    Clean but unsafe, inconsistent care

I had a mixed but ultimately troubling experience. The facility is clean, welcoming, and offers activities, therapy and some caring staff, but care is inconsistent - nurses are often short-staffed, meds/getting residents up are sometimes missed, dietary communication is poor and food not always healthy, and I witnessed safety incidents and delayed medical attention with tragic consequences. Administrative and billing problems (Medicare/eviction threats, confusing phone contact, suspected overcharging/theft, privacy issues) mean I can't recommend it despite a few attentive employees.

    Summary review

Overall sentiment in the reviews is highly mixed, with a clear split between families and residents who have positive experiences and those who report serious problems. Multiple reviewers praise the facility's cleanliness, friendly atmosphere, visible activities, and availability of services such as physical therapy, dental and vision checkups, and transportation to appointments. Several comments emphasize that staff members they encountered were transparent, caring, and provided attentive, 24‑hour care. These reviewers also noted pleasant common areas, no offensive odors, and enjoyable programming like movie nights and bingo, which contribute to a positive social environment for some residents.

However, a substantial portion of reviewers describe significant and recurring concerns around staffing, clinical care, and management. The most common negative themes are inconsistent nursing care and understaffing: reviewers report short staffing on many shifts, skipped medications, lack of timely assistance to help residents get up, and general neglectful behavior by some nurses. These staffing problems are tied to tangible adverse outcomes in the reviews, including safety incidents (one report of a resident being struck on the head), hospitalizations for pneumonia, and even allegations that delayed medical attention contributed to a stroke and death. Such reports indicate variability in clinical vigilance and escalation procedures, raising concerns about the facility's ability to manage acute medical needs reliably.

Safety, ethics, and financial practices are other significant areas of concern. Multiple reviewers allege staff theft, overcharging, and improper handling of resident funds; there are also claims of privacy breaches with phone calls and deeply serious accusations that include abuse and sexual assault. Several commenters explicitly call for regulatory intervention or closure of the facility. In addition, families report difficult interactions with administration—long phone hold times, poor customer service, difficulty obtaining contact information, and administrators reportedly not being onsite or responsive. There are also reports of billing and Medicare disputes that escalated to threats of eviction, which raises red flags about financial transparency and residents' protection.

Dining and accommodations receive mixed feedback. Some residents enjoy the hot meals and a fair selection of menu options, while others describe the food as poor, unhealthy, or inconsistent with dietary needs. Communication between dietary staff and families is sometimes cited as a problem. Room quality is generally described as clean and adequate, but several reviewers note that rooms can be outdated, and double or shared rooms are common, which may not suit all residents or families seeking privacy.

Activities and rehabilitative services are strengths for some residents: reviewers mention movie nights, bingo, outings, and regular therapy services as positives that enhance quality of life. Conversely, operational problems such as missed doctor appointments, missed procedures, chaotic staff behavior, and apparent lapses in clinical coordination undermine these strengths for other residents.

For prospective families or those evaluating their loved one’s placement, the reviews suggest a need for cautious, thorough vetting. Key items to confirm in person include current staffing ratios and how the facility manages short-handed shifts; protocols for medication administration and clinical escalation; incident reporting and past licensing or complaint history; financial billing practices and safeguards for personal funds; and routine visibility of the administration/leadership. Ask for recent inspection reports, references from current families, and specifics about how the facility handles medical emergencies, transfers, and coordination with outside providers. Given the serious allegations reported by some reviewers, verifying regulatory status and complaint history with state agencies is advisable before making a placement decision.

    About Creekside Healthcare Center

Creekside Healthcare Center sits in the San Pablo area and offers comprehensive care for older adults who need help with daily activities or medical conditions, and they've really updated the place with recent renovations so you'll see furnished rooms, private bathrooms, air conditioning, and things like cable TV, Wi-Fi, and kitchenettes, and there's a garden outside and a few good spots to sit and relax. This facility has 8 certified beds as of June 2025 and provides both short-term and long-term skilled nursing care, and you'll find licensed staff on site like registered nurses, LPNs, and certified nursing assistants, all giving care 24 hours a day, seven days a week, with a focus on keeping patient ratios manageable to offer more personal help. It's a for-profit partnership, not a non-profit, and accepts Medicare and Medicaid, so most insurance works here, and you get access to things like IV therapy, wound care, in-house rehab, pain management, occupational and physical therapy, mental health support, and even memory care and post-surgery recovery.

Daily help includes bathing, dressing, eating, and transferring, and there's feeding assistance with plans set up for each resident. They have a registered dietitian coordinating with doctors for meals, which are prepared by professional chefs, and the kitchen serves food all day with different menu options for allergies or diabetes if needed. You won't find a resident council or family council here, and it's not set up as a continuing care retirement community (CCRC), so the services focus on nursing and rehab, not independent living steps. Creekside got flagged for abuse in the past year, and they continue to have inspections with tracking of quality measures, and policies are in place for infection control, especially around COVID-19 with a dedicated unit for that and set visitor rules.

There's a strong set of activities, both indoors and outdoors, with things like fitness programs, library time, movie nights, music, community activities, gardening, and special events, and staff help arrange these based on what each person likes. Creekside staff include social workers who give counseling to residents and their families, work on discharge planning, offer community referrals, and can do home assessments before or after a stay. Services also cover occupational, physical, and speech therapy, restorative nursing, psychological support, pain management, cardiopulmonary care, and orthopedic care. There's a 24-hour call system for safety, housekeeping, laundry, and move-in help for each person, and the facility tries to provide as much comfort as possible through tranquil outdoor spaces and amenities like on-site recreation and community-sponsored events.
