    Clean facility but inconsistent care

I had a mixed experience. The facility is clean, bright and well-maintained with a lovely courtyard, regular activities (music, ice cream social), good food, and an outstanding therapy team - many nurses and CNAs were kind, professional, and really helped my family member recover. However care was inconsistent: weekend understaffing, slow call-button response, delays in meds/feeding and hygiene, foul odors/dirty bathrooms reported, and spotty communication/billing - so I'd recommend visiting in person and asking specifically about staffing and cleanliness before deciding.

About Moultrie Creek Nursing and Rehab Center

Moultrie Creek Nursing and Rehab Center sits in St. Augustine, Florida, and provides care for seniors who need help, including those with memory problems like dementia, and the place also handles those folks coming in for short-term rehab after being in the hospital or who need ongoing skilled nursing around the clock. The center meets federal Medicare and Medicaid rules, so it's ready to serve all sorts of residents and helps with regular things like bathing, dressing, and eating, and you'll see staff trained to store and give out medications, which is important because some people need medicine day and night. There's nursing staff available 24 hours a day, and they've got Certified Nursing Assistants who are known to pay close attention and show care for each resident.

The facility can house up to 120 people and usually has about 110 folks staying there each day. Residents can choose between private or semi-private rooms, and these are set in a big building with landscaped grounds and a pleasant, open feel. They offer a full range of healthcare services, including short-term rehab with things like physical therapy, occupational therapy, and speech therapy in a special gym that residents can use seven days a week. Some people come for long-term stays when they can't live alone anymore and need ongoing support or help with serious health problems, while others only stay a week or two after leaving the hospital. They also have a post COVID-19 Recovery Program and telemedicine, so you can get help from doctors using fancy equipment even during evenings or weekends.

There's always someone on duty, with nurses present night and day and Certified Nursing Assistants giving hands-on care. They average about 3.5 nurse hours per resident each day, though they do have a fairly high nurse turnover rate at about 50%. Inspection reports mention the place has had some problems in the past, like not keeping medication error rates below 5% and not always watching out for hazards that could cause accidents, and inspectors have listed seven deficiencies, including one related to infections, so those facts are worth considering for anyone thinking about staying here.

Amenities and activities are there to help make daily life more interesting, with programs to help residents socialize and keep busy. Policies include mask rules and special guidelines for visitors, especially during COVID-19. People get daily attention, and admission can happen any day of the week, with an administrator helping sort everything out. Everything operates under Southern Healthcare Management LLC, which has run this place since 2003, and the center is part of Sovereign Healthcare Holdings. The environment feels caring and friendly, focused on helping everyone get better when possible or just feel comfortable if staying long-term, and the roomy layout with nice views does contribute to a simpler, more homelike life.
