    Immaculate compassionate staff, excellent therapy

I stayed here for short-term rehab and was very pleased overall. The facility is immaculate, odor-free and welcoming; staff from nurses and CNAs (Sylvia was outstanding) to housekeeping and administration were compassionate, attentive, and went above and beyond. Therapy was excellent - I made strong progress - meals and desserts were tasty, activities were engaging, and the community felt safe and supportive; I would confidently recommend it.

    Summary of reviews

The reviews present a mixed picture of The Rehabilitation Center of North Hills: many families and patients praise a strong rehabilitation program and a generally clean, well-maintained facility, while others describe operational and clinical inconsistencies that materially affect care experience. Positive aspects most consistently cited include effective physical and occupational therapy, attentive respiratory and rehab therapists, compassionate direct-care staff and CNAs, and a bright, community-oriented atmosphere with engaging activities. Several accounts attribute measurable functional improvement to the therapy teams and note attentive, helpful staff members and daily room upkeep.

Care quality is uneven. Reviewers commend the facility for strong short-term rehab outcomes and for individual staff members who go above and beyond; however, there are recurring concerns about clinical-continuity issues. Descriptions include inconsistent wound-care follow-through, medication-administration and documentation weaknesses, and occasional gaps in nursing oversight. Some reviewers reported delays in attending to care needs and in responding to call bells, with the problem more commonly noted during night shifts. There are also concerns about delegation and competency in some clinical tasks and reports of events that resulted in hospitalization, which suggests a need for more consistent clinical protocols and oversight.

Dining, activities, and the physical environment show contrasts. The building and rooms are frequently described as clean, bright, and well-maintained, with regular linen changes and an inviting common area atmosphere. Activity programming and social engagement receive positive comments, although some families find offerings limited or moved to in-room options for safety. Food quality is variable: some reviewers describe tasty meals and attentive dietary staff, while others cite limited menu variety, restrictive diets without preferred alternatives (for example, vegetarian options), and intermittent dissatisfaction with meal service timing and portions.

Operational and management patterns emerge as key determinants of the experience. Important operational weaknesses include inconsistent staffing levels (notably at night), slow call-button responses, variable staff professionalism and communication style, and inconsistent case-management responsiveness. Infection-control communication and transparency around exposures were noted as problematic in some accounts. Security controls for personal belongings and financial items were flagged as a concern by multiple reviewers. While some families found administrators and clinicians responsive and supportive, others reported difficulty obtaining timely updates or constructive follow-through from management and case managers. Prospective residents and families should weigh the facility’s strong rehabilitation capabilities and clean environment against these variability risks; asking specific questions about night staffing levels, wound-care protocols, medication administration processes, infection-control policies, and personal-belongings safeguards will help clarify whether the center’s current operations meet their expectations.

About The Rehabilitation Center of North Hills

The Rehabilitation Center of North Hills sits in North Hills, CA, and has been managed by Pursue Health LLC since October 2014 and has Shlomo Rechnitz listed as having ownership or some level of control, and it's classified as a Special Focus Facility Candidate, which means it's been flagged for a history of serious quality issues, though it hasn't been formally marked by the government as needing the highest watch. The building's newly renovated, and inside, residents find certified beds for up to 138 people, with an average count of around 129 actually staying there each day. This place covers both short-term and long-term care, has sub-acute care, and its big focus is on rehabilitation with programs aimed at helping folks recover and reach the best possible function they can, and there are services for physical therapy, occupational therapy, and speech or language therapy, plus social services and nutrition support, which helps round out the care folks get.

Staffing includes Certified Nursing Assistants (CNAs) and Registered Nurses (RNs), and staffing rules say residents must get at least 3.45 hours of nursing care each day, 2.45 hours of that from CNAs and at least 0.55 from registered nurses, but on average, the actual numbers at this place run a little higher-CNAs offer about 0.0833 extra hours per day, and RNs do about 0.1318 more than required, with the total nurse staffing coming to 4.52 nurse hours per resident daily, which lines up almost exactly with state averages. There's always a registered nurse on site at any hour of the day or night. The nurse turnover rate is 27.0%, which is below the state's average of 40.0%, so there are a good number of steady staff, and staff roles here include Certified Nursing Assistants, Licensed Nurses, Nurse Managers, Dietary, and Environmental Services, with health, dental, and life insurance for employees along with vacation, paid sick time, holiday pay, and even a 401-K plan. They also offer career opportunities across departments as listed in their application process.

Services cover both the mind and the body and the building's got activities and amenities focused on helping recovery and keeping folks comfortable, so residents get a mix of care, therapy, social time, and things to do. There are specialized practitioners available, such as physical therapists and speech therapists, who work as part of what's called an interdisciplinary team, and everyone works together, from nurses to administrative staff to therapists, to support each resident's needs and help improve quality of life. Nutrition services are part of the plan as well, and the staff are known to follow up with patient concerns and help keep residents clean. They say they're guided by what's best for the people staying there and try to fix problems when they come up.

However, the center's had 105 total deficiencies found in inspection reports, including 9 related to infections, and these were violations of federal standards set to protect residents from infections spreading around, as well as other issues in the area of resident assessment and care planning, where sometimes residents didn't get full or accurate assessments or proper care plans mapped out. While many residents and families may see the effort from a caring staff and find helpful programs aimed at boosting recovery, inspection reports show room for improvement in some key areas, especially regarding infection control and care planning. A brochure's available for those looking to learn more about what this facility offers and how it operates.
