision, especially since the facility is flagged as a Special Focus Facility by the government because of repeated quality issues over the years, and even though nurse staffing averages 4.35 hours per resident per day-just a bit under the state average-care is available all day and night through Registered Nurses, Licensed Vocational Nurses, and Certified Nursing Assistants. There's a focus on keeping things warm and inviting, and the place does try to offer a sense of independence and community, so you'll see landscaped outdoor spaces and comfortable places inside for everyone to sit, talk, or just relax together, and there's always a handful of organized exercise, relaxation, and recreational programs going on to help strengthen muscles and keep up social connections. The center was found to have a lower nurse turnover rate than most in California-sitting at 37.3% when the state average is about 40%-but the serious inspection history can't be ignored, as there have been 102 total deficiencies listed in reports, 6 of which are tied to infection control, and there have also been issues like pharmacy service problems labeled as F0760 for medication errors and a Quality of Life and Care Deficiency (F0689) that notes lapses in preventing accident hazards and providing enough supervision, so it's honest to say some families may want to read inspection reports carefully. Ownership is split 50/50 between Jenq Chen and Tze Yun Chen, both tied to Eva Care Group, and there's no extra information about other owners or who manages the day-to-day stuff. Residents get access to a range of support-assisted living, individualized care plans, medication management, memory care, 24/7 nursing, transportation, therapy like physical, occupational, and speech, as well as programs for Parkinson's care, pain management, emotional support, and even religious services. There's lab work, wound care, behavioral management, nutritional reviews, podiatric, optometric, and dental visits, and the staff includes on-site therapists plus options for inpatient nursing and short-term rehab or long-term skilled nursing. The team is said to listen to residents' daily preferences and has amenities to make life a bit more comfortable, which includes laundry help, temporary stays, and activity programs, and though the facility keeps up a presence on Facebook-with language access and details online-it's smart to look beyond the social media posts when trying to understand what daily life is really like, especially with the long list of past care concerns to consider.
