    Friendly attentive staff and care

I'm very pleased with this facility - the staff are friendly and transparent, there's 24-hour care, free meds and physical therapy, and a strong staff-to-resident ratio. Rooms and common areas are clean, meals are hot with good variety, and activities (movie nights, bingo, outings) plus transportation and regular dental/vision checkups keep my mom happy.

    Summary of reviews

Reviews of Creekside Healthcare Center are highly mixed, showing a polarized set of experiences. Positive accounts emphasize a friendly, welcoming atmosphere, visible therapy services, and an active social program. Several families noted that the facility offers physical therapy, medication provision for some residents, transportation to appointments, and periodic dental and vision services. Social programming such as movie nights, bingo, and outings are described as meaningful engagement for residents, and a number of visitors reported clean, well-maintained common areas and an informative admissions tour.

At the same time, a number of operational weaknesses recur across reports. Clinical responsiveness is uneven: families describe delays in attending to medical issues, missed or delayed appointments, and instances where follow-up communication was inadequate. Medication-administration practices and adherence were described as inconsistent, and short-staffed shifts were frequently cited as a contributor to limited supervision and slower response times. These staffing and responsiveness gaps appear to affect both routine care and the facilityโ€™s ability to follow up after adverse events.

Dining and activities present a mixed picture. Some residents and families praised hot meals and a varied selection, while others described inconsistent food quality and gaps in dietary communication. Activity programming is available and meaningful for many residents, but staffing shortages were noted as occasionally limiting program delivery. Room accommodations range from adequate and clean to dated; there are both private and shared rooms, and reviewers flagged room maintenance and the presence of double-occupancy rooms as important considerations.

Facility management and administrative practices emerged as a notable area of concern. Complaints include long phone hold times, difficulty obtaining contact information, intermittent administrator availability on site, billing disputes, and confusion over Medicare or charge policies. Several reviews described serious allegations, including financial misconduct and sexual-misconduct claims; these are significant concerns that merit direct inquiry and verification through regulatory records and facility leadership.

Sanitation and resident comfort were reflected differently across reports: some families described an odor-free, clean environment, whereas others raised sanitation and odor concerns for specific areas. Privacy and roommate-management issues were also raised, including nighttime room entry and limited privacy protections in shared rooms. Safety and incident follow-up processes were questioned, with reviewers asking for clearer incident reporting and family notification protocols.

Given the variability in experiences, prospective residents and families should conduct an in-person visit, ask specific questions about staffing levels on each shift, medication-administration protocols, incident-reporting procedures, billing transparency and Medicare handling, room maintenance plans, and how the facility manages privacy in shared rooms. Reviewing state inspection records and asking for recent references from current families can help contextualize both the positive aspects (therapy, activities, some engaged staff) and the operational concerns (staffing consistency, clinical responsiveness, administrative transparency) found in these reviews.

    About Creekside Healthcare Center

Creekside Healthcare Center sits in the San Pablo area and offers comprehensive care for older adults who need help with daily activities or medical conditions, and they've really updated the place with recent renovations so you'll see furnished rooms, private bathrooms, air conditioning, and things like cable TV, Wi-Fi, and kitchenettes, and there's a garden outside and a few good spots to sit and relax. This facility has 8 certified beds as of June 2025 and provides both short-term and long-term skilled nursing care, and you'll find licensed staff on site like registered nurses, LPNs, and certified nursing assistants, all giving care 24 hours a day, seven days a week, with a focus on keeping patient ratios manageable to offer more personal help. It's a for-profit partnership, not a non-profit, and accepts Medicare and Medicaid, so most insurance works here, and you get access to things like IV therapy, wound care, in-house rehab, pain management, occupational and physical therapy, mental health support, and even memory care and post-surgery recovery.

Daily help includes bathing, dressing, eating, and transferring, and there's feeding assistance with plans set up for each resident. They have a registered dietitian coordinating with doctors for meals, which are prepared by professional chefs, and the kitchen serves food all day with different menu options for allergies or diabetes if needed. You won't find a resident council or family council here, and it's not set up as a continuing care retirement community (CCRC), so the services focus on nursing and rehab, not independent living steps. Creekside got flagged for abuse in the past year, and they continue to have inspections with tracking of quality measures, and policies are in place for infection control, especially around COVID-19 with a dedicated unit for that and set visitor rules.

There's a strong set of activities, both indoors and outdoors, with things like fitness programs, library time, movie nights, music, community activities, gardening, and special events, and staff help arrange these based on what each person likes. Creekside staff include social workers who give counseling to residents and their families, work on discharge planning, offer community referrals, and can do home assessments before or after a stay. Services also cover occupational, physical, and speech therapy, restorative nursing, psychological support, pain management, cardiopulmonary care, and orthopedic care. There's a 24-hour call system for safety, housekeeping, laundry, and move-in help for each person, and the facility tries to provide as much comfort as possible through tranquil outdoor spaces and amenities like on-site recreation and community-sponsored events.
