    Attentive staff, effective rehab, reassuring

I placed my mother here and have been very pleased - the staff are attentive, professional, and genuinely caring, and the rehab/physical therapy helped her regain strength. The facility is clean, secure, and offers engaging activities and good meals; leadership is responsive and welcoming to families. We have peace of mind and I would recommend this place.

    Summary of reviews

Reviews present a mixed, often polarized picture of care at Astoria Nursing and Rehabilitation Center. A recurring positive theme is the presence of compassionate, attentive caregivers and effective rehabilitation services: many families described successful therapy outcomes, engaged physical-therapy staff, and individualized rehab plans that led to measurable improvement. Several accounts praised on-site medical providers, prompt maintenance, secure entry procedures, and an active recreational program that includes music and outdoor activities. Leadership and administrative responsiveness received specific positive mention from families who felt supported and involved during admission and care planning.

At the same time, there are consistent concerns about operational reliability and consistency. Reviewers describe variability in staffing levels and marked differences between day and night shifts, which appears to affect responsiveness to nurse calls and timely assistance for basic needs. Several accounts raise clinical-process issues such as delayed medication delivery, gaps in pain management, and problems with timely repositioning and skin-integrity care. These items suggest inconsistencies in clinical oversight and shift coverage rather than a uniform standard of care across the facility.

Dining and facility observations are similarly mixed. Some family members reported excellent food service and the ability to customize menus, while others described bland or repetitive meals and limited vegetarian options. The physical environment draws both praise and critique: many reviews describe clean, well-maintained common areas and a pleasant atmosphere, yet others note aging infrastructure, outdated room furnishings, occasional odor concerns, air-conditioning failures, and overcrowded multi-bed rooms that limit privacy.

Communication and operational processes are additional areas of concern. Multiple reviewers mentioned missed or rescheduled therapy appointments, early discharges that felt premature to families, lapses in family communication, and occasional problems with supply and equipment handling (including respiratory equipment and shipments). There are also statements reflecting worries about staff conduct, unprofessional behavior, and inconsistent adherence to documentation and patient-belongings procedures — items that point to variability in staff training, supervision, and accountability.

Taken together, the pattern suggests that Astoria has several institutional strengths—particularly in compassionate caregiving, rehabilitation services, and leadership responsiveness—but also notable variability in execution across shifts and units. Prospective residents and families should consider an in-person visit and ask targeted questions about staffing levels by shift, medication-administration protocols, skin-integrity and incontinence-care practices, rooming/density options, therapy scheduling and discharge planning, and the facility’s processes for equipment handling and staff vetting. These specific inquiries can help clarify whether the facility’s strengths align with an individual resident’s clinical and comfort needs.

About Astoria Nursing and Rehabilitation Center

Astoria Nursing and Rehabilitation Center sits in Sylmar, California and has been around since 1960, so they've been taking care of folks for quite a while and they run things with a focus on safety because the place is gated and there's parking for cars if you need it. The facility is licensed by the Health and Human Services Agency under license number 920000004, and Ms. Grace Marcado owns and manages the place herself. People sometimes call it Astoria Convalescent Hospital or Astoria Nursing & Rehab, but it's all the same hospital, made to give skilled nursing and rehabilitation for up to 218 people at once, which is a pretty good size. The staff gives care for wounds, dementia, and there's help for folks who need palliative or hospice care, so people can get support for all sorts of needs. Rehabilitation services happen every day, seven days a week, and the goal is to help each person get as strong and independent as possible. Residents get three meals a day, and you can have a guest join you for a meal, plus they make sure any special diets are handled by the kitchen staff at all hours. There are nice touches, like a garden outside for fresh air, a hot tub spa to relax in, and activities to keep busy minds engaged. The staff tries to treat everyone like family, keeping things friendly, and there's a home-like feel to the place, with people who value dignity, comfort, and helping residents feel at home while they recover or live there.
