    Clean facility but unsafe practices

I had a mixed experience. Many nurses, CNAs, therapists and the administrator were professional, caring, hardworking, and the facility has been cleaner and renovated with good rehab and activities. However I also saw serious lapses - poor communication, inconsistent training, missed/incorrect meds, staff sleeping or neglecting residents, theft and difficulty getting records - that put residents at risk. Because of those safety issues I can't fully recommend this place without confirming improvements and oversight.

    Summary review

The reviews for Fowler Care Center present a highly mixed and polarized picture. A significant portion of reviewers praise the staff, rehabilitation services, and visible improvements to the facility. Multiple reviewers specifically highlight caring, friendly, and dedicated nurses and CNAs who perform daily resident grooming, hair styling, and assistive tasks. The physical and occupational therapy teams receive positive mentions for motivation and respectfulness, and at least one review singles out the administrator for being helpful and engaged with the community. Several reviews note that the facility has undergone renovations and upgrades under new ownership, resulting in a cleaner, more homey and professional environment that some would recommend.

Contrasting sharply with the positive feedback are several serious and specific allegations of neglect and deficient care. These include reports of a resident being left on the floor overnight without a blanket, omitted medications, unmet physiological needs, and night staff asleep on duty. Reviewers describe resulting health risks such as dehydration and possible pneumonia. Issues of poor staff training and inconsistent care quality are repeatedly raised, and some reviewers explicitly recommend against the facility based on these experiences. There are also multiple one-star ratings and strongly negative comments characterizing living conditions as horrible and calling for staff terminations.

Administrative and communication problems are another notable theme. While some reviewers praise the administrator and note community involvement, others report poor communication overall, difficulty obtaining medical records, and disruptive patient transfers between facilities. These contradictory comments suggest variability in administrative performance or differences over time and by unit. The inability to access records and poor communication with families are important operational weaknesses that compound clinical concerns and reduce trust.

Safety and security issues appear in several summaries. Allegations of theft of residents' belongings and accusations of 'shady' staff members are serious red flags that affect family confidence. Combined with reports of improper dining (including inappropriate diabetic meals), medication mismanagement, and staff not responding to basic needs, these concerns raise questions about oversight, training, and the robustness of policies for resident safety and nutrition.

There are also clear signals of improvement and strengths that may be concentrated in particular areas or after changes in ownership. Multiple reviews emphasize renovations, a cleaner environment, and warm, welcoming staff—phrases like 'totally recommend,' 'professional,' and 'homey' recur. Rehab services and some managerial personnel earn strong praise. This pattern suggests a facility in transition: some residents and families are experiencing high-quality, compassionate care and improved facilities, while others continue to report harmful lapses and systemic problems.

In synthesis, Fowler Care Center shows a split record. Strengths include compassionate front-line caregivers, a well-regarded rehabilitation team, visible facility upgrades, and active efforts at resident enrichment. However, the negative reports are significant and specific: alleged neglect, medication errors, sleeping night staff, theft, inadequate meals for clinical conditions like diabetes, poor communication, and difficulty obtaining records. These issues are serious enough to warrant attention from management and oversight bodies. For prospective residents or families, the reviews recommend a careful, individualized inquiry: visit more than once at different times (including evenings/nights), ask detailed questions about medication administration, staffing levels, security protocols, meal planning for clinical diets, incident reporting, and access to medical records, and seek references from recent families who can speak to changes under current ownership.

    About Fowler Care Center

Fowler Care Center in Fowler, California is a skilled nursing facility with 46 beds, and it offers both long-term care and post-acute rehabilitation for people needing a higher level of care after hospitals or when facing chronic health needs, and while there's not much detailed information available about the amenities or care types, the center does have a strong focus on keeping residents' minds, bodies, and spirits as engaged and positive as possible. The building is secure and set on grounds with enough space for residents to move about, and there's a team of professionals who provide specialized nursing care 24 hours a day, which helps people who need regular medical attention feel safe. They also offer physical therapy with programs made for each person, occupational therapy that helps residents adjust at their own pace to their surroundings, and speech therapy that works on everything from how a person communicates, to swallowing, to memory and thinking skills, so people can keep as much independence as possible. The social services team supports both residents and their families, helping everyone get through tough times and transitions, and they'll often step in to solve problems or answer important questions along the way. Activities are planned to keep days positive and help residents connect, even on hard days. Details about dining or rooms aren't clear from what's available, and some information about services appears to be missing. Every member of the staff seems focused on creating a caring, supportive environment for each resident, and there's an effort to make sure people feel looked after. Sometimes, when searching online, people might run into a CloudFront server error instead of finding more details, so it can be hard to learn much more about what's on offer.
