    Moved mom pleased with care

I moved my mom here and I'm very pleased. Management is responsive, nurses and CNAs are attentive and compassionate-she's thriving and treated like family in a clean, sanitary facility with outdoor access and decent food; I highly recommend it and hope they keep improving.

    Summary of reviews

Reviews of The Pavilion at Jacksonville present a mixed picture with distinct strengths and recurring operational weaknesses. Several families describe compassionate, attentive nursing and CNA care and note meaningful improvements in housekeeping and medication management. Some reviewers characterize the facility as safe and family-like, with residents responding positively to the personal attention and outdoor access.

Care and staffing show a divided pattern. Positive comments describe nurses and CNAs who are attentive and supportive, and a number of families reported improvements in medication administration and daily care. Conversely, other reviewers raised concerns about staff conduct and communication tone, as well as delays or variability in responsiveness to resident needs. These accounts indicate inconsistent staff professionalism and intermittent communication problems between families and leadership.

Dining and clinical oversight are described inconsistently. Food quality was mentioned as a factor in families' impressions, with some indicating acceptable meals and others citing variability. Medication and clinical practices were reported to have improved by some families, while others continue to perceive gaps in clinical oversight that warrant clarification during a tour or intake conversation.

The physical environment emerged as a frequent concern. Multiple accounts describe an older, dated interior with maintenance shortcomings โ€” worn furniture, loose or exposed cables, and seating in need of repair โ€” alongside a small activity room and generally dim common areas. At the same time, access to outdoor seating and fresh air was noted as a positive feature. Cleanliness was described as improved compared with earlier conditions, but historical sanitation concerns remain part of the facility's reputational context.

Management and administrative consistency is a notable pattern. Some reviewers praised proactive management and an engaged administrative team; others described slow or unhelpful administrative responses and communication breakdowns, including a complaint filed with the Better Business Bureau. This split suggests variability in leadership responsiveness and issue resolution.

For prospective residents and families: conduct an in-person visit focused on current staffing levels and shift coverage, observe staffโ€“resident interactions, inspect resident rooms and common areas (including lighting and outdoor access), review housekeeping and maintenance schedules, ask for recent survey or inspection results, and request details on medication administration protocols and dining menus. Given the range of experiences, verifying recent improvements and obtaining references from current resident families may provide a clearer sense of the facility's current operational reliability.

    About The Pavilion at Jacksonville

The Pavilion at Jacksonville, located at 1771 Edgewood Ave W in Jacksonville, Florida, is a for-profit nursing home that's operated by The Pavilion Group and registered as a Foreign Limited Liability Company in Delaware under the name THE PAVILION AT JACKSONVILLE PROPERTY, LLC. The facility offers 60 certified beds for about 58 residents on average each day, caring for seniors who need different types of support, from short-term rehab to long-term stays, and has services for assisted living, independent living, memory care, skilled nursing, and adult care home options handled under one roof, which helps families looking for more than one kind of care in a familiar place. With Pavilion Holdings LLC holding all direct ownership and several indirect owners like 2020 Northeast Holdings Trust and FTK Capital LLC, the management keeps its administrative work up to date, filing annual reports for 2023, 2024, and 2025, and keeping documents like annual and inspection reports available in PDF form.

The Pavilion features a recreational library, a community garden, walking paths, outdoor spaces, an arts room, fitness room, spa/wellness space, and a movie theater, plus a music program, movie nights, and resident-run activities, all intended to make daily life comfortable, active, and social for folks living there. The range of housekeeping and laundry services, along with concierge help, makes it easier for residents to focus on enjoying their day or attending activities, while scheduled transportation, parking, and shuttle services can get residents out to appointments or events as needed.

You'll find private rooms with bathrooms, kitchenettes, telephones, cable TV, air conditioning, and Wi-Fi, plus help with move-in and furnishings if needed, which makes for a more home-like setting. Residents have access to chef-prepared meals served in a dining room, including options for diabetes and other dietary needs, served with restaurant-style and all-day dining choices, though there have been reported issues about food palatability and attractiveness. Special staff are available for help with bathing, dressing, medication management, and transfers, and there's always supervision on hand for safety, with an emergency alert system in place. Certified nursing care is provided for frailer residents who need extra support, with a nurse staffing level averaging just over three hours per resident each day, though the nurse turnover rate's a bit high at just above fifty percent, which might mean you see different faces over time.

On-site health care includes dentistry, podiatry, optometry, audiology, and help with home health and hospice support, with rehabilitation programs and therapies focused on getting residents back on their feet. The facility has amenities and programs designed to support people with memory concerns or cognitive struggles. There's always a focus on creating a positive and supportive environment, with individualized care plans aimed to enhance well-being and promote satisfaction, and staff respond to questions within 24 hours. Medicaid and Medicare are both accepted for payment support.

Inspection records, including a recent May 2024 report, do show a few deficiencies- with past notes about meal quality, care planning, and communication - and it's best to look at these reports to know the whole picture. Still, the facility tries to keep residents' support systems steady and tailored to individual needs, offering flexibility with private and community amenities, and working to provide attentive and supportive care for every resident.
