    Compassionate staff helped dad recover

I placed my dad here and was grateful for the compassionate, respectful care - attentive nurses and CNAs (Leenah, Jojo, Viviana Perona), a friendly front desk (Olivia), and a top-notch PT/OT rehab team who helped him regain mobility and meet his goals. The facility was clean, homey, with good food, excellent communication and discharge planning, and staff who truly made him feel like family; I would recommend them.

    Summary of reviews

Overall impression: Reviews of Menifee Lakes Post-Acute are mixed, with a clear pattern of strong rehabilitation and several highly praised direct-care staff, alongside recurring operational and administrative shortcomings. Families and clinicians frequently highlight effective PT/OT services and individual caregivers who provide attentive, compassionate care; at the same time, reviewers describe systemic issues that can affect safety, continuity of care, and family confidence.

Care quality: The facility receives consistent praise for its rehabilitation services and for certain nurses and CNAs who demonstrate strong clinical skill and compassion. Several families credited the therapy team with meaningful functional gains and successful discharges home. Conversely, reviews indicate inconsistent clinical reliability: delayed or missed medications, lapses in following physician orders, inconsistent wound monitoring, and examples of inadequate monitoring after acute events. Telemedicine is used for some consults; while convenient in some cases, reviewers expressed concern when it appeared to substitute for timely in-person assessment. There are also notable concerns about end-of-life communication and follow-up after significant clinical events.

Staff and responsiveness: Many reviewers singled out individual staff members and teams as caring, responsive, and helpful—front-desk liaisons and particular nurses/CNAs were often named. However, a recurring theme is variable staff responsiveness tied to staffing levels and morale: long response times to call lights, delays in toileting and personal care, and a perception of overworked or inattentive staff. Communication tone with families and a small number of case-management interactions were described as abrasive or money-driven, contributing to family dissatisfaction.

Dining and nutrition: Opinions on dining are inconsistent. Some reviewers praised the food and the efforts of a dietitian, while others found meals bland, inconsistent, or not aligned with dietary needs. Nutrition planning appears to exist, but execution and accommodation of preferences or restrictions were uneven according to accounts.

Activities and facility environment: Positive comments emphasize bright, home-like common areas, seasonal activities (wreath-making, holiday décor), and well-kept rooms in portions of the building. The facility offers single rooms and some recently updated finishes. At the same time, other areas are described as older or overcrowded, with small shared rooms, noise issues, and sanitation or odor concerns in certain units. Laundry handling and the tracking of personal belongings were also noted as problematic in multiple accounts.

Management and administration: Administrative strengths include helpful discharge-planning staff and responsive front-desk personnel in certain cases. Recurrent administrative weaknesses include billing errors, unauthorized renewals, unanswered phone calls, and failures to notify families promptly about clinical changes. Infection-control practices and COVID cohorting/communication were questioned by reviewers, and some families reported inconsistency in transparency about infection status. A few reviews noted improvements after leadership changes, indicating management may be uneven over time.

Notable patterns and advice: The facility appears to offer strong rehabilitation outcomes and has several standout caregivers, which may make it a solid choice for short-term post-acute rehab when therapy intensity and individual staff assignments are favorable. However, families considering longer stays or residents with complex clinical needs should clarify key operational areas before admission: staffing ratios and typical response times, medication-administration processes, wound-care and pressure-ulcer prevention protocols, infection-control procedures, transportation for outings, laundry and belongings management, and methods for timely family communication. Asking for written policies, recent inspection results, and the names of primary clinical contacts can help set expectations and mitigate some of the persistent concerns noted in reviews.

    About Menifee Lakes Post-Acute

Menifee Lakes Post-Acute sits at 27600 Encanto Drive in Sun City, California, and has room for 99 people, though it usually cares for about 87 folks each day, and it does this with nursing staff who average 4.34 hours per resident daily, which is just under the state average. This place is a for-profit limited liability company connected to Thomas Chambers and David Johnson, and it's licensed as a skilled nursing facility, which means it's set up to help people who need both short-term rehab after a hospital stay and those very frail folks who are mostly dependent on nursing care. They keep nursing and rehab services running every hour of the day, seven days a week, and they bring in clinicians, therapists, and medical consultants to check in on health and recovery plans for whoever's staying there.

They serve three meals each day and snacks, but inspection reports show some problems with the diet and infection control, including one about not meeting standards for a nourishing and balanced diet, and another about infection prevention. In total, state inspections have found 54 different deficiencies, including 5 that deal with infection control, and one about not having a licensed pharmacist managing pharmaceutical services. Even so, the place runs a free CNA school and tries to stick to a "patients first" mindset, and the team leans on a patient-centered approach for both care and rehab. People living here can get help from speech therapy, physical therapy, occupational therapy, IV therapy, and outpatient rehab. There are also services for people dealing with mental illness, and there's an effort to make the environment feel secure while letting people keep their independence as much as possible.

Every resident gets a care plan built around what they need, with a focus on getting back to the highest level of independence if that's possible, and staff will meet with families to talk through what will work best. Menifee Lakes Post-Acute tries to offer social services that cover both medical and everyday concerns, with activities planned that fit what a resident can do, and the team works on both social and clinical needs, so there's a mix of safety and autonomy. The nurse turnover rate sticks close to the state norm, at 39 percent, and they take payments in many forms like Medicare, Medicaid, private insurance, VA benefits, and regular cash or checks. They've always got staff there no matter the hour, and they welcome visits from families thinking about moving in a relative.
