    Excellent long-term care, attentive staff

My mom stayed here for four years and I was consistently reassured by the bright, spotless facility and genuinely caring, attentive staff who greeted us by name. The food (including Asian options) was excellent, activities like bingo and outings kept her engaged, and the rehab team and on-site physician provided personalized, professional care. Overall I'm very satisfied and would highly recommend this center, with only minor room for improvement.

    Summary of reviews

The reviews for ACC Care Center: Skilled Nursing & Rehabilitation describe a facility with clear operational strengths alongside recurring operational weaknesses. Strengths most often cited include a clean, bright physical environment, an active and well-regarded rehabilitation program, and a variety of culturally appropriate dining options. Many families and residents praised individual caregivers, therapists, and support staff for being friendly, patient, and competent; several accounts highlight regular physician presence, thorough family meetings, and volunteers/activities that contribute to a home-like setting.

At the same time, a consistent pattern of clinical and operational concerns emerges. Reviewers raised issues about inconsistent staffing levels and responsiveness, which caregivers and family members link to hurried interactions and delays in attending to resident needs. Several accounts indicate problems with pressure-injury prevention and wound care, inconsistent bathing and hygiene routines, and gaps in pain and medication management. There are also reports of missed or cancelled appointments and therapy scheduling problems that affected continuity of care.

Management and workplace dynamics are another notable theme. Some reviews point to leadership turnover, perceived favoritism or clique-like behavior, and low staff morale; reviewers described these factors as having a direct effect on day-to-day resident experience. Additional operational concerns include lapses in documentation and registry processes, inconsistent handling of personal belongings and end-of-life arrangements, and perceptions that billing or payment processes receive significant focus.

Overall, experiences appear polarized: many long-term residents and families describe compassionate, coordinated care and positive outcomes, while other families report significant lapses that they found unacceptable. Prospective residents and families would benefit from an in-person evaluation that includes direct conversations with nursing leadership, review of wound-prevention and medication-management protocols, observation of staffing levels across shifts, and clarification of admission and billing policies. Checking how the facility manages personal belongings and end-of-life procedures is also advisable given the concerns described.

About ACC Care Center: Skilled Nursing & Rehabilitation

ACC Care Center: Skilled Nursing & Rehabilitation, located in Sacramento and operating since 1986, is a non-profit facility that serves seniors who need skilled nursing, rehabilitation, assisted living, memory care, or independent living. The center works to keep its environment friendly and homelike, with sunny courtyards, an open dining room, and lots of clean, bright spaces for people to gather. The design is modern, with accessible amenities and large areas for meals and social meetings, plus neighborhood-style community rooms. ACC Maple Tree Village, opened in 2020, adds 69 assisted living apartments and 30 Memory Care support apartments next to the main center, and ACC Greenhaven Terrace offers 166 independent living units. Since 2014, 24 assisted living one-bedroom apartments also provide dining, activity areas, and a beauty salon, so people get different support as their needs change.

The facility provides short-term rehabilitation after a hospital stay as well as long-term skilled nursing care, with 99 beds, including 14 certified beds in 2025, and stays focused on helping people recover and live as comfortably as possible. Nursing staff keeps a high nurse-to-resident ratio, and a Medical Director is onsite for regular visits. Residents receive help with daily living like bathing, dressing, and moving about, and those who need special care get wound treatment, medication management, and 24-hour supervision. For folks needing therapy, there's physical, occupational, and speech therapy onsite, along with support from a team trained in compassionate, culturally sensitive care.

Rooms come with private bathrooms, air conditioning, Wi-Fi, telephone, cable TV, and kitchenettes, while many offer patios or balconies. Shared amenities include libraries, fitness rooms, computer stations, a pool, and both indoor and outdoor community spaces for activities, movie nights, martial arts, Zumba, and arts programs. Dining features a main room and a cozy bistro, with meals adjusted for dietary restrictions and different cultures. Residents and families can voice concerns through councils, and a 24-hour call system keeps help close at hand. There's also community support through ACC Rides Transportation, events like the Fall Gala, and educational offerings like the Lifelong Learning & Wellness Program and caregiver classes.

The organization runs programs for seniors such as caregiver support, social activities, utilities education, and a job program for older adults. It provides non-ambulatory care, recreational activities, podiatry visits, and help with non-emergency transportation. Staff keeps the environment safe and clean, with pride in service, and programs encourage social connection and well-being. Medicare and Medicaid are accepted, and parking, housekeeping, and move-in help take some stress out of transitions.

ACC Care Center has earned a 5-star rating from the Center for Medicare and Medicaid Services since 2013 and has multiple awards, such as the AHCA Silver Award and a "High Performing" rating from U.S. News & World Report. Residents often describe the staff as patient, respectful, and professional. This community is not a CCRC (Continuing Care Retirement Community), but it does aim to offer a full range of care options so residents can get more or less help as needed, all within one supportive organization.
