    Compassionate, clean, organized, excellent care

I placed my loved one here and have been very happy - the nurses, CNAs and therapists are knowledgeable, compassionate, and organized, staff remember names and communicate promptly, and the center is very clean. Rehab, meals and daily care exceeded expectations and the administration is attentive; overall a warm, safe, reassuring place with just a little room for improvement.

    Summary of reviews

The reviews for Verdugo Valley Skilled Nursing present a mixed but actionable picture. Many families praised the facility's strengths: compassionate nursing assistants and licensed nurses, a broadly well-regarded therapy team (PT/OT/ST) that supports post-surgical and discharge goals, and a dietary staff that engages with residents. Several accounts describe a rehabilitation-focused environment with organized case management, engaging activities, holiday decorations, and a warm, home-like atmosphere. Individual staff members and front-desk personnel were frequently singled out for positive, reassuring interactions.

At the same time, a number of consistent operational concerns emerge. Staffing variability and delayed responsiveness to resident needs were described repeatedly, with call-response delays and perceived gaps in routine assistance. Reviewers also cited inconsistent cleanliness and sanitation maintenance in some areas, as well as specific equipment and maintenance shortcomings. Communication and coordination problems were reported at both the clinical and administrative levels โ€” examples include delayed updates to families, challenges coordinating with hospitals and hospice, and disagreements about care plans or discharge readiness. There are also several accounts indicating issues with personal-property management (lost or missing clothing) and occasional conflicts around visitation or administrative responsiveness.

Dining and activities received largely positive comments overall: many residents enjoyed meals and found the dining staff personable, and programming (including spiritual options and social activities) was described as lively and supportive of resident well-being. However, there are also isolated complaints about meal quality and about perceived discrepancies between promised and delivered meal services, which suggest variability in dining consistency.

Management and leadership impressions are polarized. Some families praised individual case managers and supervisors for proactive involvement and clear communication; other accounts describe poor follow-through, unprofessional interactions, and a lack of timely problem resolution. A small number of serious allegations involving care-plan non-adherence and end-of-life communication problems were raised and should be clarified directly with facility leadership.

For prospective residents and families: an in-person visit and targeted questions are advisable. Observe staffing levels on the unit, ask about average nurse/CNA ratios and call-light response times, review the facility's property and laundry policies, clarify meal-service expectations and therapy scheduling processes, and request how the facility coordinates with hospitals and hospice providers. Asking to speak with the current case manager and to see recent sanitation and maintenance records can help assess whether the facility's strengths align with your priorities and whether identified operational weaknesses have been addressed.
