    Immaculate facility with excellent rehab

I placed my mother here for four years and have been very pleased. The nurses, CNAs and rehab team are caring and attentive - Oleg Ashkenazi (PT), Olga Senchea (OT) and Simone Davenport (LVN) helped her regain mobility so she's back to daily life. The facility is immaculate and well-run, makes great use of space with many patios and lounges, spa-like upgraded showers, engaging activities (Clair's events and candlelight dinners), excellent wound care and comprehensive OT/PT services. Staff and administration (Mr. Haas and his golden doodle Ruffles included) are compassionate, responsive and professional. I highly recommend this place for dignified, reliable care.

    Summary of reviews

Reviews describe a facility with a pronounced divide between strong rehabilitation and activity services and inconsistent nursing and room-level care. Many families praise the therapy teams (PT/OT) and specific therapists and nurses for measurable mobility gains, effective wound care, and a compassionate approach; the facilityโ€™s activity program and some amenities such as upgraded showers, visiting patios, and pet-therapy initiatives are also commonly cited as positive contributors to resident quality of life. Public spaces and some clinical areas are described as clean and well-presented, and several accounts note an accessible administration and engaged therapy leadership.

At the same time, a significant set of concerns recurs across accounts. There is notable variability in day-to-day nursing and aide performance: reports range from attentive, kind caregivers to staff who are slow to respond or appear insufficiently trained for complex clinical needs. Staffing shortages and workload pressures are described as contributing factors to delayed assistance, inconsistent hygiene and room cleanliness at the bedside, and irregular assistance with activities of daily living. Several narratives raise specific clinical-process concerns including inconsistent physician rounds, gaps in clinical monitoring and medication oversight, and dietary management issues that can increase aspiration risk for vulnerable residents.

Operational weaknesses are also highlighted. Communication with families and the facilityโ€™s complaint-resolution practices are frequently described as unsatisfactory; some reviewers cite difficulty obtaining clear explanations after adverse events. Transfer and mobility procedures have been questioned, with accounts indicating incidents during transfers and inconsistent use of safe-transfer protocols. Security and emergency-preparedness are raised as issues by multiple sources, and language barriers among staff create occasional communication challenges. Additional practical concerns include inconsistent handling of resident belongings and potential infection-control vulnerabilities related to room-sharing and testing practices.

Taken together, the pattern suggests a facility that offers strong rehabilitative programming and meaningful activities but presents inconsistent performance in core nursing, safety, and operational oversight. Prospective residents and families would likely benefit from direct inquiries prior to admission: ask about current staffing ratios (day and night), specific clinical protocols for transfer safety and aspiration prevention, physician rounding frequency, infection-control and roommate-testing policies, language-capability support, and the facilityโ€™s process for handling family concerns and clinical incidents. Observing mealtime service, visiting common areas, and meeting key therapy and nursing staff can help assess whether the facilityโ€™s strengths meet an individual residentโ€™s clinical and personal needs.

About Sherman Oaks Health & Rehab

Sherman Oaks Health & Rehab sits at 14401 Huston Street in Sherman Oaks, California, and offers skilled nursing care, post-acute care, and rehabilitation services, so people coming here can expect help from a team that includes nurses, physicians, and others like Miguel Baltazar, who works as an Admissions Coordinator, Elba Rodriguez, who's a Registered Nurse, and A.J. Bray, a Patient Relations Coordinator; they're part of a group that values working together with each resident and their family to try to improve health in a physical, emotional, and sometimes spiritual way, considering needs as they change because that's all part of their approach. The facility's rules protect people using housing choice vouchers or different sources of income, and there are protections in place for LGBTQ individuals, including not allowing discrimination against residents based on gender identity or orientation, which makes things more comfortable for people from different walks of life. Sherman Oaks Health & Rehab runs with a focus on hospital and health care services, has licensed staff, tries to learn and improve, and, being a privately held business with about 11 to 50 employees, gives each person staying there some personal attention, though not everything about the building's layout, parking, and amenities is known, and the property's not currently for sale or rent. The facility offers various apartment rentals, and health and rehabilitation programs, but details about each apartment or interior feature aren't really clear at this time. The focus seems to be on providing care, good communication between staff and patients, and keeping an adaptable, respectful community.
