    Clean, homey facility; excellent care

I'm very pleased with Cottonwood - a clean, homey facility with private, spacious rooms, fresh homemade meals, and plenty of activities. The staff are professional, friendly, and attentive; nursing and therapy were excellent and my family member felt safe and well cared for, so I'd recommend them for rehab or long-term care.

    Summary of reviews

Overall impression: Reviews portray Cottonwood Rehabilitation and Healthcare Center as a clean, well-maintained, and home‑like skilled nursing facility with repeatedly praised caregiving teams. Families and residents frequently highlight compassionate nursing and therapy staff, an above-average resident-to-staff ratio, and a welcoming atmosphere. The facility's physical environment — private, roomy rooms, warm décor, a private courtyard and pet-friendly visitation — is described positively, and infection-control practices received favorable mention.

Care and clinical services: The facility is positioned as a viable option for both short-term rehabilitation and longer-term skilled nursing. Physical therapy resources (including a therapy pool) and proactive therapy teams are called out as strengths, and many accounts describe measurable recovery progress. That said, outcomes are not uniformly described; reviewers indicate variable rehabilitation consistency and effectiveness across residents. Nursing teams are generally characterized as professional and caring, but there are recurring concerns about inconsistent responsiveness to resident requests and periods when direct-care staff appear distracted or insufficiently engaged.

Dining and activities: Dining is frequently described as homemade, fresh, and customizable, with several mentions of high food quality. However, reviewers also note operational issues with meal delivery timing and occasional service delays. The activity program is noted as active and varied — crafts, outings and social programming contribute to the facility’s home-like feel — though some families would like more detail and communication about scheduled offerings.

Facilities and management: The facility’s cleanliness, layout and aesthetic appeal are consistently praised. Management receives mixed feedback: many reviews commend proactive communication and strong leadership that addresses concerns, while others raise allegations of poor management–staff relations and low staff morale. These personnel issues are presented as concerns that could affect continuity of care if not addressed. Communication about care plans, amenities and daily routines is generally good but has occasional gaps according to reviewers.

Notable patterns and considerations for families: Strengths center on compassionate staff, a comfortable environment, and robust therapy resources. Recurrent operational themes to probe further when evaluating the facility in person include consistency of staff responsiveness across shifts, meal-service schedules and contingency plans for delays, the variability of rehab outcomes, and current staff morale or management–staff relations. Prospective residents and families may benefit from asking about current staffing ratios, supervision of direct-care aides, therapy scheduling and outcome tracking, and examples of how management addresses staff concerns and maintains communication with families.

About Cottonwood Rehabilitation and Healthcare Center

Cottonwood Rehabilitation and Healthcare Center sits in Durango, Colorado, just east of the Mercy Regional Medical Center complex, and folks know it for providing short-term rehabilitation and long-term skilled nursing care in a small, hometown setting with 40 certified beds. The staff uses specialized therapy services and state-of-the-art technology to help people with different needs, whether they're recovering from the hospital or dealing with conditions that make them dependent on nursing. They offer programs in cardiac and orthopedic rehabilitation, pain and diabetes management, wound healing, pulmonary care, and more, always aiming to help their guests feel comfortable and safe each day. There's a focus on collaboration with residents and families; they try to treat everyone like family, and people can see that through their approach to care and community connection.

Cottonwood serves as a VA contracted facility, so veterans can use their benefits here, and the team works to help guests get back home through a full-service rehab program. Skilled nursing staff provide expert care, and the building has amenities meant to create a warm and welcoming feel. They also provide specialty services for behavioral health along with regular intermediate and long-term care. Nurse turnover is 48.6%, which is lower than the state's average, and reported nurse hours per resident are above the state average, too. You'll hear that management has been led by Refoel Gottlieb since September 2022, and the owner structure is set up as a for-profit partnership, managed through Centennial Colorado Holdco, and the current administrator is Christy Bradley. The facility has had some documented deficiencies, including quality of life and care issues related to behavioral health and infection control, but these were rated as having no actual harm, though there's always a chance for more if they're not watched. Staff and leadership do their work within the standards required by their membership in the Colorado Health Care Association District VI.
