Riverwood Healthcare and Rehabilitation Center sits in Starke, Florida, where it serves seniors and adults with a wide range of needs in a for-profit setting managed by Riverwood Nursing Snf Jv LLC and affiliated with the Sherman Family and other stakeholders, and the facility has seen new management since December 2024, with Jeffrey Goldstein and Aaron Presser stepping into key roles in July of the same year, so it's recently changed hands. The center's equipped with 120 certified beds and usually cares for about 107 people each day, offering several types of care such as Independent Living, Assisted Living, Memory Care, Home Care, Adult Day Services, Long-Term Care, Medicare-Certified Home Health Care, Hospice, and short-term Respite Care. Skilled nurses and rehabilitation staff work day and night to give comfort, medical attention, and extra help to people whether they're staying for a short while to recover or living there long-term, and there are different activity programs and amenities inviting residents to join in and feel right at home, with renovated common areas, welcoming outdoor spaces, and advanced equipment to help keep everybody safe and supported. Memory care is a focus for those who need extra help, and rehabilitation programs aim to get folks back on their feet when possible. The facility says it tailors care plans for each person, hoping to keep everyone as independent and comfortable as possible. Still, there are problems worth knowing, like five deficiencies cited in a June 2024 inspection, including one tied to infection control, and across recent inspections, there have been nineteen citations in total, with three relating to infection control, as well as issues with how drugs and biological materials are stored, and whether resident assessments are accurate; nurse turnover is pretty high at 52.3%, and nurses spend an average of 3.46 hours per resident per day, which is less than the Florida state average of 3.9. Even so, there's a sense that staff tries to treat residents like individuals, and they offer a range of programs focused on comfort, safety, and connection, making Riverwood a choice for those who want a broad set of senior living and healthcare options in the Starke area.

Compassionate caregivers, good therapy, issues
I had a mixed experience. The staff - nurses, CNAs, therapy and admissions - were warm, attentive and often went above and beyond; therapy and rehab were especially strong. Unfortunately cleanliness and food were inconsistent: a persistent urine smell in halls, spotty housekeeping, and meals sometimes cold or unfit to eat. I also saw understaffing and safety lapses (ignored call buttons, falls and hospital readmissions) that need urgent attention. Overall, compassionate caregivers and good therapy, but serious hygiene, food and safety issues must be fixed.
