    Caring staff, unsafe, disorganized facility

I had a mixed experience. Many CNAs, nurses, PT staff, and folks like Seth, Casey and wound nurse Emily were caring, professional, and kept residents active, but chronic understaffing - especially nights and weekends - led to missed meds, poor third-shift coverage, safety incidents, and limited shower/bathroom assistance. Administration was unresponsive and disorganized with conflicting information, laundry and medication mistakes, and building maintenance issues (AC leaks, poor TV, rundown appearance). Food and housekeeping declined (cold/slopped meals, small portions, missed cleanings), which made the place feel unsafe and untrustworthy despite excellent front-line staff.

With support from Blue Palms Healthcare Management, the facility tries to run efficiently and comply with all regulations, though recent inspection reports have noted some deficiencies, including one about resident rights notifications, one about not reporting suspected abuse or neglect, and one about missed care with daily living needs, plus a noted infection-related deficiency. Blue Palms is a for-profit, limited liability company, and is owned by Daytona Fl Holdings Company LLC. They have 100 certified beds and provide an average of 3.39 nurse hours per resident per day, with a nurse turnover rate of about 63.6 percent.

Families are encouraged to talk with doctors, review inspection records, and consider all financing choices like Medicare or Medicaid.
