    Excellent rehab, caring staff, clean

I stayed here for rehab and I'm going home this week - grateful for the excellent physical therapy and strong, caring nursing that got me ambulatory. The facility is very clean, airy with mountain views and a lovely courtyard accessed by sliding glass doors; meals were well prepared and dietary needs respected. Staff and management were professional, cordial and genuinely caring, activities were varied, and the whole experience felt supportive. I would recommend this facility.

    Summary of reviews

Overall impression La Crescenta Health Care Center appears to deliver strong rehabilitative care and an attentive bedside experience for many residents, with multiple accounts of successful progress from short-term rehab to assisted living and home discharge. Reviewers commonly highlight professional, caring nursing aides and therapists, a well-organized rehabilitation department, and management involvement in outreach and vaccination efforts. At the same time, the facility elicits mixed impressions on operational consistency: positive descriptions of cleanliness and organization coexist with reports of sanitation and lighting problems in portions of the building.

Care and staff Care quality is often characterized as compassionate and competent, especially in therapy and day-to-day nursing aide work. Physical therapy is a repeatedly noted strength, with families describing measurable functional improvement. Staff conduct is frequently described as courteous and helpful, and reviewers also credit prompt responses to maintenance and bedside needs (for example, fixing beds or managing pain). However, there are recurring concerns about staffing levels—especially overnight—which some reviewers say contributes to slower response times. Separately, a subset of comments point to unevenness in staff responsiveness, family communication, and how some clinical incidents are handled and communicated to families.

Dining and nutrition Dining impressions are mixed. Several reviewers praised prompt meal delivery, ample portions, and accommodation of dietary restrictions, while other reviewers described meals as bland or not to their taste. This pattern suggests the facility generally achieves reliable meal service and portioning but may have inconsistent culinary quality or menu variety that does not satisfy all residents.

Activities and environment The activity program is a clear asset: the facility offers a range of social and recreational options (birthday parties, karaoke, arts and crafts, movies), and there is direct access to an outdoor courtyard and accessible patio, which support engagement and mobility. The building also has pleasant mountain views and airy common spaces. Conversely, some residents and families described certain resident rooms and wings as having limited natural light and a somber ambiance, which can affect perceived quality of life for more sensitive residents.

Facilities, safety, and operations Many reviewers described the facility as clean and well-maintained; others raised sanitation and odor concerns in specific areas, indicating variation across units or shifts. There are also operational weaknesses identified around incident response and infection-control follow-up, and a few accounts describe delays or gaps in follow-through for ordered items and ancillary services (for example, transportation cancellations or items not delivered after placement). Financial and logistical processes drew attention as well: reviewers raised questions about billing transparency and nonstandard transportation/payment arrangements, which suggest a need for clearer communication and tighter administrative controls.

Notable patterns and considerations for families Strengths to weigh: robust rehabilitation services, generally caring and professional front-line staff, active programming, outdoor access, and an overall atmosphere that many families find well managed. Areas to probe during a tour or meeting: staffing levels at night and peak times, how the facility handles clinical incidents and infection-control follow-up, consistency of cleanliness across units, meal sample tasting and menu planning, transportation policies and billing transparency, and room lighting/placement options. These operational clarifications can help families balance the facility’s clear clinical and programmatic strengths against the variability described in other operational areas.

About La Crescenta Health Care Center

La Crescenta Health Care Center sits as a for-profit, 92-bed skilled nursing facility, serving older adults who need help after a hospital stay, surgery, or illness, and it's got everything from medication management and help with bathing, dressing, and transfers, to 24-hour supervision and 12-16 hour nursing care each day, and with furnished rooms that come with Wi-Fi, air conditioning, private bathrooms, cable TV, telephones, and even a kitchenette, it tries to make day-to-day living comfortable. Residents find several community features-an arts room, library, fitness room, walking paths, movie theater, garden, and business room, along with daily scheduled activities, and there's a focus on keeping folks busy with movie nights, music, resident-run programs, and a variety of activities tailored to individual interests, all managed by a staff that includes a professional chef who prepares restaurant-style meals with options for allergies and diets like diabetes, with a registered dietitian on hand working with doctors for nutrition planning. The place takes both Medicare and Medicaid, has 88 average residents per day, and maintains four or so nurse hours per resident every day, so people who need help with personal care like bathing, dressing, and eating, or more advanced needs like wound care, intravenous therapy, pain management, enteral therapy, and both short and long-term care, can get it, and there's also a dedicated COVID unit with "dedicated heroes" staff for infection control, not to mention a strong emphasis on safety and communication with public health officials, and they've put in place visitation rules following health guidelines. All rooms have emergency alert systems just in case, and there's transportation, parking, housekeeping, laundry, and dry cleaning available, plus a Rehabilitation Therapy Department that tackles everything from basic tasks like cooking and cleaning, to dedicated occupational, physical, and speech therapies, and pain management. Inspectors found 63 deficiencies in recent reports, including 6 related to infection, and as of June 2025, 3 beds remain available; the most recent report on April 25, 2025, cited 12 deficiencies including issues with infection control and garbage disposal, so there are things to consider on that front. Staff work directly with residents and their physicians to create care plans, and offer psychological services, social and emotional support, restorative nursing, feeding assistance, and retraining when needed. La Crescenta Health Care Center does not operate inside a Continuing Care Retirement Community and is affiliated with Mariner Health Care, and while it provides a wide range of care and activities, it's wise to look closely at its inspection history and nurse turnover rate (37.3%) before making any decisions. Everything is delivered in English, and the center offers both non-ambulatory care and individual support with daily living, plus special assistance programs, a music program, a game room, fitness sessions, wellness and spa services, a community garden, and outdoor programs, so residents find both medical support and things to keep them engaged each day.
