    Kind staff, unsafe systemic problems

I had a mixed and ultimately disappointing experience. Many nurses, therapists and the receptionist were kind, professional and went above and beyond-helping my family member improve and keeping the room clean and comfortable-but care was wildly inconsistent. The facility is short-staffed, communication is poor (doctors show up unpredictably and staff are often uninformed), food and laundry services are substandard, and there were unsafe lapses like catheter neglect and delayed consults. Because of those systemic problems I cannot recommend this facility.

About Valley of the Moon Post Acute

Valley of the Moon Post Acute is a 27-bed licensed skilled nursing facility run by Sonoma Valley Hospital, and people there speak both English and Spanish so it's good for folks from different backgrounds. The place mainly helps people who need care after a hospital stay but aren't ready to go back to living on their own just yet, and that's often the case after surgery, illness, or injury. It's set up to be fully accessible for people who use walkers or wheelchairs, so there's no trouble getting around, and you can find both private rooms and studio apartments, plus companion rooms for those who don't mind sharing. Meals get served in a dining room with options for special diets, like if someone needs meals for diabetes or allergies, and you can eat all day or sit down for restaurant-style service, which can make things feel cozy and normal. There's an emergency call system in each room, and caregivers, nurses, and even doctors are on-site and available around the clock, so if something comes up you don't have to wait for help. Services happen by appointment, and you can get therapy for physical, occupational, and speech needs along with respiratory therapy, and they'll set you up with disease management too.

For medical support, you get care from a full team, including a psychologist, social worker, registered dietitian, palliative care nurse, wound care nurse, and infection control nurse, and there's support for medication management and daily living activities like bathing and dressing. Medical imaging such as MRI and CT, plus IV antibiotics, transfusions, and surgical services, are available if you need them. There's an option for both short-term and long-term rehabilitation, and there's an intensive care unit and emergency services if your condition gets more serious. The facility also offers things like art programs, worship services, movie nights, cards and games, and even outings like patio social events, plus transportation and daily housecleaning which helps residents focus on recovering or just living. High-speed Wi-Fi and a telephone are in the rooms, so folks can keep in touch. There's even education and support groups for residents and the community. Shared and private rooms, furnished with the basics, let you choose how you want to live, and a 24-hour call system means someone's always listening for needs.

Outpatient care helps people who don't need the full hospital setup, with care plans for those still transitioning to independence and even a discharge plan for when you're ready to go home. Non-ambulatory care is there for folks who can't get around by themselves. Insurance, including Medicare and Medicaid, is accepted, which helps cover care. Valley of the Moon Post Acute focuses on rehabilitation with physical, occupational, and speech therapy, wound and respiratory care, and support for people who've had joint replacements or strokes. There's a strong team of professionals, including on-site doctors who help make decisions with nurses, therapists, and clinicians, and support goes day and night so no one has to wait for help. US News and World Report gave it a top overall rating of 5 out of 5, and it's rated "high performing" for short-term rehab, which means people often recover well there. The beds are always ready for new patients from Sonoma Valley Hospital, and reports say the quality of care is high, with patients being satisfied with the safety and support. In all, Valley of the Moon Post Acute offers skilled nursing care, therapy, and support to help people recover, regain independence, or live safely for the long term.
