    Welcoming, compassionate, professional care facility

I'm very pleased with this welcoming, clean, fully remodeled facility - the staff (nurses, CNAs, therapists, housekeeping, kitchen and activities) are compassionate, professional, and genuinely caring. Social worker Erick was outstanding and responsive, rehab helped regain strength, meals and activities are excellent, and communication with families is strong; I highly recommend this warm, secure place with only minor room for improvement.

    Summary of reviews

Vineland Post Acute is generally characterized by a strong rehabilitation orientation and a caregiving staff that many families describe as compassionate and attentive. Therapy services (physical and occupational) are highlighted repeatedly for helping residents regain mobility and strength; the on-site gym and rehab-focused equipment support those outcomes. Nursing and certified nursing assistants are commonly described as warm and family‑like, and case managers and social workers — often identified by name — are credited with clear communication, coordination of appointments and transportation, and active discharge planning.

Dining and activities are consistent strengths. The facility offers customizable meal options, dietary accommodations, and periodic special dining events that families appreciate. Activity programming is described as engaging, with a named activity coordinator running varied offerings that support socialization. Housekeeping and kitchen staff are frequently called out for hardworking service, and outdoor spaces such as patios and a yard provide additional opportunities for residents to spend time outside.

Facility condition and operational consistency show variation. Several accounts describe clean, well-lit common areas and a remodeled appearance in parts of the building, while others indicate odor concerns and smaller, dated rooms in other areas. These comments suggest uneven maintenance or staggered renovations across units. Reviewers also describe positive end-of-life and hospice support in a small, family-oriented atmosphere that many families found comforting.

Management and communication patterns are mixed. Administrative and social-work staff are often praised for being supportive and accessible, yet there are recurring notes about inconsistent physician presence and bedside communication style. Separate operational concerns include gaps in personal-property handling (for example, difficulty resolving lost items) and questions about the facility’s assurance of continuous 24-hour coverage. Overall, the dominant themes are high-quality, rehabilitative and person-centered care delivered by an engaged front-line team, tempered by variability in facility condition and a few operational processes that prospective families may wish to confirm during a tour.

    About Vineland Post Acute

Vineland Post Acute sits in North Hollywood, California, and has 49 certified and licensed beds, so it's a smaller place where most days about 41 residents stay, and it's run as a for-profit company by Thomas Chambers and David Johnson. The facility is really focused on post-acute care, helping people recover after hospital stays, and they do offer skilled nursing care round-the-clock, with nurses providing an average of 4.57 hours per resident each day, though the nurse turnover rate at 35.2% is something folks sometimes wonder about. They work on making care plans personal, usually working with the family to decide on treatments, and the therapy team offers a mix of physical, occupational, and speech therapy, using a dedicated therapy room with lots of equipment, all meant to help residents regain strength and independence. People can also get help from social services for care planning, discharge, and dealing with requests or concerns, and there are uplifting activities and social programs designed so everyone can join in no matter their needs. The place has outdoor patios with shaded seats, an entrance seating area, a wide room with a fireplace and art on the walls, and both front and back outdoor entrances you can see if you look at photos.

Vineland Post Acute aims to provide a comfortable, family-friendly setting during recovery, and they say they're committed to mental health, good teamwork, and putting patients first, all while trying to create a secure environment that addresses both clinical and social needs. They help with support services if someone needs post-acute rehabilitation, hospital discharge recovery, or home healthcare, and there's always a team of clinicians, therapists, and medical consultants ready to help with therapy and skilled nursing care day and night. They've got a 5-star Medicare rating, but even with that they've had 47 inspection deficiencies, including four for infection control like lacking a complete infection prevention program, and some resident rights deficiencies such as not always making sure the environment stayed as safe, clean, or homelike as it should be. The Centers for Medicare and Medicaid Services do inspect and report on the facility, so reports and complaints are a matter of public record. Vineland Post Acute does follow local health department guidance on visitation and infection control, inviting tours for families looking into care.
