    Compassionate attentive care, excellent rehab

My father received compassionate, attentive care here - nurses and CNAs had great bedside manners, therapists were amazing with his rehab, and the front office made Medicare/Medicaid and discharge arrangements easy. The facility was very clean, staff friendly and professional (special thanks to Angelica, Nina and Administrator Turner), and I felt genuine peace of mind. I would recommend this center for short- or long-term care.

    Summary of reviews

Reviews present a mixed picture: the facility receives frequent praise for its therapy services and for individual caregivers, social workers and some administrative staff, while also drawing repeated concerns about operational consistency. Many families and former residents highlight excellent physical and occupational therapy, attentive wound care, effective discharge planning and staff members who go above and beyond. Admissions and front-office personnel are often described as helpful, and several accounts specifically commend social-work support and Medicare/Medicaid navigation.

Care quality and staff performance appear uneven. Positive accounts emphasize compassionate nurses, skilled CNAs and therapists who spend time with patients. Conversely, other reports describe inconsistent professionalism, delays in responding to call lights or pain requests, problems with transfer assistance, and instances of medication- and clinical-management breakdowns that required hospital transfer. Reviewers raised concerns about the facility’s ability to maintain consistent supervision and timely clinical oversight across shifts.

Dining and activities are similarly variable. Some reviewers note improvements under new kitchen management, courteous dining staff and acceptable meals; others report cold meals, difficulty obtaining dietary accommodations, and food-quality problems. Activity programming is present but limited in scope for certain residents — reviewers referenced few off-site outings and inconsistent engagement opportunities for long-stay residents.

Facility condition and environmental controls are inconsistent across accounts. Several reviewers describe clean, well-maintained areas and effective housekeeping; others describe sanitation concerns, pest-control issues in some units, malfunctioning equipment (for example bed controls), exposed wiring or other maintenance deficits, and uncomfortable/old furnishings. Emergency situations and infrastructure problems (extended heating outages, power events) highlighted gaps in preparedness and continuity, though a few reviewers noted staff responsiveness during power disruptions.

Management, communication and administrative practices show notable divergence. Positive notes include proactive admissions tours, helpful social workers, and staff who assist with insurance navigation. However, many families reported poor telephone responsiveness, long hold times, difficulty obtaining medical records, missing personal property or laundry mix-ups, and allegations of problematic billing or insurance-handling. A small number of accounts describe serious post-incident concerns (including hospitalizations and infection-related complications) and unresolved questions from families; there are also isolated allegations concerning mail/property handling that families should verify.

Overall pattern and recommendation: Advanced Care Center demonstrates clear strengths in rehabilitation services and in individual caregivers who provide compassionate support. At the same time, recurring operational issues — notably inconsistent staffing conduct, responsiveness, environmental maintenance, communication systems, and documentation practices — produce variable resident experiences. Prospective residents and families should arrange an on-site visit, ask specific questions about staffing levels and night-shift practices, review incident-reporting and medication-administration protocols, inspect rooms and recent maintenance records, confirm dietary and laundry procedures, and request written emergency-preparedness and property-handling policies. Checking recent inspection reports and speaking with current families can help clarify how consistently the facility sustains the positive aspects noted in these reviews.

    About Advanced Care Center

Advanced Care Center sits in Clearwater, Florida, as an average-sized nursing home with 120 certified beds and about 113 residents daily, and lately, new management has taken over, which might mean some things may change when it comes to standards or routines there, and folks will find that the facility has a secure memory care unit and offers a wide list of services like medication management, speech therapy, wound care, tracheotomy care, amputee recovery, IV antibiotic therapy, dentistry, podiatry, pain management, physical, occupational, and recreational therapy, respiratory therapy, mental health support, diabetes management, nutritional counseling, pharmacy and lab work, x-rays, optometry, and more. The place holds a B+ overall grade with an A+ on recent inspections, though there are documented deficiencies noted in reports, things like resident care planning, screenings for mental health or disability, and maintaining a clean environment, with no actual harm found but some had potential for more than minimal harm, so you get the good with what still needs work. This center allows both Medicare and Medicaid and has inpatient rehab, post-acute, respite, and hospice care, along with having registered nurses, LPNs, CNAs, therapists, and other medical staff available with a daily average of 3.51 nurse hours per resident, just below the state average, plus a relatively high nursing turnover rate around 54.8%. There are useful property amenities like dining rooms and activity rooms, a fitness center, salon/barbershop, kitchens, a garden, guest parking, housekeeping, laundry services, and even Wi-Fi, cable TV, and air conditioning in both private and semi-private rooms with some featuring kitchenettes and private bathrooms. Residents get support with walking, wheelchairs, bathing, dressing, toileting, meals, laundry, medication, wound care, and therapy, and there's a strong focus on tailored care plans where staff-doctors and therapists-adapt treatment for each person, whether it's physical therapy to boost strength, palliative care to ease symptoms, or specialized help in areas like respiratory or complex wound needs, and when someone needs extra help, personal care assistants offer attention all round the clock through the 24-hour call system and supervision. The atmosphere leans friendly and supportive, with knowledgeable staff who try to make things comfortable, and the place stays busy with most beds full almost all the time. Meals come prepared daily, with nutrition managed by a professional chef, and there's always a mix of scheduled activities, arts and crafts, social programs, health and wellness offerings, movie nights, gardens for sitting, game rooms, and outings. Diagnostic and specialty medical services cover needs right on-site, from labs to imaging and urgent care. Families can schedule tours or virtual visits, read up on inspection records and pricing, and request initial consultations to discuss specific care needs or get info on Medicare, Medicaid, or long-term care insurance. The facility has an open-door policy for licensed clinicians working shifts, and a network of certified, experienced staff covers all shifts around the clock. The Advanced Care Center emphasizes health care and safety but doesn't shy away from showing inspection scores or discussing where they've got room for improvement, and while you won't find perfection, you'll see plenty of opportunities for full-service medical and personal care-especially for seniors with complex needs or those recovering from illness or surgery. The facility has both resident councils-meaning residents can discuss concerns or ideas-and runs community outreach and wellness classes as part of its wider approach to care. The place connects well to area hospitals too, making transfers or emergency visits quick if they're needed. Residents can expect help with their individual goals, a supportive community, and an environment that tries to balance medical attention with comfort, privacy, and the daily things that help a person feel at home.
