I moved my father here and I'm very pleased - the facility is clean, affordable, and offers private and semi-private rooms with provided furniture and accessible bathrooms. Staff are consistently friendly, sincere, and helpful - excellent nursing, housekeeping, and administration - and there are active daily activities like Bible study, games, exercises, and weekly bus trips. Meals are served three times a day in a tidy dining area, and SSI/low-income bed options are available. Overall a very positive experience; I'd choose this place again.

Summary of reviews
Rialto Assisted Living receives broadly mixed feedback that suggests strengths in day-to-day resident interaction and weaknesses in several operational areas. Many families and residents describe compassionate, helpful caregiving staff, supportive administrators, and access to allied clinicians such as physical therapists, a dietitian, and social work. Housekeeping and clean common spaces are noted positively in several accounts, and the facility is frequently described as affordable with options for SSI and low-income beds. Where interior updates exist, reviewers commented favorably on refreshed rooms, carpeting, and accessible bathrooms.
Care quality descriptions are variable. Positive accounts highlight attentive caregivers, organized nursing, and steady improvement in resident condition under the facility’s program. However, a recurring concern is inconsistent responsiveness to call bells and assistance requests; reviewers attribute some delays to limited staffing levels. Some families described delays in attention to incontinence-related care and uneven nursing follow-up, creating a mixed picture of clinical consistency.
Dining receives mixed evaluations: some residents find the meals well-prepared with adequate portions and evening snacks, while others describe limited menu variety and inconsistent food quality. Activity programming is a relative strength — the community offers daily activities, devotional groups, exercise, games, and regular off-site bus trips — which supports social engagement for many residents. Family volunteers and board-game groups also contribute positively to the social environment.
Facility condition and amenities are uneven. Portions of the building and grounds are described as clean and orderly, but other accounts note run-down carpets, uneven groundskeeping, and a generally dated appearance. Accessibility features are present, with wheelchair access and accessible bathrooms in many units, but private and semi-private rooms are often described as small and modestly furnished. Internet connectivity varies by room, which may limit residents’ ability to stay connected.
Management and communication show both strengths and gaps. Specific staff and administrators receive praise for helpfulness and professional admissions support, including end-of-life coordination; at the same time, reviewers cite inconsistent administrative follow-up and occasional lack of response to concerns. There is also at least one allegation of missing personal items during a transition, which stands out as a serious individual claim that prospective families should inquire about directly.
Overall, Rialto presents as an affordable, social community with active programming and caring front-line staff, but with operational challenges in staffing consistency, some aspects of maintenance and housekeeping, meal variety, and connectivity. Prospective residents and families should weigh the facility’s value and social strengths against these operational trade-offs and ask specific questions about staffing levels, response times, maintenance plans, internet access, and personal-item safeguards during tours and admissions conversations.

About Rialto Assisted Living
Rialto Assisted Living sits on a single story in Bloomington, CA, and offers both assisted and independent living for adults aged 55 and up, giving folks private or semi-private rooms, with patio options for fresh air, and all rooms have call lights in both the rooms and bathrooms for safety. The building feels cozy and home-like, and the staff help with bathing, dressing, and managing medication. Meals come three times a day, plus snacks, all managed by a registered dietician, and residents eat in a dining room where the focus is on taste and health. Staff handle laundry and housekeeping each week, so you don't have to worry about that, and there's always someone around for personal care or extra assistance, including help with incontinence, diabetic care, and non-ambulatory needs.
Recreational activities keep everyone busy, with a full-time activity director arranging exercise classes, movies, games, arts and crafts, and weekly shopping trips, and the community features a central gazebo where people gather to enjoy the weather or chat. There's a beauty salon right on site, along with indoor common areas and a large living room space so socializing and relaxation are simple. There's also respite care for short stays, hospice services for those needing end-of-life support, and specialized care for folks living with Alzheimer's disease.
Transportation's available for shopping trips, and to get to doctor and dental appointments, plus there's parking for residents and visitors. Optometry, podiatry, dental, and respiratory services come to the community, along with speech, occupational, hearing, and physical therapies, and there's medical and psychological help if needed. Pets are allowed with some restrictions, and devotional services are offered offsite. Residents join in on a structured calendar of social and educational events, councils, entertainment, and trips outside the community. Rialto Assisted Living holds a state license and tries to keep a helpful, friendly culture, as noted by their good reviews. Staff stay close by to help and make sure everyone's comfortable, safe, and able to enjoy a good quality of life.
