    Compassionate, effective care; minor improvements

I'm very pleased with the care my family member received-compassionate, attentive staff and outstanding CNAs (Michelle stood out). Rehab and therapy were effective and helped regain mobility; nurses and doctors were knowledgeable. The building is clean, warm and activity-filled, meals were good, and admissions/leadership (Maria Thompson) were professional and helpful. Overall very good care with just a little room for improvement.

    Summary of reviews

Reviews of Bay Marina Post Acute present a polarized but actionable portrait. Many families and patients describe strong clinical capabilities โ€” especially robust therapy programs (physical, occupational, speech), skilled nursing care including wound management and on-site IV antibiotic capability, and effective rehabilitation outcomes. The facilityโ€™s physical plant and public areas receive frequent praise: reviewers describe a modern, well-maintained building, welcoming interiors, and an efficient admissions and visitor process that supports family engagement.

Staffing impressions are mixed. Numerous comments highlight compassionate CNAs, effective therapists, and administrators who facilitate family communication and coordination of care; several reviewers specifically called out strong end-of-life support and timely family updates. At the same time, other reviewers describe inconsistent staff professionalism, variability in bedside nursing responsiveness, and concerns about medication and pain-management consistency. These patterns suggest variability across shifts or units rather than uniform staffing performance.

Dining and activities are also uneven in perception. The activities program is consistently noted as a strength โ€” engaging social programming, games, and community interaction are mentioned repeatedly. Food quality comments are mixed: some families praise tasty, sufficient meals, while others identify limited variety or substandard meal experiences. Facility-level operational issues that emerge across reviews include intermittent sanitation and incontinence-care delays, questions about personal-property and medication security controls, weekend supervision shortfalls, and problems with phone and internal communication systems.

Management and leadership earn both recognition and critique. Several reviewers praised administrative responsiveness, an accessible admissions team, and named leadership that appears active in the facility. Conversely, others describe variability in care-team communication and social-work responsiveness, and raise concerns about clinical-incident follow-up and safety oversight. Given the range of experiences reported, prospective residents and families would benefit from focused questions during a tour: current staffing ratios (including weekend coverage), medication-management and property-security procedures, infection-control and sanitation audits, call-button response metrics, and examples of recent quality-improvement work. Overall, Bay Marina demonstrates clear strengths in therapy, rehabilitation, and facility appearance, but reviewers also identify important operational weaknesses that warrant direct inquiry prior to placement.

    About Bay Marina Post Acute

Bay Marina Post Acute sits on Fruitvale Avenue in Oakland as a skilled nursing and rehabilitation facility where folks can recover from surgery, serious illness, or injury, and the place has a tendency to feel pretty warm and cheerful thanks to a steady flow of social activities, exercise programs, and recreational events, and folks here might spend their days enjoying visiting beauticians, spiritual services, and transportation for appointments or outings, while staff provide round-the-clock nursing care with licensed nurses and certified nursing assistants always nearby. The place offers both short-term and long-term care with physical, occupational, and speech therapy, and folks will find care for things like pain management, wound care, post-surgical needs, and IV therapy, all with diets planned by a dietitian to suit each resident's needs. The building is a single story and set up with semi-private rooms, and they usually have about 85 folks staying at once, out of 94 certified beds, and the staff bring some language diversity, though English is the main language. The staff uses a resident-focused approach with individual care plans, and they try to keep things bright, friendly, and supportive while helping people stay as independent as possible. Bay Marina Post Acute is run by Corporate Interface Services LLC, under a for-profit structure, and is affiliated with Windsor, and the whole setup stays under federal standards for skilled nursing, including infection control. Still, recent state inspections did turn up deficiencies, including problems with infection control, supervision, and the reporting of suspected abuse or neglect, and there's a recorded nurse turnover rate of about 40 percent, which means some staff changes happen fairly often. With a nurse staffing ratio averaging about four nurse hours per resident per day, Bay Marina works to offer comprehensive care and rehabilitation, but it isn't accepting new patients right now. The facility takes a holistic approach, looking after physical, social, and emotional health, and keeps a small, approachable feel much of the time, but like any place, it has its challenges as noted in inspection reports.
