    Compassionate staff and excellent rehab

I stayed eight weeks at Chatsworth and felt truly at home - the facility is beautiful, spotless, and welcoming (warm lobby, garden seating and even a friendly dog). The nursing, CNAs and rehab team (PT/OT) were compassionate, professional and consistent; Jennifer, Kris and Michelle went above and beyond and arranged personalized therapy that helped my recovery. I highly recommend Chatsworth for rehabilitation, assisted living or memory care.

    Summary of reviews

Chatsworth Assisted Living Community at PGA National elicits a mix of strong praise and notable operational concerns. The facility is frequently described as physically attractive, clean, and welcoming: reviewers mention a warm lobby, private cheerful rooms, garden seating, a small library, and pet-friendly touches that contribute to a homelike environment. The inpatient rehabilitation program and therapy teams (PT/OT) receive consistent commendation for helping residents regain mobility and independence, and many families singled out individual therapists and nursing staff for compassionate, effective care.

Clinical care is an area of mixed performance. While many reviewers praise attentive nurses and rehabilitative outcomes, there are recurrent concerns about medication management and clinical oversight for complex conditions — most prominently diabetes management, with examples of missed or delayed medication and dietary non-adherence that in at least one instance led to hospital readmission. Reviewers also cited unreliable nurse-call responsiveness and delays attending to resident needs. These items point to gaps in medication-administration controls, escalation pathways, and staffing during critical periods.

Staff behavior and availability vary across accounts. Numerous comments describe caring, professional aides, nurses, and administrative leaders who go above and beyond; others describe brisk, impatient, or unprofessional interactions and long waits for assistance. This variability extends to housekeeping and dining: many reviews praise good food and accommodating meal service, while others describe inconsistent meal quality and failures to follow dietary requests. Therapy quality is generally seen as a strength, though a few reviewers noted variability in leadership or supervisory approach within the therapy department.

Management and administrative practices show both positives and concerns. Several families appreciated accessible directors and staff who arranged transportation and offered clear guidance. Conversely, there are complaints about billing or length-of-stay practices, a cited Medicare complaint, and at least one allegation of an unjustified attempt to extend a stay. Infection-control concerns were also raised in connection with a COVID-19 exposure during a stay.

For prospective residents and families: Chatsworth offers a strong rehabilitation program, attractive accommodations, and many compassionate caregivers, making it a reasonable option for rehab, assisted living, or memory-care placement—provided that facility-level variability is addressed to your satisfaction. When evaluating the community, ask specific questions about medication-administration protocols (especially for insulin-dependent residents), nurse-call response times, staffing ratios at night, dietary accommodation procedures, infection-control practices, and billing/length-of-stay policies. A focused tour that includes observation of mealtime, a review of current staffing patterns, and direct conversations with therapy and nursing leadership will help clarify whether the facility’s strengths align with your relative’s needs.

    About Chatsworth at PGA National

Chatsworth Assisted Living Community at PGA National sits in Palm Beach Gardens and offers several types of care under one roof, so at this place you'll find assisted living, independent living, memory care, continuing care retirement community options, and even a skilled nursing facility and short-term rehabilitation, and they focus on keeping the environment both comfortable and safe for their residents, especially those dealing with Alzheimer's or other forms of dementia. They've got a capacity for 48 residents and have served seniors since 1999, and as part of the Erickson Senior Living network, they bring over 30 years of experience caring for older adults. Residents can get help with daily needs like dressing, medication, grooming, and meals, and there are personalized care plans made by an interdisciplinary team including on-site doctors, skilled nurses, and geriatric nurse practitioners who are available around the clock. Each private room has a full bath, kitchenette, good closet space, big windows with lots of light, and some even have patios, which is helpful for both independence and comfort.

Inside and outside, the place offers a good list of features-gardens, walking paths, an enclosed courtyard with a fountain, community living rooms, sunlit dining areas, a barber and beauty salon, a gift shop, bird-watching spots, and organized resident outings like beach trips and baseball games, so folks have choices for how to spend their time. They also run plenty of activities-movie nights, trivia, arts and crafts like painting or flower arranging, music therapy, gardening, spiritual services, happy hours, ice cream socials, and exercise classes-plus they organize social gatherings and programming tailored to what each resident likes, while making sure there are fitness and wellness options too. The dining program offers meal services with food that's generally acceptable, and they also manage daily bed-making and weekly laundry, so upkeep isn't a worry.

They keep everything fairly safe and accessible with an Emergency Alert System and staff around at all hours for both daily support and emergencies, and anyone needing short-term rehabilitation gets access to physical, speech, and occupational therapy right there on site, since the nursing home and skilled nursing care are part of the same location. Folks can try to stay as independent as possible, but extra help is always available if something changes, and people with increasing care needs can stay in place without having to relocate. The facility is close to places like the West Palm Beach VA Medical Center and Florida O & P Services, making outside medical visits more convenient.

Chatsworth has a state license and runs a supportive living environment with a strong memory care focus, and it has a steady rating of 3.9 from 22 reviews, which suggests most residents and families find the care dependable. It's clear they try to help seniors enjoy life with dignity, while still feeling part of a caring community.
