    Compassionate, organized care; highly recommended

I'm very grateful for the compassionate, professional staff at Pine Ridge - nursing, rehab, housekeeping and kitchen were attentive, the facility was clean and welcoming, and volunteers brightened our visits. Tanya's PT and Patricia in OT were outstanding and helped my mom recover and go home comfortable, neat, and well cared for. Overall a top-notch, organized place with kind caregivers; I'd recommend it while acknowledging there's always room for small improvements.

    Summary of reviews

Overall impression: Pine Ridge Rehabilitation & Healthcare Center is described by many families as a facility with strong interpersonal care and effective rehabilitative services. Reviewers frequently highlight staff kindness, individualized attention, and successful therapy outcomes that enabled residents to return home. Cleanliness, organized operations for admissions and discharge, and a generally welcoming environment are commonly praised.

Care and clinical services: The facility receives consistently positive comments about nursing and therapy teams. Physical therapy is identified as a particular strength, with clinicians praised for skill and knowledge and for producing measurable rehabilitation gains. Occupational therapy and caregiver interactions are described as supportive, and hospice and end-of-life care are noted as compassionate and attentive. These accounts indicate generally effective clinical pathways for short-term rehab and palliative needs.

Staffing and operations: Despite the strengths above, there are recurring operational concerns related to staffing. Families described periods of insufficient coverage during peak times, inconsistent responsiveness from certified nursing assistants, and a high turnover that can disrupt continuity of care. Comments about administrative responsiveness are mixed — some visitors commend leadership efforts, while others feel operational issues remain. These patterns suggest management attention is needed to stabilize staffing and improve consistency across shifts.

Dining, housekeeping, and facilities: Housekeeping and dining teams receive favorable remarks for keeping residents well-groomed and rooms tidy; food quality is described as appealing by several families. The facility is characterized as clean, organized, and pleasant in appearance, and affordability is noted by some visitors.

Activities and family communication: Activity programming is an area of improvement. Reviewers mentioned limited engagement options during midday hours and a desire for more scheduled activities and volunteer interactions to brighten residents' days. Families also expressed interest in enhanced remote communication tools (for example, room webcams or other monitoring/communication options) to stay connected when away.

Notable patterns and recommendations: The dominant positive themes are compassionate staff, effective therapy services, and a clean, organized facility that supports grooming and discharge goals. The dominant concerns are operational: staffing shortages, turnover, inconsistent CNA responsiveness, and limited activity programming. Prospective residents and family members should weigh the facility's strong therapy and hospice offerings and clean environment against potential variability in day-to-day staffing and engagement. Visitors considering Pine Ridge may wish to discuss current staffing levels, activity schedules, and family-communication options with administration during tours to assess how the facility's strengths align with their priorities.

About Pine Ridge Rehabilitation & Healthcare Center

Pine Ridge Rehabilitation & Healthcare Center sits in Pagosa Springs, Colorado, and you'll find it's surrounded by the San Juan Mountains, which makes for a peaceful place to stay especially if you like mountain views and simple beauty from your window. This center has 60 certified beds, and it's licensed to provide skilled nursing and both short-term rehab, often for people going from hospital to home, and long-term care for folks who are very frail and need daily help. Pine Ridge does focus on some specific types of help, offering memory care for people who have troubles with remembering things, along with orthopedic rehabilitation for folks who need therapy after surgery or injury, and wound care which is important for some people who need extra help to heal. They say they give "genuine hometown care," and you'll notice a warm and welcoming atmosphere, where they try to treat residents, guests, and staff like family, which people do appreciate when they're away from home or needing a little more support.

There's a team of skilled nursing staff, and the place uses special therapy programs plus some modern technology for both assessment and rehabilitation, so if you or your loved one needs ongoing therapy, or care for wounds, they've planned for it. The facility is a for-profit corporation, privately held, and affiliated with Centennial Colorado Holdco, with Mitch Mitchell listed as the administrator, which can be good to know when you want to put faces to names. The campus has amenities designed for resident comfort and well-being, including ways to help people with dietary management, and they also offer virtual tours so you can look around online first if you're curious about what you'll find there.

You will notice, though, that Pine Ridge has had some inspection reports that show areas for improvement. For example, a standard report dated January 10, 2024, listed 4 deficiencies, with a total of 18 deficiencies noted in past inspections. These include issues like pharmacy services marked as F0758 because of the need to address how medications are managed, nutrition and dietary standards listed as F0812 with some impact on residents, and resident assessment and care planning (F0658) with a severity that suggests there's room to be safer. There were two infection-related deficiencies as well, so it's clear there's attention on following the right infection control practices.

If you need a place that's prepared for specialized care-memory care, wound care, orthopedic rehabilitation-or somewhere with a reputation for trying to offer a safe, home-like environment, Pine Ridge does offer these things, and you can't help but find it has a focus on both comfort and specialized services, while still keeping an eye on where they might do better. The facility has a simple website at pineridgehc.com, which helps with checking updates or learning about specific programs, and they operate with between 51 and 200 employees, so it's a medium-sized team looking after the residents there. All in all, Pine Ridge offers skilled care in an area known for its mountain scenery, with a strong focus on supporting long-term needs and rehabilitation, even as they keep addressing areas that matter most for health and safety.
