    Attentive staff excellent rehab recommended

I had a very positive experience - the nurses, CNAs and therapists were attentive, professional and genuinely caring; excellent PT/OT helped my grandfather recover and regain strength. The facility was clean and well-organized, food was good, COVID precautions and teamwork were impressive, and staff went above and beyond with communication and support. There were a few hiccups (slow call-light/phone response, occasional mixed staff performance and a missed room/wound issue that was addressed), but overall I highly recommend and would return.

    About Summerfield Healthcare Center

Summerfield Healthcare Center sits at 1280 Summerfield Road in Santa Rosa, California, where there's room for up to 70 people, though most days you'll see about 48 residents living there, and even though there's a nurse turnover rate of 42.6%, residents still get at least 4.65 nurse hours each day from licensed staff, and you'll find the nurses and caregivers on-site around the clock working under the watch of in-house physicians, and this place mainly offers skilled nursing care both for short stays and for long-term help. The place itself is run directly by Flagstone Healthcare North Inc. and is managed by The Ensign Group, and it's listed as Summerfield Healthcare Center-Post Acute Rehab, focusing on healthcare, licensed beds, and all the requirements you'd expect from a properly operating healthcare business. There's an executive director called Cason Bush, registered nurses like Janice Arends and Corina Hernandez, and it's described as a modern, state-of-the-art center with reliable technology and hands-on therapy right on the grounds.

Outdoors, the yard's kept nice with manicured landscaping, a groomed courtyard, and patio spots where folks like to sit when the weather's good. Inside, people get private or semi-private rooms, many with electric beds, phone and cable TV services, and some nice touches like comfortable furniture and art on the walls. Housekeeping and laundry keep things clean, and the kitchen serves up nutritious, satisfying meals three times a day, with menus designed to meet health needs. There are family gathering rooms where groups can spend time together, and accessible spaces so folks with walkers or wheelchairs can get around easily.

Care is pretty thorough, covering skilled nursing, wound care, dementia care, pain management, intravenous therapy, enteral feeding, and restorative nursing. They've got physical, speech, and occupational therapy all done in-house, along with a hospice program, respite stays for short-term help, and in-depth care plans made together with residents, their families, and the healthcare team. Infection control protocols are active across the facility.

On the activities side, residents can join music sessions, fitness activities, religious meetings, games, gardening, outside entertainers, volunteer opportunities, or group trips-there's a monthly calendar filled up from morning to night, and folks get a chance to stay engaged and meet other people. Family and visitors are welcome in the community's gathering rooms, and there's a friendly, welcoming atmosphere for everyone.

Summerfield, though modern and well-maintained, has had some quality of life and care deficiencies reported, such as not always following treatment orders or resident preferences, and issues have come up around resident rights like transfer and discharge procedures-these are important points to keep in mind. Still, the facility holds a 5-star overall quality rating from Medicare.gov at this time, which says something about the care most residents receive.

Overall, Summerfield Healthcare Center puts an emphasis on both strength and recovery, whether folks are there short-term after an illness or surgery or need longer care, and while it's not without faults, most residents find the care attentive and the surroundings comfortable enough to help them manage daily life with support.
