    Caring staff restored my strength

I've had a very positive experience - caring, professional staff treat me like family, the facility is clean and welcoming, meals and activities are excellent, and nursing/therapy helped me regain strength and manage my Parkinson's. Residents are friendly and staff go above and beyond; a few minor things could be tightened up, but I feel respected, comfortable, and eager to return.

    Summary of reviews

Reviews of Trinidad Rehabilitation and Healthcare Center present a mixed but instructive picture. A strong and recurring strength is the facilityโ€™s direct-care staff: many accounts describe compassionate, attentive, and highly engaged nurses, aides, and therapists who provide personalized support and create a family-like atmosphere. Therapy and rehabilitation services are frequently praised for helping residents regain strength, and the facility receives positive mention for dementia care and professional clinical staff. The environment is commonly described as clean, well-decorated, and welcoming, with holiday meals, social events, and pet visits contributing to a warm community feel.

At the same time, there are recurring operational concerns that prospective residents and families should evaluate. Clinical responsiveness appears inconsistent: reviewers described delays in respiratory treatments and other medication or treatment delivery, implying gaps in treatment coordination and medication supply management. These concerns are expressed alongside isolated but impactful operational lapses such as admissions/amenity setup issues and missed preparation for scheduled medical services. Together, these point to variability in how clinical processes are managed from shift to shift.

Dining and nutrition receive mixed feedback. Special meals and holiday dinners were highlighted positively, and the facility can produce well-executed events. Conversely, there are examples of delayed meals and variable meal quality, indicating unreliable meal-service continuity that may affect residentsโ€™ daily experience. Families should ask about meal timing, tray delivery protocols, and how dietary concerns are handled.

Activities and social programming are a clear strength. Reviews emphasize engaging programming (trivia, themed events, hot chocolate socials), volunteer involvement, and staff who go beyond basic care to create meaningful resident experiences. The physical facility is generally described as clean and comfortable, with instances of thoughtful accommodations (for example, furniture covers and decoration for holidays) contributing to a pleasant atmosphere.

Management and administrative performance are an area of noted variability. Some reviews express strong confidence in management and operational planning, while others criticize leadership and business practices. This divergence creates a polarized pattern of resident experience: many families describe excellent care and satisfaction, while a subset raise serious operational concerns. Prospective residents and families would be well served to observe staffing patterns, ask about medication- and treatment-administration protocols, review admission and amenity-check procedures, and request examples of how the facility handles service lapses and family communication.

Overall, Trinidad Rehabilitation and Healthcare Center is characterized by dedicated frontline staff, robust activities, and a clean, welcoming environment, paired with intermittent operational weaknesses in clinical responsiveness, meal-service reliability, and administrative consistency. Those considering the facility should weigh the strong interpersonal and programmatic strengths against documented variability in process-driven areas and perform targeted due diligence during tours and admission discussions.

About Trinidad Rehabilitation and Healthcare Center

Trinidad Rehabilitation and Healthcare Center sits in Trinidad, Colorado, and you can see the Sangre de Cristo Mountains from some windows, which gives a bit of comfort for the people staying there, so it's nice for looking out during the days. This facility operates under the management of Refoel Gottlieb since September 2022, with direct ownership by Centennial Colorado Holdco LLC, and affiliations that include several indirect owners and the Centennial Colorado Holdco group, and the administrator on site is named Stacie Currie. Trinidad Rehabilitation and Healthcare Center is a for-profit skilled nursing facility and is also part of CHCA District VI, and it provides a lot of different care options, maybe more than you see in some other places, like long-term care or skilled nursing for people who really can't get by without daily help, short-term rehabilitation for folks coming out of a hospital, and different levels of assisted living, independent living, adult day services, memory care for dementia, hospice care, and both non-medical home care and Medicare-certified home health care for those who need help but want to stay at home.

The center has amenities set up for residents who need long-term care, so it's got certified beds and daily capacity for the folks living there, and they do try to create a warm and welcoming environment even though it's a medical place. Staff includes skilled nursing staff, and there are all sorts of therapy programs using modern technology for recovery, especially in memory care, cardiac rehabilitation, and orthopedic care, and the idea is always to help residents recover and go back home if they can. Dietary services, nursing care, and infection control measures are part of the routine, and they say they're focused on making a difference for their residents, wanting to treat everyone like they're part of a family, which is something people like to hear.

However, the Trinidad Rehabilitation and Healthcare Center doesn't have high ratings. Based on the most recent inspection on June 5, 2024, they've been rated much below average overall, and that's for overall quality, health inspections, staffing, and quality measures. The center had 9 new deficiencies found during its June 2024 inspection, and altogether it's got 37 deficiencies, including one related to infection control, which means there have been problems with protecting residents from the spread of infections. Other recent deficiencies include failing to protect people from abuse and neglect, not following nutrition and dietary guidelines for food preparation and storage, and not observing nurse aide job performance or providing regular training, which didn't cause actual harm but created a potential for more than minimal harm to several residents. Reports document these issues, and the center is currently looking for more skilled staff to help care for residents and improve their services.

People come here for healthcare and rehabilitation, and the center wants to be a place where people can recover and feel at home, but it's important to know about the low inspection ratings and the range of deficiencies that were found. The facility does have special therapy and care services, and it covers lots of different needs, including those for people who are very frail or need more support as they get older.
