    Spotless facility but understaffed, unsafe

I found the facility spotless - clean dining area, tidy one-bedroom units and several friendly, attentive employees who genuinely care. However chronic understaffing (nights with one aide for many residents), untrained aides, medication errors, poor incident reporting and frequent falls have led to fractures, bedsores and hygiene neglect. Management and doctors were often unresponsive and the menu/food was inadequate. I appreciate the cleanliness and a few great caregivers, but I'd use extreme caution and oversight before placing a loved one here.

    Summary of reviews

The reviews present a mixed picture of Professional Home Care III, Inc. Strengths cited include generally clean interior spaces, an orderly dining area, and tidy, organized one-bedroom units with ample closet space. Several reviewers also noted friendly interactions with staff and the presence of at least one consistently attentive employee, which some families found reassuring.

Care quality concerns are prominent and recurring. Reviewers described patterns consistent with inadequate supervision and clinical oversight, including medication errors, delayed or incomplete incident documentation, and care gaps that contributed to pressure injuries and falls. These descriptions suggest weaknesses in medication-management procedures, incident-reporting workflows, and fall-prevention practices that could increase resident risk if not addressed.

Staffing and workforce issues were a central theme. Accounts point to chronic understaffing, especially on night shifts, resulting in high resident-to-staff ratios. Reviewers linked low staffing levels to delayed personal care, inconsistent hygiene support, and missed or irregular assistance. Several comments also raised concerns about limited staff training and low compensation, which reviewers associated with turnover and uneven care competence.

Dining and nutrition were described inconsistently. Several reviews characterized the facility’s food as low quality or nutritionally inadequate; specific items were cited as examples of limited offerings. At the same time, the dining area itself was noted as clean and orderly. These observations indicate a separation between dining-room upkeep and meal-planning or kitchen-quality issues.

Facility maintenance feedback was mixed. While many interior spaces and resident units were described as clean and organized, reviewers also noted maintenance problems such as leaks and delayed repairs. This suggests that routine housekeeping may be functioning reasonably well, but larger maintenance-response processes may be slower or inconsistent.

Management and clinical coordination drew criticism for perceived unresponsiveness. Reviewers described limited administrative engagement, low supervision, and difficulties obtaining timely physician input or care coordination. There are also mentions of concerns about staff conduct and, in some reviews, allegations of inappropriate staff behavior; these raise questions about oversight, complaint investigation, and staff accountability.

Activities and engagement were not a prominent theme in the feedback received; reviewers did not emphasize robust programming or resident engagement offerings. Prospective families should therefore ask the facility directly about activity schedules and individualized engagement plans.

Taken together, the pattern in the reviews points to operational weaknesses—notably staffing levels, training, medication and incident controls, and maintenance responsiveness—that affect care consistency and resident safety. At the same time, there are tangible positives in cleanliness of common areas and unit organization, and evidence of individual staff members providing attentive care. Families considering this provider should arrange focused conversations about staffing ratios (including night coverage), staff training and turnover, medication and incident-report policies, nutrition planning, maintenance response times, and the facility’s complaint and oversight processes before making a placement decision.
