    Good care but safety concerns

I had a mixed experience. The building and therapy were spotless, and many staff were kind, responsive, and went above and beyond - rehab and personalized care were excellent and leadership often communicated well. That said, I saw short-staffing, inconsistent communication, and a few serious safety/transfer lapses, so I'd recommend the place with caution and close family oversight.

    Summary review

Overall impression: Reviews for Chautauqua Springs Health Center are highly polarized, with a large number of families and former residents offering strong praise for specific staff, rehab services, and aspects of the facility, while a significant minority report serious safety, neglect, and management concerns. The dominant theme is variability: many reviews describe excellent, compassionate frontline care and outstanding therapy services, while others recount troubling incidents of neglect, delayed emergency responses, and administrative failures. Prospective families should expect that individual experiences can differ markedly depending on time, unit, and personnel on duty.

Care quality and clinical services: Rehabilitation (physical and occupational therapy) and certain clinical teams (dialysis, some nursing units) receive consistently strong positive feedback. Many reviewers credit the rehab department with meaningful functional improvement and timely, focused therapy that led to successful discharges home. Conversely, there are multiple, specific reports of substandard clinical care: missed baths and basic hygiene, unadministered breathing treatments, medications left unattended, oxygen/stethoscope/equipment not available during a crisis, and delays in transferring residents to higher levels of care. Several reviewers described situations that led to emergency department visits or ICU admissions; these accounts emphasize delays of 60–90 minutes or more before appropriate action was taken. These conflicting reports indicate pockets of very good clinical practice coexisting with episodes of dangerous lapses.

Staff and culture: Many reviews highlight individual staff members and teams who are compassionate, respectful, and go above and beyond (names such as Kizzy, Emily, Penny, and reference to staff that felt like family appear repeatedly). These comments point to caring front-line staff, engaged therapists, and some accessible administrators who provide timely updates and personal attention. At the same time, there are numerous allegations of rude or neglectful staff behavior, staff abuse, and professional misconduct. Short staffing and use of agency or rotating personnel are repeatedly cited as contributors to inconsistent care and slower response times. Some reviews claim poor staff morale and punitive management responses (including an allegation of an employee terminated for having COVID), which, if true, could exacerbate the operational variability reflected in these reviews.

Management, communication, and record-keeping: Communication is a split theme—some families report thoughtful, frequent updates from nurses and administration, while others report poor communication, withheld medical records, and unhelpful or “invisible” medical leadership (reports of an invisible doctor, unhelpful nurse practitioner, or useless director of nursing appear). A few reviews specifically mention that medical records were held by the legal department or refused to be sent, creating major frustration. There are also mentions of state investigations and allegations of unsafe workplace practices; however, reviews do not provide comprehensive outcomes for these investigations. This mix suggests the facility’s administrative performance is inconsistent and may depend on which managers are involved or the timing of stays.

Facilities, cleanliness, and dining: Facility descriptions are also mixed. Many reviewers praise cleanliness, a well-maintained interior, attractive common areas (courtyard, cafeteria, social lobby), and effective housekeeping. Conversely, there are some strong negative reports describing exterior filth, black mold in rooms/showers, overflowing trash, and persistent odors (urine and heavy disinfectant). Dining receives both praise and criticism—some call out good dining options and accommodating staff, while others complain about meal timing (breakfast served late), cold food, and poor food quality. These divergent accounts further underscore the variability: certain units and times appear well-run, while others fall short of expected standards.

Safety incidents and concerning patterns: Several reviews recount alarmingly delayed emergency responses and specific clinical omissions that resulted in ER transfers or intubation en route. Reports include lack of response to call lights, unavailability of basic respiratory equipment during distress, and decisions to administer treatments in-house instead of transferring when transfer may have been more appropriate. There are also repeated claims about medications running out, missed treatments, and locked doors limiting resident movement. Taken together, these are significant red flags that merit careful scrutiny by families considering placement, and they point to the need for clear emergency and escalation protocols at the facility.

Notable strengths and staff recognition: Despite the concerning accounts, the facility receives repeated, heartfelt praise from many families who credit specific teams and staff for excellent, empathetic care that improved residents’ function and well-being. Positive aspects most consistently commended are excellent rehab outcomes, compassionate nursing aides, helpful administrative points of contact, timely updates, and a community feel. Several reviewers recommended the facility highly and described it as a cornerstone of local care.

Summary assessment and practical advice for families: The overall pattern is one of extremes—Chautauqua Springs Health Center appears capable of delivering very high-quality, person-centered rehab and compassionate caregiving in many cases, but there are also credible reports of neglect, management problems, and safety lapses. If you are evaluating this facility, visit in person, meet unit staff and administrators, ask about staffing ratios and use of agency staff, review emergency and transfer protocols, verify how medications and oxygen are managed, and request written policies on record access and family communication. Speak with families of current residents, ask for recent survey/investigation outcomes from the state, and get direct contact names (nurses, unit manager, administrator) to monitor care continuity. The mixed reviews suggest the experience can be excellent under the right conditions but that vigilance and clear communication will be important to ensure consistent, safe care.
